Background: Connective tissue growth factor (CTGF) has been implicated in the cardiac and kidney complications of type 2 diabetes, and the CTGF -945 G/C polymorphism is associated with susceptibility to systemic sclerosis, a disease characterised by tissue fibrosis. This study investigated the association of the CTGF -945 G/C promoter variant with cardiac complications (left ventricular (LV) hypertrophy (LVH), diastolic and systolic dysfunction) and chronic kidney disease (CKD) in type 2 diabetes. Methods: The CTGF -945 G/C polymorphism (rs6918698) was examined in 495 Caucasian subjects with type 2 diabetes. Cardiac structure and function were assessed by transthoracic echocardiography. Kidney function was assessed using estimated glomerular filtration rate (eGFR) and albuminuria, and CKD defined as the presence of kidney damage (decreased kidney function (eGFR <60 ml/min/1.73 m(2)) or albuminuria). Results: The mean age +/- SD of the cohort was 62 +/- 14 years, with a body mass index (BMI) of 31 +/- 6 kg/m(2) and median diabetes duration of 11 years [25th, 75th interquartile range; 5, 18]. An abnormal echocardiogram was present in 73% of subjects; of these, 8% had LVH alone, 74% had diastolic dysfunction and 18% had systolic +/- diastolic dysfunction. CKD was present in 42% of subjects. There were no significant associations between the CTGF -945 G/C polymorphism and echocardiographic parameters of LV mass or cardiac function, or kidney function both before and after adjustment for covariates of age, gender, BMI, blood pressure and hypertension. CTGF -945 genotypes were not associated with the cardiac complications of LVH, diastolic or systolic dysfunction, nor with CKD. Conclusions: In Caucasians with type 2 diabetes, genetic variation in the CTGF -945 G/C polymorphism is not associated with cardiac or kidney complications.

Retinal microvascular function predicts chronic kidney disease in patients with cardiovascular risk factors

James D. Theuerle, Ali H. Al-Fiadh, Edmond Wong, Sheila K. Patel, Gizem Ashraf, Thanh Nguyen, Tien Yin Wong, Francesco L. Ierino, Louise M. Burrell, Omar Farouque

Summary: Endothelial dysfunction is a precursor to atherosclerosis and is associated with the coexistence of CVD and CKD. This study found that retinal arteriolar endothelial dysfunction is present in CVD patients with early-stage CKD and serves as an indicator of long-term CKD progression in those with normal renal function.

Summary: The CLARITY trial is a randomized controlled trial investigating the effectiveness of angiotensin receptor blockers in COVID-19 patients. The trial design is Bayesian adaptive, with the primary outcome being clinical status assessed using a 7-point ordinal scale. The trial will follow the intention-to-treat principle in data analysis.

Summary: Joining a function-enhanced Fc-portion of human IgG to the SARS-CoV-2 entry receptor ACE2 produces an antiviral decoy with strain transcending virus neutralizing activity. Fc-modification was utilized to optimize SARS-CoV-2 neutralization and Fc-effector functions of ACE2-Fc decoy proteins. Different Fc-modifications had distinct effects on neutralization and effector functions.

The Need for Individualized Risk Assessment in Cardiovascular Disease

Hui Yin Lim, Louise M. Burrell, Rowena Brook, Harshal H. Nandurkar, Geoffrey Donnan, Prahlad Ho

Summary: Despite advancements in the field of cardiovascular disease, currently available clinical markers and tests are not sufficient to predict individual risk. A more precise and complex tool is needed to predict individual thrombosis and bleeding risks, enabling early interventions with a favorable risk-benefit profile. This review explores the fundamental concept of thrombosis and how personalized risk assessment can be improved using various tools.

Plasma Angiotensin Converting Enzyme 2 (ACE2) Activity in Healthy Controls and Patients with Cardiovascular Risk Factors and/or Disease

Hui Yin Lim, Sheila K. Patel, Ping Huang, Mark Tacey, Kay Weng Choy, Julie Wang, Geoffrey Donnan, Harshal H. Nandurkar, Prahlad Ho, Louise M. Burrell

Summary: The study found that plasma ACE2 activity is very low or undetectable in young healthy individuals and has minimal clinically relevant associations with coagulation function. However, patients with cardiovascular risk factors and/or disease have increased plasma ACE2 activity, suggesting that it may be an important biomarker for endothelial dysfunction and atherosclerosis.

Summary: Compared with norepinephrine, angiotensin II infusion can suppress renin release while maintaining aldosterone levels, providing a potential treatment for vasoplegia after cardiac surgery. Plasma renin concentration before surgery is associated with vasopressor requirement for norepinephrine but not angiotensin II.
