Increased Benefit of Alteplase in Patients with Ischemic Stroke and a High Body Temperature

Background: In observational studies, a high body temperature has been associated with unfavorable outcome. In in vitro studies, the fibrinolytic activity of alteplase decreased 5% per degree Celsius reduction in temperature. The modifying effect of body temperature on treatment with alteplase in patients with acute ischemic stroke is unclear. We assessed the influence of baseline body temperature on the effect of alteplase on functional outcome in patients with acute ischemic stroke, included in the Paracetamol (Acetaminophen) in Stroke (PAIS) trial. Methods: PAIS was a randomized, double-blind clinical trial to assess the effect of high-dose paracetamol on functional outcome in patients with acute stroke. For this study, we selected all patients with ischemic stroke and randomization within 6 h of symptom onset. We estimated the effect of treatment with alteplase on the modified Rankin Scale score at 3 months with ordinal logistic regression, stratified by baseline body temperature. We made adjustments for confounding factors and expressed associations as adjusted odds ratios (aOR) with 95% confidence intervals (CI). We also tested for interaction between treatment with alteplase and body temperature. Results: We included 647 of the 1,400 patients in PAIS in our study. Treatment with alteplase was associated with improved functional outcome at 3 months (aOR 1.51, 95% CI 1.09-2.08). In the 286 patients (44%) with a baseline body temperature of 37.0 degrees C or higher, alteplase was associated with a larger effect (aOR 2.13, 95% CI 1.28-3.45) than in patients with a temperature below 37.0 degrees C (aOR 1.11, 95% CI 0.71-1.69). A test for interaction between body temperature and alteplase did not reach statistical significance (p = 0.18). Conclusion: Patients with ischemic stroke and a high body temperature may have a larger benefit of treatment with alteplase than patients with lower body temperatures. These findings are in line with those from in vitro studies, in which lowering temperature decreased the fibrinolytic activity of the enzyme alteplase. This interaction should be explored further in randomized clinical trials of thrombolytic therapy or modification of body temperature. Trials of therapeutic hypothermia should be controlled for treatment with thrombolytics, and trials of thrombolytic treatment should consider body temperature as a potential effect modifier. Summary: The COVID-19 pandemic had a significant impact on stroke admissions, IV thrombolysis (IVT), and mechanical thrombectomy volumes, with a decline observed during the first year. However, mechanical thrombectomy volumes remained stable in high-volume centers. These findings suggest a preservation of stroke care for higher severity of disease during the first year of the pandemic.

Summary: The study aimed to assess the relationship between the effect of endovascular thrombectomy (EVT) on the NIHSS scores and the infarct volume. The results showed that EVT can reduce infarct volume and lower NIHSS scores. The reduction in infarct volume explained one third of the treatment benefit of EVT in terms of neurological deficit.

Summary: This study compared the outcomes of endovascular treatment (EVT) for acute ischemic stroke (AIS) between level 1 and level 2 intervention centers, and assessed whether differences in outcomes could be explained by center volume (CV). The results showed that there were no significant differences in the modified Rankin scale (mRS), NIHSS score, procedure time, and door-to-groin time between the two center types, except for the recanalization rate.

Summary: The imaging modality used for selecting patients for endovascular thrombectomy (EVT) in the late window varies among clinicians with different specialty training. In an international survey, interventionists were more likely to proceed directly to EVT and prefer less advanced imaging techniques compared to non-interventionists. When faced with uncertainty, non-interventionists were more likely to follow clinical guidelines, while interventionists relied more on their assessment of evidence.

Summary: This study aimed to evaluate the validity of early outcomes as quality indicators for acute ischemic stroke patients undergoing endovascular therapy. The results showed significant variation in both early outcomes and case mix characteristics between hospitals. The variation in NIHSS score at 24 to 48 hours was influenced by case-mix adjustment, while the variation in expanded thrombolysis in cerebral infarction score was strongly influenced by EVT-patient volume. Both outcomes may be suitable for comparing hospitals on quality of care, with proper case-mix adjustment for NIHSS score.

Summary: SMRs of acute regionally organized diseases not only measure differences in quality of care between hospitals, but primarily measure differences in case-mix between hospitals. Adding proxies of disease severity improves the model to calculate SMRs, but real disease severity scores would be preferred. However, such scores are not available in administrative data, making the usefulness of the current SMR as a quality indicator very limited.

Summary: This study aimed to explore the values, needs, and preferences of stroke patients throughout the continuum of care. Through qualitative interviews with 36 patients who had experienced ischemic stroke within the past 18 months, it was found that patients valued a compassionate professional who is responsive to their needs. They also expressed a need for tailored health information, psychological support, pro-active follow-up care, and improved coordination of care. It is recommended for professionals to adopt a value-based care approach to meet the individual needs of patients in terms of information, communication, and follow-up care.

Summary: This study investigated the safety and technical efficacy of early minimally invasive endoscopy-guided surgery in patients with spontaneous supratentorial intracerebral hemorrhage (ICH). The results showed that this surgery, performed within 8 hours after symptom onset, is safe and effectively reduces ICH volume.

Summary: The purpose of this study was to investigate the influence of admission systolic blood pressure on functional outcome and the effect of endovascular thrombectomy. The study found that high admission systolic blood pressure was associated with worse functional outcome after stroke, but did not seem to affect the effect of endovascular thrombectomy.
