A multicenter phase II study of belotecan, a new camptothecin analogue, in elderly patients with previously untreated, extensive-stage small cell lung cancer

Belotecan is a new camptothecin analogue and a potent topoisomerase I inhibitor. The aim of this phase II study was to investigate the efficacy and toxicity of belotecan in previously untreated elderly patients with small cell lung cancer (SCLC). A total of 26 patients, aged a parts per thousand yen65 years, with previously untreated, extensive-stage SCLC were enrolled in the study. Belotecan was administered by daily intravenous infusion at 0.5 mg/m(2)/day for 5 consecutive days every 3 weeks. The overall response rate and disease control rate of chemotherapy on an intention-to-treat basis were 35 and 54 %, respectively. The median overall survival was 6.4 months, and the median time to progression was 2.8 months. The most common toxicity was hematologic. Grade 3 or 4 neutropenia occurred in 80.8 % of patients, and grade 3 or 4 thrombocytopenia in 15.3 %. Non-hematologic toxic effects of grade 3 or 4 were uncommon. Belotecan had modest efficacy and well-tolerated toxicity in previously untreated, elderly SCLC patients. Single belotecan could be a promising treatment option, considering its lower toxicity in elderly patients who are unsuitable candidates for platinum plus etoposide chemotherapy.

Summary: A study of 13,254 adults in 1117 ICUs across 90 countries found a high prevalence of pressure injuries, with 59.2% being ICU-acquired. Risk factors for ICU-acquired pressure injuries included age, sex, weight, comorbidities, and organ support. Increasing severity of pressure injuries was associated with higher mortality rates, highlighting the importance of prevention and proper care in ICU settings.

Summary: The burden of nosocomial exposure to TB in South Korea is still substantial. Hospitalization with unrecognized active TB, especially among the elderly TB patients could be a serious public health issue in South Korea. Patients above 60 years of age accounted for 63.99% of the total number and 69.70% of the total duration of hospitalization, with those in their 80s showing the highest risk.

Summary: This study found that the use of fluoroquinolone was not associated with treatment effectiveness in patients with isoniazid-monoresistant tuberculosis. Both initial baseline regimens (6-9REZ and 2REZ/7-10RE) were similarly effective, while additional use of fluoroquinolone was beneficial in the 2REZ/7-10RE subgroup.

Non traumatic acquired acute transdiaphragmatic intercostal hernia induced by coughing

Summary: Transdiaphragmatic intercostal hernia is a rare condition that is usually caused by trauma but rarely caused by coughing. We report a rare case of a non traumatic acquired acute transdiaphragmatic intercostal and abdominal hernia induced by coughing. The patient had risk factors for intercostal hernia and computed tomography showed herniation of the lung and intra-abdominal organs through a ruptured diaphragm. Surgery was successful in closing the defects with interrupted sutures.

Clinical significance of normalized airflow obstruction in patients with chronic obstructive pulmonary disease

Summary: This study compared the clinical features and prognosis of patients with FEV1/FVC<0.70 at baseline, based on the normalized airflow obstruction status during follow-up. The results showed that transient airflow obstruction in the normalized obstruction group may represent an early manifestation of COPD. Monitoring is needed for such patients to ensure early detection and management.
