Summary: The aim of this study was to determine whether preoperative treatment of inflammatory bowel disease (IBD) with tumor necrosis factor inhibitors (TNFis) increases the risk of postoperative infectious complications. The results showed that preoperative TNFi exposure was not associated with postoperative infectious complications.

Summary: This study compared the long-term outcomes of primary ileocecal resection (ICR) and anti-tumor necrosis factor (TNF) therapy in patients with ileocecal Crohn's disease (CD). The results showed that the risk of the composite outcome was 33% lower with ICR compared to anti-TNF therapy. ICR was also associated with a reduced risk of systemic corticosteroid exposure and CD-related surgery.

This review article discusses the challenges faced by patients with Crohn's disease and the need for standardized approaches in clinical evaluation and treatment.

Summary: This study investigated the impact of inflammatory tissue characteristics on the response to anti-tumor necrosis factor (anti-TNF) treatment in Crohn's disease (CD). The results showed that the activation of fibroblasts might contribute to resistance to anti-TNF therapy. Characterizing fibroblasts in inflamed tissues at the time of diagnosis may help identify patients who are likely to respond to anti-TNF therapy.

While surgery was previously performed mainly for ileal/ileocaecal location and stricturing behavior, there is now an increasing complexity among patients at the time of surgery with a higher number of patients having abdominal perforating disease. To improve care for patients with penetrating abdominal CD, multidisciplinary team discussions are essential, along with proper surgical planning and evaluation of patients prior to surgery.

Summary: This study investigated the various factors that contribute to the development of postoperative complications in patients who undergo surgery for complicated Crohn's disease. The results showed that patient age, smoking status, steroid use, perianal disease, and presence of a stoma were significantly associated with the occurrence of complications. Close monitoring of patients with Crohn's disease who undergo surgery for the presence of complications is recommended.
