Approximately 30 000 cases of non-Hodgkin lymphoma (NHL) occur in the equatorial belt of Africa each year. Apart from the fact that Burkitt lymphoma (BL) is very common among children and adolescents in Africa and that an epidemic of human immunodeficiency virus (HIV) infection is currently ongoing in this part of the world, very little is known about lymphomas in Africa. This review provides information regarding the current infrastructure for diagnostics in sub-Saharan Africa. The results on the diagnostic accuracy and on the distribution of different lymphoma subsets in sub-Saharan Africa were based on a review undertaken by a team of lymphoma experts on 159 fine needle aspirate samples and 467 histological samples during their visit to selected sub-Saharan African centres is presented. Among children (<18 years of age), BL accounted for 82% of all NHL, and among adults, diffuse large B-cell lymphoma accounted for 55% of all NHLs. Among adults, various lymphomas other than BL, including T-cell lymphomas, were encountered. The review also discusses the current strategies of the International Network of Cancer Treatment and Research on improving the diagnostic standards and management of lymphoma patients and in acquiring reliable clinical and pathology data in sub-Saharan Africa for fostering high-quality translational research.

Burkitt lymphoma with a granulomatous reaction: an M1/Th1-polarised microenvironment is associated with controlled growth and spontaneous regression

Massimo Granai, Stefano Lazzi, Virginia Mancini, Ayse Akarca, Raffaella Santi, Federica Vergoni, Ester Sorrentino, Raffaella Guazzo, Lucia Mundo, Gabriele Cevenini, Claudio Tripodo, Gioia Di Stefano, Benedetta Puccini, Maurilio Ponzoni, Elena Sabattini, Claudio Agostinelli, Nuray Bassullu, Tulay Tecimer, Ahu Senem Demiroz, Leah Mnango, Stephan Dirnhofer, Leticia Quintanilla-Martinez, Teresa Marafioti, Falko Fend, Lorenzo Leoncini

Summary: This study provides insights into the immune cell subsets involved in the early phase of Burkitt lymphoma, which may help in maintaining the tumor in a self-limited state or inducing its regression. These findings may open up new potential therapeutic avenues for EBV-positive BL patients in the era of cellular immunotherapy.

Variant histology in nodular lymphocyte predominant Hodgkin lymphoma in an adult population: disease investigations and characteristics from a retrospective cohort

Arthur Stacey, Alexandra Jane Marks, Kikkeri N. Naresh

Summary: A subset of nodular lymphocyte-predominant Hodgkin lymphoma patients have variant histological patterns, which are associated with advanced disease stage and increased recurrence risk. Analysis of histopathology reports from 33 adult patients revealed that 39% had variant histology. Variant histology was significantly associated with stage IV disease.

Summary: Despite the effectiveness of immuno-chemotherapy, there is a significant rate of relapse or refractory disease in patients with DLBCL. Our study focuses on the changes in gene expression profiles (GEP) that accompany DLBCL relapse, and we have developed a 30-gene discriminator that can identify clinically distinct high and low-risk subgroups in DLBCL patients. This signature has also been found to predict better outcomes in patients treated with ibrutinib-R-CHOP.

Summary: This study aimed to describe the distribution of Tumour Infiltrating Lymphocytes (TILs) in the intratumoural stroma (sTILs) and the leading/invasive edge stroma (LE-TILs) in breast cancer patients from Kenya, and evaluate TILs across breast cancer subtypes with established risk factors and clinical characteristics. The results showed that TILs were enriched in more aggressive breast cancers, and the associations of sTILs/LE-TILs measures with examined factors varied.

Summary: In this retrospective study, 160 patients with high-grade B-cell lymphoma, not otherwise specified (HGBL-NOS), were examined to determine their characteristics and outcomes. The study found that HG BL-NOS tumors are heterogeneous and most patients presented with advanced stage disease and high-risk clinical factors. Poor performance status, high lactate dehydrogenase, and a dual-expressor immunophenotype were identified as main prognostic factors for progression-free survival and overall survival.

Summary: This study retrospectively observed bone marrow abnormalities in 259 patients receiving CAR-T cell immunotherapy. The study found that 25.5% of patients showed severe marrow hypocellularity and 6.2% showed serous atrophy at a median of 35.5 days post-infusion. Additionally, the study found correlations between bone marrow pathology findings and disease burden and survival rates.

Clinical Validation of FusionPlex RNA Sequencing and Its Utility in the Diagnosis and Classification of Hematologic Neoplasms

Xueyan Chen, Wenjing Wang, Jeffrey Yeh, Yu Wu, Vivian G. Oehler, Kikkeri N. Naresh, Yajuan J. Liu

Summary: This study validated the use of FusionPlex Pan-Heme panel analysis for routine clinical testing, which can sensitively and rapidly identify a wide range of gene fusions for precise diagnosis and prognostication. The validation results showed good concordance between the FusionPlex panel and other methods. In an independent cohort, this analysis reliably detected gene fusions, contributing to accurate classification of hematologic neoplasms and discovery of new fusion partners.

Pediatric autoimmune disorders with gastrointestinal expressions: from bench to bedside

Paola Francalanci, Barbara Cafferata, Rita Alaggio, Paola de Angelis, Antonella Diamanti, Paola Parente, Massimo Granai, Stefano Lazzi

Summary: Autoimmune enteropathy is a rare disorder characterized by severe diarrhea and malabsorption, mostly affecting infants and young children. It is characterized by villous blunting, crypt hyperplasia, and immune cell infiltration in the small intestine, with the possibility of involvement of the esophagus, stomach, and colon. The presence of anti-enterocyte antibodies aids in diagnosis.
