Summary: Lung cancer is the leading cause of cancer-related deaths worldwide, with non-small cell lung cancer (NSCLC) accounting for 80% of cases. Immune checkpoint inhibitors (ICIs) have revolutionized the treatment of metastatic and early-stage NSCLC. However, the elderly face challenges in treatment due to comorbidities and reduced organ function. Despite less toxicity compared to chemotherapy, ICIs may be less effective in patients older than 75, possibly due to immunosenescence. This review explores the role of immunotherapy in elderly NSCLC patients, considering the biological aspects of immunosenescence.

This article discusses the potential mechanisms, cell of origin, and genomic alterations in both de novo and transformed SCLC, as well as treatment options including chemotherapy, radiotherapy, TKIs, immunotherapy, and antiangiogenic agents.

Summary: This study found that lung microbiota in patients with non-small cell lung cancer differed significantly from those with benign lung disease, with the genera Prevotella and Veillonella being the most strongly associated with non-small cell lung cancer. Veillonella was also found to significantly promote the progression of lung cancer in vivo. Metabolic prediction revealed enrichment of certain pathways that may be involved in the progression of non-small cell lung cancer. These findings suggest that the composition and function of lung microbiota undergo significant changes during the progression of non-small cell lung cancer, and these differing genera may have potential as diagnostic markers and therapeutic targets.

Results showed that sublobectomy was associated with shorter operation time, but 5-year overall survival and disease-free survival did not significantly differ between the two groups. Cox regression analyses indicated that consolidation tumor ratio, visceral pleural infiltration, and smoking were independent risk factors for worse overall survival.

Summary: This retrospective study evaluated the impact of age on the therapeutic efficacy of chemotherapy combined with immune checkpoint inhibitors in elderly patients with NSCLC. The results showed that the pemetrexed regimen was associated with poorer clinical outcomes in elderly patients, while there were no significant differences in outcomes between elderly and non-elderly patients in the paclitaxel regimen group. Factors such as PD-L1 tumor proportion score, performance status, and albumin level were found to be significantly associated with progression-free and overall survival in elderly patients with NSCLC.

Summary: A study of 13,254 adults in 1117 ICUs across 90 countries found a high prevalence of pressure injuries, with 59.2% being ICU-acquired. Risk factors for ICU-acquired pressure injuries included age, sex, weight, comorbidities, and organ support. Increasing severity of pressure injuries was associated with higher mortality rates, highlighting the importance of prevention and proper care in ICU settings.

Hospitalization with unrecognized active TB, especially among the elderly TB patients could be a serious public health issue in South Korea. Patients above 60 years of age accounted for 63.99% of the total number and 69.70% of the total duration of hospitalization, with those in their 80s showing the highest risk.

Summary: Transdiaphragmatic intercostal hernia is a rare condition that is usually caused by trauma but rarely caused by coughing. We report a rare case of a non traumatic acquired acute transdiaphragmatic intercostal and abdominal hernia induced by coughing. The patient had risk factors for intercostal hernia and computed tomography showed herniation of the lung and intra-abdominal organs through a ruptured diaphragm. Surgery was successful in closing the defects with interrupted sutures.

Monitoring is needed for such patients to ensure early detection and management.
