This article summarizes an extensive review of South American experiences with primary health care since the Declaration of Alma-Ata. It aims to address the following specific questions: What are the enabling and constraining historical and structural conditions for primary health care policies and practices? How has health care reform supported or undermined primary health care? What evidence exists on the effectiveness of primary health care? What strategies are common to best practices? What evidence exists on the roles of citizen participation and intersectoral action? And finally, what are the policy lessons to be learned from these experiences? Narrative synthesis was used to identify and examine patterns in the data consistent with these questions. Conditions that were found to promote successful implementation of primary health care are outlined, together with features of effective primary health care systems that help create more equitable health services and health outcomes.

Perceptions of adolescent pregnancy in the rural context and the Colombian armed conflict: a qualitative approach based on social determination of health

Summary: Adolescent pregnancy in rural areas is a persisting health issue that lacks proper understanding, with limited qualitative studies addressing it as a complex social process. This research in Colombia explored the social determinants of adolescent pregnancy in the Amaime river basin, revealing issues such as social stigma, lack of sex education, and sociocultural influences on behaviors. The study contributes to sexual and reproductive health interventions by providing analytical insights into the social dimensions of the issue.

Perceived quality of life and meanings attributed to the experience of extreme maternal morbidity: a qualitative study

Summary: The study aims to understand the impact of experiencing extreme maternal morbidity (EMM) on the meanings of life and daily life reconstruction. Participants perceive EMM as a suffering that is worthwhile and construct meanings from each stage of the experience. Women's perception of quality of life depends on both the experience itself and social support, as well as the survival of the baby. The role of health services is critical in alleviating loneliness, worry, and stress by providing compassionate care.
