Feasibility of robotic pancreaticoduodenectomy

Background: Laparoscopic pancreaticoduodenectomy is feasible, but requires adaptations to established surgical techniques. The improved dexterity offered by robotic assistance provides the opportunity to see whether laparoscopic pancreaticoduodenectomy can be performed safely when faithfully reproducing the open operation. Methods: Patients were selected for robotic pancreaticoduodenectomy when generally suitable for laparoscopy. Obese patients were excluded, and those with pancreatic cancer were highly selected. A prospectively designed database was used for data collection and analysis. Results: Of 238 patients undergoing pancreaticoduodenectomy, 34 (14.3 per cent) were operated on robotically. No procedure was converted to conventional laparoscopy or open surgery, despite three patients requiring segmental resection of the superior mesenteric/portal vein and reconstruction. The mean duration of operation was 597 (range 420-960) min. The mean number of lymph nodes retrieved and analysed from patients with neoplasia was 32 (range 15-76). Four patients required blood transfusions and five developed postoperative complications exceeding Clavien-Dindo grade II. There were four grade B pancreatic fistulas. One patient died on postoperative day 40. Excess mean operative cost compared with open resection was (sic) 6193. Conclusion: Selected patients can safely undergo robotic pancreaticoduodenectomy. The main downsides are high costs and prolonged operating times compared with open resection.

Summary: Recent major events in pancreas transplantation (PT) include the COVID-19 pandemic and the first world consensus conference on PT. Studies following the conference have shown that PT alone is safe and effective, simultaneous pancreas and kidney (SPK) outcomes are excellent despite certain risk factors, and the use of HCV positive donors can lead to positive outcomes. The results of using sirolimus as primary immunosuppressant and costimulation blockade in SPK have not shown improvement.

Summary: This study demonstrates the feasibility of cold triangle robotic pancreaticoduodenectomy (C-Tr-RPD) for pancreatic cancer. The surgical procedure involves cold dissection and has been successfully performed in 127 cases. Although there were postoperative complications, C-Tr-RPD improves staging of PDAC compared to traditional surgery.

Summary: In this study, we provided a detailed description of the technical challenges associated with robotic pancreatoduodenectomy with vein resection (RPD-VR) and shared tips and tricks for a successful operation. Our results showed that RPD-VR can be effectively performed using different vascular techniques for different types of vein resection. We strongly recommend that RPD-VR should be performed by experienced surgeons at high volume centers.

Summary: This study aimed to define the prognostic value of Ca 125 in resected PDAC of the head of the pancreas. Data from PDAC patients who underwent pancreatic resection between 2010 and 2018 were analyzed, and it was found that Ca 125 can predict overall survival and correlates with tumor stage, lymph node status, and tumor grade in borderline resectable and locally advanced PDAC.

However, further refinement is needed in terms of perfusion protocols and organ viability assessment criteria. The European Society for Organ Transplantation (ESOT) has formed a working group to review literature and develop evidence-based guidelines for using MP in improving donor pancreas quality and quantity for transplantation.

Safety and safety protocols for living donor nephrectomy in Italy

Summary: This study investigated living donor kidney transplantation (LDKTx) activities in Italy and the safety measures implemented in living donor nephrectomy (LDN). The results showed no postoperative donor deaths and a 10-year donor survival rate of 93.38%. There were cases of renal disease in some donors, but the proportion was relatively low. The survey also revealed the presence of safety measures in some transplant centers, but improvements are needed.
