Antonella Meloni, Laura Pistoia, Alessandra Quota, Giuseppe Messina, Paolo Ricchi, Sergio Bagnato, Calogera Gerardi, Roberto Lisi, Liana Cuccia, Stefania Renne, Antonino Vallone, Riccardo Righi, Vincenzo Positano, Alessia Pepe, Filippo Cademartiri

Summary: The aim of this multicenter study was to assess the predictive value of multiparametric cardiac magnetic resonance (CMR) for cardiovascular complications in sickle cell disease (SCD) patients. Reduced left ventricular ejection fraction and increased right ventricular mass index showed a significant prognostic value in patients with SCD.

Gender Differences and Cardiometabolic Risk: The Importance of the Risk Factors

Antonella Meloni, Christian Cadeddu, Lucia Cugusi, Maria Pia Donataccio, Martino Deidda, Susanna Sciomer, Sabina Gallina, Cristina Vassalle, Federica Moscucci, Giuseppe Mercuro, Silvia Maffei

Summary: Metabolic syndrome is a clinical condition characterized by the aggregation of major risk factors for cardiovascular disease and type 2 diabetes, including dyslipidemia, high blood pressure, abnormal blood sugar, and abdominal obesity. These risk factors have an independent effect but become synergistic when combined, leading to a doubled risk of cardiovascular diseases and a 1.5-fold increase in all-cause mortality. This article explores gender differences in the epidemiology, etiology, pathophysiology, and clinical expression of metabolic syndrome components, as well as gender differences in new biochemical markers of metabolic syndrome and cardiovascular risk.

Impact of COVID-19 on the cardiovascular health of women: a review by the Italian Society of Cardiology Working Group on 'gender cardiovascular diseases'

Federica Moscucci, Sabina Gallina, Valentina Bucciarelli, Alberto Aimo, Giovanna Pela, Christian Cadeddu-Dessalvi, Savina Nodari, Silvia Maffei, Antonella Meloni, Martino Deidda, Giuseppe Mercuro, Roberto Pedrinelli, Maria Penco, Susanna Sciomer, Anna Vittoria Mattioli

Summary: COVID-19 is a highly variable disease with different symptoms and signs. Men are more susceptible to the infection, with a higher rate of hospitalization and mortality compared to women. The impact of cardiovascular diseases on COVID-19 outcomes differs between men and women, potentially due to sex-related biological factors and behavioral traits. The renin-angiotensin system and the angiotensin-converting enzyme 2 play key roles in disease pathogenesis, showing hormone-dependent actions. Men also have a higher incidence of myocardial injury associated with inflammation and coagulation disorders. The relationship between cardiovascular diseases and COVID-19 during pregnancy is also discussed.

Summary: This study developed a prognostic scoring system to differentiate β-thalassemia patients based on their mortality risk and validated it in a separate dataset. The scoring system showed good predictive ability for mortality outcome, allowing for risk categorization and informing management decisions.

Bone status and HCV infection in thalassemia major patients

Antonella Meloni, Laura Pistoia, Silvia Maffei, Paolo Ricchi, Tommaso Casini, Elisabetta Corigliano, Maria Caterina Putti, Liana Cuccia, Crocetta Argento, Vincenzo Positano, Alessia Pepe, Filippo Cademartiri, Cristina Vassalle

Summary: The purpose of this study was to investigate the relationship between hepatitis C virus (HCV) infection and bone mineral density (BMD), vitamin D, and bone turnover biomarkers in multi-transfused patients with thalassemia major (TM). The results showed that HCV infection was positively associated with the prevalence of low bone mass, suggesting that HCV infection is an additive risk factor for low bone mass in TM patients. Osteocalcin may play a role in this association.

Left Ventricle Detection from Cardiac Magnetic Resonance Relaxometry Images Using Visual Transformer

Lisa Anita De Santi, Antonella Meloni, Maria Filomena Santarelli, Laura Pistoia, Anna Spasiano, Tommaso Casini, Maria Caterina Putti, Liana Cuccia, Filippo Cademartiri, Vincenzo Positano

Summary: This paper focuses on using the Visual Transformer (ViT) neural network architecture to automatically detect the left ventricle (LV) from cardiac magnetic resonance (CMR) relaxometry sequences. Performance evaluation showed variations in performance based on slice location, but overall performance was comparable to other state-of-the-art methods. The study confirms the feasibility of using ViT architectures for LV detection and establishes a benchmark for relaxometry imaging.

Myocardial tissue characterization by segmental T2 mapping in thalassaemia major: detecting inflammation beyond iron

Antonella Meloni, Laura Pistoia, Vincenzo Positano, Nicola Martini, Rita Laura Borrello, Sara Sbragi, Anna Spasiano, Tommaso Casini, Pier Paolo Bitti, Maria Caterina Putti, Liana Cuccia, Massimo Allo, Francesco Massei, Paola Maria Grazia Sanna, Raffaele De Caterina, Emilio Quaia, Filippo Cademartiri, Alessia Pepe

Summary: In this study, myocardial T-2 values were measured by a segmental approach in patients with thalassaemia major (TM), comparing them with T-2* values for the detection of myocardial iron overload (MIO), evaluating their potential in detecting subclinical inflammation, and correlating with clinical status. The study found that T-2 values were significantly higher in TM patients than in a cohort of healthy subjects, and there was a significant correlation between T-2 and T-2* values. Patients with decreased T-2* values also had reduced T-2 values, and these patients had higher hepatic and pancreatic iron deposition.

Pattern and clinical correlates of renal iron deposition in adult beta-thalassemia major patients

Antonella Meloni, Luigi Barbuto, Vincenzo Positano, Laura Pistoia, Anna Spasiano, Tommaso Casini, Francesco Massei, Crocetta Argento, Piera Giovangrossi, Angelica Barone, Luigia Romano, Filippo Cademartiri

Summary: We evaluated the pattern and clinical correlates of renal T2* measurements in adult β-thalassemia major (β-TM) patients. A total of 11.1% of β-TM patients showed renal iron overload, which was associated with liver iron concentration (LIC) values and pancreas T2* values. Renal iron overload was more common in patients with cardiac iron overload. MRI LIC and lactate dehydrogenase (LDH) were strong predictors of global kidney T2* values. An MRI LIC > 4.83 mg/g dw predicted the presence of renal iron overload.

Pancreatic fatty replacement as risk marker for altered glucose metabolism and cardiac iron and complications in thalassemia major

Antonella Meloni, Mario Nobile, Petra Keilberg, Vincenzo Positano, Maria Filomena Santarelli, Laura Pistoia, Anna Spasiano, Tommaso Casini, Maria Caterina Putti, Liana Cuccia, Pier Paolo Bitti, Giuseppe Messina, Giuseppe Peritore, Stefania Renne, Emanuele Grassedonio, Emilio Quaia, Filippo Cademartiri, Alessia Pepe

Summary: This multicenter study assessed the extent of pancreatic fatty replacement and its correlation with demographics, iron overload, glucose metabolism, and cardiac complications in a cohort of well-treated patients with thalassemia major. The results showed that pancreatic fatty replacement was associated with age, body mass index, and history of hepatitis C virus infection. It was also found that pancreatic fatty replacement was a risk marker for alterations of glucose metabolism, cardiac iron, and complications in thalassemia major patients.
