Stereotactic radiotherapy of intracranial benign tumours

Stereotactic radiotherapy can be delivered in one fraction or in multiple fractions schedule. It is used in benign tumours such as meningiomas, mainly localized in the base of the skull, for acoustic schwannoma and pituitary tumours. Whatever the tumour, results with the Gamma Knife are the most numerous, but those obtained by linear accelerators, adapted or dedicated, are comparable. The peripheral dose is preferred to the dose delivered to the isocentre. One fraction stereotactic irradiation should be proposed in small lesions and fractionated treatment for turnouts larger. Whatever the tumour, the results are satisfactory with a control rate of 90%. However, this value reflects a disparity assessment, radiological stability for meningiomas, radiological stability and preservation of useful hearing in schwannoma and radiological stability and a decrease in hormonal secretions for pituitary adenomas. Overall complication rates are low. In total, the treatment of benign lesions with stereotactic irradiation gives satisfactory results with few complications. Increased cardiac uptake of (18F)-fluorodeoxyglucose incidentally detected on positron emission tomography after left breast irradiation: How to interpret?

Summary: Radiation-induced heart disease is a complication that typically occurs years after thoracic irradiation. However, recent studies suggest that it can also be an earlier complication with subclinical cardiac injury. A case study described a patient who had abnormal cardiac metabolism detected incidentally after breast irradiation, but remained asymptomatic with normal angiography, indicating radiation-induced hibernating myocardium.

The Impact of an Incidental Dose on Axillary Tumor Control and Toxicity in Localized Breast Cancer: A Retrospective Analysis

Summary: This retrospective study evaluated the incidental axillary dose delivered during whole breast irradiation using different techniques, as well as the risk factors for axillary recurrence in breast cancer patients. The results showed that the irradiation technique affected the incidental dose delivered to the axillary area, but had no impact on the risk of axillary recurrence. The risk of lymphedema was found to be associated with the use of high tangential beams and the mean dose delivered at level II.

Summary: Stereotactic radiotherapy is a viable alternative for patients with localised stage non-small-cell lung carcinoma who are inoperable or refuse surgery. The microscopic tumour extension varies according to histological type, with different adapted margins needed for squamous cell carcinoma and adenocarcinoma. Multivariate analysis showed a significant relationship between maximum microextension distance, size, and shrinkage coefficient.

Efficacity of Deep Inspiration Breath Hold and Intensity-Modulated Radiotherapy in Preventing Perfusion Defect for Left Sided Breast Cancer (EDIPE): A Prospective Cohort Study Protocol

Summary: Breast radiotherapy can cause radiation-induced cardiac disease, especially in left breast cancers. A prospective single-center study will be conducted to explore alternative approaches, such as deep inspiration breath hold radiotherapy and intensity modulated radiation therapy, to reduce the risk of myocardial perfusion defects in patients with left breast cancer. The study will use stress and resting myocardial scintigraphy to assess myocardial perfusion and aims to demonstrate the effectiveness of reducing cardiac dose in preventing early and medium-term perfusion disorders.

Stereotactic Radiation Therapy of Single Brain Metastases: A Literature Review of Dosimetric Studies

Summary: Solid tumors often cause brain metastases (BMs), and stereotactic radiotherapy (SRT) plays a major role in their treatment. This systematic review summarizes the dosimetric studies of SRT for single BMs and highlights the challenges in comparing different studies due to variations in factors such as PTV margins and volumes, total dose, number of fractions, and isodose prescription. Optimal dosimetric parameters, such as conformity and gradient indexes, need to be considered to minimize radiation exposure to healthy tissue. Future well-designed studies are needed to establish guidelines for selecting the most suitable SRT technique based on the characteristics of treated BMs.

Patient-Derived Tumoroid for the Prediction of Radiotherapy and Chemotherapy Responses in Non-Small-Cell Lung Cancer

Summary: Radiation therapy and platinum-based chemotherapy are common treatments for lung cancer patients. Factors such as the patient's physical state and tumor heterogeneity contribute to the low overall survival rate in lung cancer. This study demonstrates that a patient-derived tumoroid model can be used to predict the efficacy of radiation therapy and cisplatin-based chemotherapy in non-small-cell lung cancer, and the results are correlated to clinical outcomes.
