Which Are the Optimal Criteria for the Diagnosis of Allergic Bronchopulmonary Aspergillosis? A Latent Class Analysis

Puneet Saxena, Hansraj Choudhary, Valliappan Muthu, Inderpaul Singh Sehgal, Sahajal Dhooria, Kuruswamy Thurai Prasad, Mandeep Garg, Biman Saikia, Ashutosh Nath Aggarwal, Arunaloke Chakrabarti, Ritesh Agarwal

Summary: This study compared the diagnostic performance of various criteria for evaluating ABPA and found that optimal diagnostic performance could be achieved by modifying the existing ISHAM criteria, with increased cost. The ISHAM-ABPA Working Group criteria were only marginally better than the Patterson criteria in diagnosing ABPA among patients with asthma younger than 66 years.

Sensitization to A fumigatus in subjects with non-cystic fibrosis bronchiectasis

Inderpaul Singh Sehgal, Sahajal Dhooria, Kuruswamy Thurai Prasad, Valliappan Muthu, Ashutosh Nath Aggarwal, Amit Rawat, Arnab Pal, Amanjit Bal, Mandeep Garg, Arunaloke Chakrabarti, Ritesh Agarwal

Summary: This study evaluated the prevalence of Aspergillus sensitization and chronic infection in non-CF bronchiectasis patients, particularly in TB-related bronchiectasis. The results suggest that investigations for chronic Aspergillus infection should be routinely performed in non-CF bronchiectasis patients, especially in those with TB-related bronchiectasis.

Diagnostic accuracy of CT and radiographic findings for novel coronavirus 2019 pneumonia: Systematic review and meta-analysis

Summary: By evaluating 56 studies, it was found that the combined prevalence of ground glass opacities and consolidation on chest CT is the highest, while radiographic abnormalities are visible in half of the patients. CT has a higher diagnostic performance for COVID-19 pneumonia, but should not be used as the primary tool for diagnosis.

A randomised trial of prednisolone versus prednisolone and itraconazole in acute-stage allergic bronchopulmonary aspergillosis complicating asthma

Ritesh Agarwal, Valliappan Muthu, Inderpaul Singh Sehgal, Sahajal Dhooria, Kuruswamy Thurai Prasad, Mandeep Garg, Ashutosh Nath Aggarwal, Arunaloke Chakrabarti

Summary: This study compared the efficacy and safety of prednisolone-itraconazole combination therapy with prednisolone monotherapy for ABPA, showing a lower exacerbation rate at 12 months in the combination therapy group. However, there were no significant differences in the decline in serum total IgE and time to first ABPA exacerbation between the two groups.

The Integral Role of Radiology in the Diagnosis and Management of COVID-19-Associated Mucormycosis Infections

Anuj Prabhakar, Nidhi Prabhakar, Mandeep Garg, Ajay Kumar

Summary: There has been a sudden increase in COVID-19-associated mucormycosis (CAM) infections, with rhino-oculo-cerebral and pulmonary mucormycosis being the most common forms. Radiology plays a crucial role in the management of CAM, with CT and MRI used for diagnosis and evaluation of the extent of the disease. Chest CT scans are helpful in diagnosing pulmonary mucormycosis, and a high index of suspicion combined with appropriate imaging findings can lead to early diagnosis and timely treatment.

Valliappan Muthu, Inderpaul Singh Sehgal, Kuruswamy Thurai Prasad, Sahajal Dhooria, Mandeep Garg, Ashutosh N. Aggarwal, Arunaloke Chakrabarti, Ritesh Agarwal

Summary: The study found that elderly patients with ABPA have lower immunological severity, less extensive bronchiectasis, and fewer exacerbations compared to non-elderly subjects.

Summary: New challenges in imaging and management of COVID-19 pneumonia have emerged, especially in relation to superinfections and co-infections. Radiologists play a crucial role in suggesting the likely cause of secondary infections and identifying the organism.

Efficacy of 12-months oral itraconazole versus 6-months oral itraconazole to prevent relapses of chronic pulmonary aspergillosis: an open-label, randomised controlled trial in India

Inderpaul S. Sehgal, Sahajal Dhooria, Valliappan Muthu, Kuruswamy T. Prasad, Ashutosh N. Aggarwal, Arunaloke Chakrabarti, Hansraj Choudhary, Mandeep Garg, Ritesh Agarwal

Summary: This study compared the effects of oral itraconazole treatment for 6 months and 12 months on patients with chronic pulmonary aspergillosis. The results showed that oral itraconazole for 12 months significantly reduced the relapse rate within 2 years and prolonged the time to first relapse.

Radiation Exposure and Lifetime Attributable Risk of Cancer Incidence and Mortality from Low- and Standard-Dose CT Chest: Implications for COVID-19 Pneumonia Subjects

Mandeep Garg, Vahid Karami, Javad Moazen, Thomas Kwee, Ashu Seith Bhalla, Daryoush Shahbazi-Gahrouei, Yu-Hsuan Joni Shao

Summary: Since the COVID-19 outbreak, there has been a significant increase in CT scans for the detection of lung disease. However, CT scans expose patients to ionizing radiation, which can increase the risk of cancer. This review discusses the known health effects of radiation, the risk of radiation-induced cancer at CT scan doses, and the potential increase in cancer incidence from CT scans performed during the COVID-19 pandemic.

Ultra-Low Dose CT Chest in Acute COVID-19 Pneumonia: A Pilot Study from India

Mandeep Garg, Shritik Devkota, Nidhi Prabhakar, Uma Debi, Maninder Kaur, Inderpaul S. Sehgal, Sahajal Dhooria, Ashish Bhalla, Manavjit Singh Sandhu

Summary: The rapid increase in CT acquisitions during the COVID-19 pandemic raised concerns about radiation exposure and health risks. This study evaluated the utility of ultra-low dose CT (ULDCT) for acute COVID-19 pneumonia compared to standard-dose CT (SDCT). ULDCT showed comparable diagnostic quality with significantly reduced radiation dose, making it a good alternative for COVID-19 pneumonia evaluation.

Role of Diffusion-Weighted Magnetic Resonance Imaging for Characterization of Mediastinal Lymphadenopathy

Eniyavel Ramamoorthy, Mandeep Garg, Paramjeet Singh, Ashutosh N. N. Aggarwal, Nalini Gupta

Summary: This study assessed the diagnostic performance of diffusion-weighted magnetic resonance imaging (DW-MRI) in characterizing mediastinal lymph nodes, and compared its performance with morphological parameters. The results showed that apparent diffusion coefficient (ADC) was the strongest independent predictor of malignancy, and the addition of other parameters did not increase sensitivity and specificity.
