Summary: This systematic review and meta-analysis compared the efficacy and safety of tenecteplase with alteplase for the treatment of acute ischemic stroke (AIS) patients. However, patients receiving tenecteplase might have a higher incidence of early neurologic improvement compared to those receiving alteplase.

Summary: This descriptive research analyzed the vascular territory and treatment outcomes in Iranian intravenous thrombolysis ischemic stroke patients based on data from the SITS registry. The results showed no significant association between vascular territory and mortality, functional independence, or brain hemorrhage occurrence. Age, diabetes, NIHSS score, and admission duration were identified as important predictors of three-month mortality, excellent outcome, and functional independence.

Summary: The burden of strokes is increasing, and current therapeutic drugs are not ideal. Staphylokinase has been identified as a potential thrombolytic agent that is efficient, fibrin-selective, inexpensive, and can evolve. Recent findings have shown that a nonimmunogenic variant of staphylokinase is comparable to alteplase, with a reduced risk of intracranial hemorrhage and the advantage of single bolus administration. Furthermore, detailed analysis has revealed a bottleneck in staphylokinase's activity that, if eliminated, could provide up to 1000-fold higher activity than alteplase. These limitations provide new possibilities for improvement by protein engineers.

Summary: This study evaluated the efficacy and safety outcomes of intravenous tenecteplase (TNK) compared with intravenous alteplase (ALT) for acute ischemic stroke (AIS) patients. The meta-analysis showed no significant differences between TNK and ALT in terms of functional outcome, mortality, and safety. Therefore, TNK may be a reasonable alternative to ALT for thrombolysis in AIS patients.

Summary: This paper evaluates the efficacy and safety of urokinase, ateplase, tenecteplase, and reteplase for intravenous treatment of acute ischemic stroke (AIS) through computational simulations. The results show that urokinase has the quickest lysis completion but the highest risk of intracranial hemorrhage (ICH) due to excess fibrinogen depletion. Tenecteplase and alteplase have similar thrombolysis efficacy, but tenecteplase has a lower risk of ICH and better resistance to PAI-1. Reteplase has the slowest fibrinolysis rate, but systemic plasma fibrinogen concentration is unaffected during thrombolysis.

Summary: The COVID-19 pandemic had a significant impact on stroke admissions, IV thrombolysis (IVT), and mechanical thrombectomy volumes, with a decline observed during the first year. However, mechanical thrombectomy volumes remained stable in high-volume centers. These findings suggest a preservation of stroke care for higher severity of disease during the first year of the pandemic.

Summary: The study aimed to assess the relationship between the effect of endovascular thrombectomy (EVT) on the NIHSS scores and the infarct volume. The results showed that EVT can reduce infarct volume and lower NIHSS scores. The reduction in infarct volume explained one third of the treatment benefit of EVT in terms of neurological deficit.

Summary: This study compared the outcomes of endovascular treatment (EVT) for acute ischemic stroke (AIS) between level 1 and level 2 intervention centers, and assessed whether differences in outcomes could be explained by center volume (CV). The results showed that there were no significant differences in the modified Rankin scale (mRS), NIHSS score, procedure time, and door-to-groin time between the two center types, except for the recanalization rate.

Summary: The imaging modality used for selecting patients for endovascular thrombectomy (EVT) in the late window varies among clinicians with different specialty training. In an international survey, interventionists were more likely to proceed directly to EVT and prefer less advanced imaging techniques compared to non-interventionists. When faced with uncertainty, non-interventionists were more likely to follow clinical guidelines, while interventionists relied more on their assessment of evidence.

Summary: This study aimed to evaluate the validity of early outcomes as quality indicators for acute ischemic stroke patients undergoing endovascular therapy. The results showed significant variation in both early outcomes and case mix characteristics between hospitals. The variation in NIHSS score at 24 to 48 hours was influenced by case-mix adjustment, while the variation in expanded thrombolysis in cerebral infarction score was strongly influenced by EVT-patient volume. Both outcomes may be suitable for comparing hospitals on quality of care, with proper case-mix adjustment for NIHSS score.

Summary: SMRs of acute regionally organized diseases not only measure differences in quality of care between hospitals, but primarily measure differences in case-mix between hospitals. Adding proxies of disease severity improves the model to calculate SMRs, but real disease severity scores would be preferred. However, such scores are not available in administrative data, making the usefulness of the current SMR as a quality indicator very limited.

Summary: This study aimed to explore the values, needs, and preferences of stroke patients throughout the continuum of care. Through qualitative interviews with 36 patients who had experienced ischemic stroke within the past 18 months, it was found that patients valued a compassionate professional who is responsive to their needs. They also expressed a need for tailored health information, psychological support, pro-active follow-up care, and improved coordination of care. It is recommended for professionals to adopt a value-based care approach to meet the individual needs of patients in terms of information, communication, and follow-up care.

Summary: The purpose of this study was to investigate the influence of admission systolic blood pressure on functional outcome and the effect of endovascular thrombectomy. The study found that high admission systolic blood pressure was associated with worse functional outcome after stroke, but did not seem to affect the effect of endovascular thrombectomy.
