Visceral Adiposity and Left Ventricular Mass and Function in Patients With Aortic Stenosis: The PROGRESSA Study

Background: Recent studies have reported that obesity, metabolic syndrome, and diabetes are associated with left ventricular (LV) hypertrophy (LVH) and dysfunction in patients with aortic stenosis (AS). The purpose of this study was to examine the association between amount and distribution of body fat and LVH and systolic dysfunction in AS patients. Methods: One hundred twenty-four patients with AS were prospectively recruited in the PROGRESSA (Metabolic Determinants of the Progression of Aortic Stenosis) study and underwent Doppler echocardiography and computed tomography scan. Presence and severity of LVH was assessed according to LV mass indexed for height(2.7) and LV dysfunction according to global longitudinal strain (GLS). Computed tomography was used to quantify abdominal visceral (VAT) and subcutaneous (SAT) adipose tissue, and total adipose tissue (TAT). Results: Body mass index (BMI) correlated strongly with TAT (r = 0.85), moderately with VAT (r = 0.70), and SAT (r = 0.69), and weakly with the proportion of VAT (VAT/TAT ratio: r = 0.19). In univariate analysis, greater BMI, TAT, VAT, SAT, and VAT/TAT were associated with increased LV mass index and greater VAT and VAT/TAT ratio were associated with reduced GLS. Multivariate analysis revealed that larger BMI (P < 0.0001) and greater VAT/TAT ratio (P = 0.01) were independently associated with higher prevalence of LVH, and only the VAT/TAT ratio (P = 0.03) was independently associated with reduced GLS. Conclusions: The results of this study suggest that total and visceral adiposity are independently associated with LVH in patients with AS. Furthermore, impairment of LV systolic function does not appear to be influenced by total obesity but is rather related to excess visceral adiposity. These findings provide impetus for elaboration of interventional studies aiming at visceral adiposity in the AS population.

Summary: The use of transcatheter edge-to-edge mitral valve repair (TEER) has significantly increased in symptomatic patients with severe mitral regurgitation (MR) in recent years. Current guidelines recommend TEER as a viable option for patients with primary or chronic secondary severe MR and high or prohibitive surgical risk. However, anatomical and morphological limitations of the mitral valve have initially restricted the use of this mini-invasive technique. The introduction of the latest fourth generation (G4) MitraClip system has overcome these limitations and expanded the applications of TEER.

Summary: This study aimed to evaluate the impact of paravalvular leak (PVL) on outcomes after transcatheter aortic valve implantation (TAVI). The findings show that patients with any degree of PVL, even mild, have a higher risk of all-cause mortality, rehospitalization, and cardiovascular mortality following TAVI. These results support the implementation of strategies to prevent any degree of PVL during TAVI.

Summary: This study conducted a genome-wide meta-analysis and discovered 17 loci associated with calcific aortic stenosis. The integration of functional, expression, and cross-phenotype data revealed the mechanisms of this disease. Dyslipidemia, inflammation, calcification, and adiposity were identified as important factors in the etiology of calcific aortic stenosis.

Genetics and pathophysiology of mitral valve prolapse

Summary: Mitral valve prolapse (MVP) is a common condition affecting 2-3% of the general population, with a complication rate up to 10-15% per year. Complications include mitral regurgitation, heart failure, atrial fibrillation, ventricular arrhythmia, and cardiovascular death. MVP can be syndromic or non-syndromic, and the genetic defects associated with MVP are still being studied. Animal models are used to advance the understanding of MVP genetics and pathophysiology. Genetic counseling is important in managing patients with MVP.

Summary: The decision of intervention in AS patients with HF depends on the severity of AS and further examination may be required due to discordant measurements. The choice of intervention for severe AS patients with HF can be difficult due to their intermediate-to-high operative risk and should be discussed with the patient. There is no current recommendation for AVR in patients with moderate AS and HFrEF, but ongoing studies are examining its clinical significance.

However, larger AVA(CT) thresholds improve specificity.

Summary: This study aimed to assess the incremental prognostic value of N-terminal-pro-B-type natriuretic peptide (Nt-proBNP) for risk stratification in mixed aortic valve disease (MAVD) patients. The results showed that Nt-proBNP ratio was a powerful predictor of early and long-term mortality, even in patients with both non-severe AS/AR. Therefore, Nt-proBNP can be used as a strong prognostic factor for predicting mortality in this group of patients.
