Summary: Gastro-oesophageal reflux disease after lung transplantation may contribute to chronic lung allograft dysfunction. Antireflux surgery can improve long-term allograft function and show a survival benefit. This study compares outcomes between medical and surgical management of reflux in lung recipients.

Summary: This study compared the advantages and disadvantages of robotic-assisted redo fundoplication and laparoscopic-assisted surgery in children. The results showed that the robotic-assisted group had shorter surgery time, less intraoperative blood loss, shorter length of hospital stay, and better treatment outcomes, with lower overall economic costs. Therefore, robotic-assisted redo fundoplication may be a better approach.

Summary: This study evaluated the efficacy of laparoscopic fundoplication in patients with chronic cough without asthma and pathologic gastro-oesophageal reflux. It found that 61% of the patients experienced relief in cough after the surgery, while predictors of cough response to fundoplication were not accurately predicted by typical GORD symptoms or pH study variables.

Summary: This systematic review investigates the effect of preoperative fasting duration and drugs on gastro-oesophageal reflux (GOR) in dogs and cats during anaesthesia. The study found that many factors seem to affect the development of GOR, but due to the limited number of studies and low level of evidence, no reliable conclusions can be drawn.

Summary: This study aimed to review the current management of gastro-oesophageal reflux disease (GORD), including treatment costs, safety profile, and effectiveness. It found widespread overprescribing of proton pump inhibitors (PPIs) in Western and Eastern nations, with observational associations of adverse effects with long-term PPI use. Laparoscopic fundoplication is supported as a long-term management option for PPI-responsive GORD and PPI-refractory GORD. The importance of establishing a definitive diagnosis before invasive management, especially in PPI-refractory heartburn, is emphasized.

Summary: The study followed up on Korean patients with GERD who underwent anti-reflux surgery for up to 1 year, finding that over 90% of patients had good symptom resolution at one year post-operation. The proportion of patients with hiatal hernia and comorbidities increased in the later period (2015-2018), with a significant decrease in operation time. Symptom control and complication rate remained comparable between the early and late periods.

Summary: Laparoscopic fundoplication is the standard treatment for medically refractory gastroesophageal reflux disease. While both primary and reoperative fundoplication patients experience improvement in GERD-related symptoms, the degree of improvement is greater in primary patients. Perioperative morbidity rates following reoperative and primary procedures can be similar when performed by an experienced surgeon.

Summary: This retrospective cohort study evaluated 353 patients who underwent laparoscopic fundoplication (LF) for GERD between 2011 and 2017. The results showed that 16% of patients developed GERD-like symptoms after the surgery, and only 9% of these patients had recurrent pathologic acid reflux.

Summary: This study evaluated the long-term results of sleeve gastrectomy with Rossetti fundoplication for the treatment of GERD, esophagitis, and Barrett's esophagus in patients with morbid obesity. The results showed that this surgical approach is well tolerated, feasible, and safe in obese patients, providing adequate weight loss results and complete resolution of clinical signs of GERD.

Summary: In this study, the long-term outcomes of laparoscopic and open fundoplication in children were compared. The recurrence rate was found to be higher and recurrence occurred earlier in laparoscopic surgery compared to open surgery. However, both surgical approaches resulted in high patient and parental satisfaction.

Summary: Regurgitation and gastro-oesophageal reflux (GOR) are common complications in dogs under anaesthesia. Studies on these complications showed inconsistencies in definitions and assessment methods, making meaningful comparisons difficult. Standardization and re-evaluation of methods may be necessary to address these discrepancies.

Summary: A 'lite' version of the EQ-5D-5L valuation protocol, using composite time trade-off data only, was proposed and used to develop an EQ-5D-5L value set for Uganda. The Tobit model with constrained intercepts and corrected for heteroscedasticity was considered the preferred model for the value set. The established value set ranges from -1.116 (state 55555) to 1 (state 11111), with 'pain/discomfort' as the most important dimension.

Summary: This study aimed to determine the long-term benefits and costs of diagnosing and treating FH based on prognostic factors and 'cholesterol burden'. A cost-effectiveness model was developed from the perspective of the UK NHS using routine data from FH patients. The results showed that diagnosing and treating FH resulted in positive net health gains, particularly in individuals with higher pre-treatment LDL-C.

Summary: This study estimated the cost-effectiveness of different radiotherapy modalities for early breast cancer in the UK. The results showed that partial breast radiotherapy delivered in 5 fractions was the most cost-effective option for eligible patients, while whole-breast radiotherapy delivered in 5 fractions was the most cost-effective option for ineligible patients. These findings suggest that hypofractionation and partial breast radiotherapy have important benefits for the UK health system.

The findings revealed technical issues with smart inhalers and concerns about relying solely on algorithm recommendations. However, both staff and families recognized the benefits of technology in improving medication adherence and patient responsibility.

Summary: Viruses are abundant and important in ecosystems, and the two methods of whole genome shotgun metagenome and viral-like particle enriched metagenome sequencing are commonly used for comparing viruses in different environments. In this study, both methods were applied to investigate the stool virome in HCC patients and healthy controls, and they both successfully identified altered viral profiles in HCC patients. Ultra-deep sequencing recovered more viruses, and VLPM method can detect RNA viruses. Using both methods would capture different parts of the total virome and identify shared and specific viral signatures.

Summary: The study found that the combined approach yielded more relatives tested compared to the direct and indirect approaches in cascade testing. Hospital data indicated that those directly contacted were more likely to complete cascade testing. The cost-effectiveness analysis found that contacting first and second-degree relatives directly was the most cost-effective approach. The study also found that combining direct and indirect approaches may be more acceptable to families and healthcare providers.

Summary: Medical therapy for stroke prevention has developed rapidly over the past 30 years, and there is an urgent need to evaluate whether surgery remains cost-effective. A decision model was developed to estimate the lifetime costs and utilities of modern medical therapy with and without carotid endarterectomy in patients with symptomatic stenosis. It was found that surgery remains cost-effective, but these results are sensitive to unknown factors.
