Summary: This study reveals that there is an inverse relationship between the volume of primary, elective, and total PCI performed in hospitals and in-hospital mortality in patients with acute MI undergoing primary PCI. Additionally, a lower ratio of primary to total PCI volume is associated with higher in-hospital mortality, suggesting it as an indicator of quality of care for acute MI.

Summary: This case report presents the first reported case of surgery in a pregnant woman treated with dual antiplatelet therapy without discontinuation of ticagrelor. Despite insufficient platelet inhibition of ticagrelor and moderate inhibition of aspirin, a caesarean section was performed for the patient due to the need for emergency surgery, with local hemostatic measures to prevent excessive bleeding.

Summary: In this study, 400 patients with STEMI were analyzed to compare the 2-year clinical outcomes of BP-DES and DP-DES in PPCI. The results showed that BP-DES were associated with lower incidence of MACE and ST within 2 years of follow-up. Subgroup analysis revealed that BP-DES were also linked to lower cardiac deaths compared to DP-DES.

Summary: A retrospective cohort study was conducted to describe the trends and determinants of initial P2Y12 inhibitor choice and switching in patients undergoing PCI for ACS. It was found that the preference for prasugrel and ticagrelor as initial therapy has increased over time, while deescalation (switch to clopidogrel) has also become more common. Baseline bleeding risk was associated with initial treatment choice, but had limited association with deescalation.

Summary: This study aimed to investigate the treatment patterns, LDL-C levels and healthcare resource utilization of patients with hypercholesterolaemia or mixed dyslipidaemia at high or very high cardiovascular risk in the Netherlands in 2018. The findings revealed suboptimal treatment with the majority of patients receiving statin monotherapy and only a small percentage receiving combination therapy. Alternative treatment strategies are required to further lower cardiovascular risk.

Summary: Tailoring antiplatelet therapy based on CYP2C19 pharmacogenetic testing can improve cardiovascular outcomes and reduce healthcare costs. To evaluate the feasibility of CYP2C19-guided de-escalation of prasugrel/ticagrelor to clopidogrel, a proof-of-concept study was conducted in community pharmacies. The study found that community pharmacies are a suitable location for point-of-care PGx testing and the majority of patients and pharmacists view it as valuable.

Patients who did not survive had lower blood pressure, increased heart rate, blood lactate, and blood glucose levels. The presence of diabetes, multivessel coronary artery disease, and prior coronary events also increased the risk of mortality.

Summary: This study evaluated CCTA-based ESS values in coronary arteries without atherosclerosis and identified factors affecting ESS values. The findings showed that ESS was highest in the left anterior descending artery and lower in men compared to women, even after adjusting for lumen diameter. ESS values were also highest in small segments and had a correlation with the distance from the ostium.
