The Use of Antiplatelet Therapy in the Outpatient Setting: Canadian Cardiovascular Society Guidelines Executive Summary

Antiplatelet agents are a cornerstone of therapy for patients with atherosclerotic vascular disease. There is presently a lack of comprehensive guidelines focusing on the use of antiplatelet drugs in patients currently manifesting or at elevated risk of cardiovascular disease. The Canadian Antiplatelet Therapy Guidelines Committee reviewed existing disease-based guidelines and subsequently published literature and used expert opinion and review to develop guidelines on the use of antiplatelet therapy in the outpatient setting. This Executive Summary provides an abbreviated version of the principal recommendations. Antiplatelet therapy appears to be generally underused, perhaps in part because of a lack of clear, evidence-based guidance. Here, we provide specific guidelines for secondary prevention in patients discharged from hospital after acute coronary syndromes, percutaneous coronary intervention, or coronary artery bypass grafting; patients with a history of transient cerebral ischemic events or strokes; and patients with peripheral arterial disease. Issues related to primary prevention are also addressed, in addition to special clinical contexts such as diabetes, heart failure, chronic kidney disease, pregnancy or lactation, and perioperative management. Recommendations are provided regarding pharmacologic interactions that may occur during combination therapy with warfarin, clopidogrel, and proton-pump inhibitors, or aspirin and nonsteroidal anti-inflammatory drugs, as well as for the management of bleeding complications. Summary: Meta-analysis of studies suggests that targeted intracoronary thrombolytic therapy as an adjunct to primary percutaneous coronary intervention may be safe and potentially effective in improving complete ST-segment resolution and reducing in-hospital major adverse cardiac events, with no significant difference in bleeding outcomes. Further validation in larger randomized controlled trials is needed.

International research has highlighted the importance of timely reperfusion therapy and different approaches such as fibrinolysis and primary PCI. Clinician leaders have developed comprehensive care systems to optimize patient outcomes.

Ticagrelor or clopidogrel dual antiplatelet therapy following a pharmacoinvasive strategy in ST-segment elevation myocardial infarction

Summary: This study compared outcomes in STEMI patients who continued on clopidogrel versus those who switched to ticagrelor after fibrinolysis treatment. The results showed that switching to ticagrelor was associated with a lower risk of recurrent ischemic events at 1-year compared to continuing clopidogrel, with no significant differences in major bleeding or intracranial hemorrhage. These findings support the idea of switching pharmacoinvasive treated STEMI patients to ticagrelor in clinical practice.

Summary: In patients with atrial fibrillation and acute coronary syndrome and/or percutaneous coronary intervention, prior oral anticoagulant use was associated with a reduced risk of ischemic events but not increased bleeding. The results of the AUGUSTUS trial can be applied to patients regardless of their prior anticoagulant treatment status.

Long-Term Outcomes for Patients With Acute Coronary Syndrome and Nonvalvular Atrial Fibrillation

Summary: Patients with a history of nonvalvular atrial fibrillation (NVAF) have worse short-term outcomes in acute coronary syndrome (ACS), but the long-term prognosis is unclear. This study found that patients with previous NVAF had a higher risk of adverse outcomes, particularly in STEMI and NSTEMI, compared to those without NVAF.
