Background: Osmophobia is frequent in children with migraine (20-35%) but can also occur in up to 14% of cases with tension-type headache (TTH). So far, the prognostic role of this symptom in children with primary headaches has never been evaluated. Methods: A longitudinal prospective study was conducted on 90 young patients with TTH (37 with osmophobia, 53 without osmophobia). We evaluated whether osmophobia could predict the diagnosis transformation from TTH to migraine after a 3-year follow-up. Results and Discussion: In our cases the rate of diagnosis change was significantly greater in cases with osmophobia (62%) than in those without (23%). Osmophobia persisted at a 3-year follow-up in the majority of our cases (85%) and it was found to be one of the major predictors for the development of migraine; other predictors of evolution to migraine were phonophobia, a probable rather than certain diagnosis of TTH and olfactory triggers (p < 0.05). Conclusion: Our data confirm that osmophobia has an important diagnostic and prognostic role in children with primary headaches and should be systematically investigated at diagnosis and during follow-up.

European clinical guidelines for Tourette syndrome and other tic disorders-version 2.0. Part III: pharmacological treatment

Veit Roessner, Heike Eichele, Jeremy S. Stern, Liselotte Skov, Renata Rizzo, Nanette Mol Debes, Peter Nagy, Andrea E. Cavanna, Cristiano Termine, Christos Ganos, Alexander Muenchau, Natalia Szejko, Danielle Cath, Kirsten R. Mueller-Vahl, Cara Verdellen, Andreas Hartmann, Aribert Rothenberger, Pieter J. Hoekstra, Kerstin J. Plessen

Summary: The first European guidelines for Tourette Syndrome (TS) were published in 2011 by the European Society for the Study of Tourette Syndrome (ESSTS). The updated part on pharmacological treatment emphasizes the importance of psychoeducation and behavioral approaches as first-line treatments. Pharmacological treatment, particularly with dopamine blocking agents like aripiprazole, is indicated in some patients, especially when behavioral approaches are not effective or feasible. Treatment should be individualized based on the patient's needs, preferences, and co-existing conditions.

Summary: A cross-sectional study compared the effects of social distancing and lifestyle changes during the COVID-19 lockdown on children and adolescents with and without Neurodevelopmental Disorders (NDDs). Results showed that NDDs significantly impacted remote learning, sociality, scolding, and anxiety levels. There was agreement between parents and children regarding remote learning, lifestyle, and scolding. This study highlights the importance of supportive strategies for youth with NDDs during periods of lockdown.

The Pediatric Trochlear Migraine: Diagnostic and Therapeutic Implications

Vincenzo Raieli, Federica Reina, Daniela D'Agnano, Giovanna Martina Nocera, Mariarita Capizzi, Francesca Marchese, Vittorio Sciruicchio

Summary: Trochlear Migraine is characterized by strictly unilateral migraine and ipsilateral trochleodynia, with relief of migraine after successful treatment of trochleodynia. The term should be reserved for clinical migraine attacks strictly localized in the trochlear region.

Study on Atypical Migraine Auras in the Pediatric Age: The Role of Cortical Spreading Depression and the Physiopathogenetic Hypothesis Arising from Our Clinical Cases

Summary: Migraine is a complex neurologic disorder that affects multiple systems of the central nervous system. Keeping an aura diary can help in diagnosing and preventing under-diagnosis of rare migraine manifestations. Complex migraine manifestations require a thorough diagnostic work-up to exclude dangerous neurologic pathologies.

Cranial Autonomic Symptoms and Migraine: What Relationship and What Meaning? A Review

Summary: The presence of CAS in migraine, usually associated with trigeminal autonomic cephalalgias, has also been reported in adult and pediatric populations. Experimental evidence suggests that increased parasympathetic outflow can enhance the sensitization of nociceptive receptors involved in migraine. Identifying and understanding these symptoms in migraine may be crucial for diagnosis and effective management.

Investigating Visual Perception Impairments through Serious Games and Eye Tracking to Anticipate Handwriting Difficulties

Chiara Piazzalunga, Linda Greta Dui, Cristiano Termine, Marisa Bortolozzo, Matteo Matteucci, Simona Ferrante

Summary: Dysgraphia is a learning disability characterized by below-average handwriting production. This study examines the role of visual perception in handwriting skills through gamified visual perception tests played with an eye tracker. Machine learning models using game performance, eye-tracking, and drawing data as predictors successfully predicted children at risk of dysgraphia. These results suggest the potential of a new tool for early screening of dysgraphia based on visual perception skills.

Summary: This study investigated the clinical course and prognostic factors of pediatric migraine. The study found that familial neurological comorbidity and sleep disorders were unfavorable factors for predicting the long-term prognosis of pediatric migraine. However, patients with severe migraine pain admitted to the emergency department had a similar prognosis to those in outpatient samples.

A Narrative Review of Visual Hallucinations in Migraine and Epilepsy: Similarities and Differences in Children and Adolescents

Summary: Since the first descriptions of visual hallucinations in migraine and occipital lobe epilepsy, recognizing epileptic seizures of the occipital lobe has become a challenge due to the similarities with migraine symptoms. Detailed clinical examination of visual impairment can help differentiate the two diseases, especially in children where overlaps are common. Understanding the distinctive features of visual aura in migraine and visual hallucinations in occipital epilepsy can provide insights into the underlying mechanisms of these conditions.

Symptomatic, Alexithymic, and Suicidality-Related Features in Groups of Adolescent Self-Harmers: A Case-Control Study

Alessia Raffagnato, Marina Miscioscia, Rachele Fasolato, Sara Iannattone, Perla Valentini, Eleonora Sale, Michela Gatta

Summary: Non-suicidal self-injury (NSSI) is a growing problem among adolescent groups, both clinical and nonclinical, and is associated with various psychopathological symptoms. However, differences between clinical and nonclinical self-harmers in symptom dimensions, alexithymia, and self-harm-related variables have been understudied. This study investigated these differences by comparing an Italian sample of 63 self-harmers receiving mental health services (clinical group), 44 self-harmers not receiving mental health services (subclinical group), and 231 individuals without a history of NSSI (control group). Questionnaires were used to assess psychopathological symptoms, alexithymia, and NSSI-related variables. The results showed that the clinical and subclinical groups had more severe symptoms and alexithymic traits compared to the control group, with the clinical group showing higher NSSI frequency, disclosure, self-punishment as motive, and suicidal ideation compared to the subclinical group. These findings have implications for clinical practice and prevention efforts among adolescents.

Summary: A multicenter web-based survey was conducted among members of the Italian Society of Pediatric Gastroenterology, Hepatology and Nutrition (SIGENP) and Italian Society of Pediatric Neurology (SINP) to evaluate the diagnostic and therapeutic approaches to CVS in Italy. The survey revealed poor consensus among clinicians in the diagnosis and management of children with CVS, emphasizing the need for a national consensus guideline to standardize practice.
