Current developments and perspectives in psoriasis

Keratin 17 in psoriasis: Current understanding and future perspectives

Summary: Keratin 17 (K17) is overexpressed in psoriasis and plays a key role in the pathogenesis of the disease. It has both pro-proliferative and pro-inflammatory effects on keratinocytes and triggers autoreactive T cells and psoriasis-related cytokine production. The expression, stability, and protein-protein interactions of K17 are regulated by cytokines through transcriptional, translational, and post-translational modification processes. Targeting K17 has shown positive outcomes in psoriasis-like mouse models, suggesting a potential therapeutic approach. The future perspectives of K17 in psoriasis are also discussed for further studies.

Biologics for Reducing Cardiovascular Risk in Psoriasis Patients

Summary: Psoriasis is a chronic inflammatory skin disease associated with a high prevalence of cardiovascular disease, obesity, dyslipidemia, hypertension, diabetes mellitus, and metabolic syndrome. Cardiovascular disease is the leading cause of morbidity and mortality in psoriasis patients. Biologics targeting inflammatory cytokines have shown efficacy in improving psoriasis and reducing the risk of cardiovascular disease. This review discusses the cardiovascular risk in psoriasis patients, the association between psoriatic inflammation and atherosclerosis, and the efficacy of biologics in reducing the risk of cardiovascular diseases.

Immunotherapy for SMARCB1-Deficient Sarcomas: Current Evidence and Future Developments

Summary: Mutations in subunits of the SWI/SNF complex, especially the core subunit SMARCB1, occur frequently in human tumors, and SMARCB1 loss is a key driver event in several malignancies. Even though SMARCB1-deficient tumors have a simple genome and low mutation burden, they have been found to be immunogenic, with some responses to immune checkpoint inhibitors reported. This review discusses the potential of immunotherapy, including immune checkpoint inhibitors, as a promising therapeutic strategy for SMARCB1-defective tumors. The heterogeneity among these tumors and the challenges in developing personalized immunotherapy based on molecular profiling are also highlighted.

Antifungal use in children with acute leukaemia: state of current evidence and directions for future research

Summary: Invasive fungal disease (IFD) is a common and serious complication in children treated for leukaemia. The unique challenges in prescribing antifungals for children with leukaemia include variations in IFD risk, drug toxicities, and interactions between antifungal and chemotherapeutic agents. Recent advances in understanding IFD epidemiology, clinical trials in adults, and paediatric studies have provided data for optimal antifungal use in children. A panel of experts compiled a list of 10 key clinical questions and conducted a literature review to guide future research. With changing IFD epidemiology, evolving leukaemia treatment, and novel antifungal agents, advocating for paediatric clinical studies remains vital for IFD prevention and treatment in children with leukaemia.

Psoriasis patient preferences for the use of biologics during the coronavirus era

Summary: Understanding patient preferences for the use of biologics in psoriasis treatment can guide proper treatment selection. This study used a web-based questionnaire survey to investigate psoriasis patients' preferences for biologics and the factors influencing these preferences. The results showed that patients prioritize attributes such as administration route and visit frequency, as well as the risk of serious infections requiring hospitalization.

Summary: The study found a significant relationship between physical activity levels and future cardiovascular disease risk in patients with psoriasis, with significant correlations between physical activity levels and diastolic reflection area as well as the likelihood of future CVD. The diastolic reflection area (DRA) may serve as a surrogate marker for cardiorespiratory fitness in these patients, providing a noninvasive, objective measurement of exercise adherence with potential clinical utility.

Exploring Mast Cell-CD8 T Cell Interactions in Inflammatory Skin Diseases

Summary: The skin is equipped with immune cells like mast cells and CD8 T cells which play a role in recognizing and responding to environmental challenges. These cells are essential for maintaining tissue balance and regulating the immune response in skin diseases. Studies have shown that mast cells can influence the migration and function of CD8 T cells, while CD8 T cells can also activate mast cells. The close proximity between mast cells and CD8 T cells has been observed in dermatoses such as alopecia areata. This review summarizes the bidirectional interactions between human mast cells and CD8 T cells, focusing on their involvement in psoriasis, atopic dermatitis, and vitiligo, and discusses unresolved questions.

Prevalence, comorbidities and mortality of generalized pustular psoriasis: A literature review

Summary: Generalized pustular psoriasis (GPP) is a rare auto-inflammatory skin disease with acute episodes of pustule formation. Understanding the epidemiology and clinical data of GPP is challenging due to its rarity. This review article provides an overview of GPP prevalence estimates in key demographics, genetic mutations associated with the disease, incidence of comorbidities, mortality rate, and causes of death.

Acrodermatitis continua of Hallopeau and generalised pustular psoriasis: Should they be the same or different entities

Summary: This article reviews the similarities and differences between acrodermatitis continua of Hallopeau (ACH) and generalized pustular psoriasis (GPP). IL-36RN mutations are more commonly found in GPP patients, while ACH patients have a lower frequency of these mutations. Although ACH can occur before, simultaneously, or after the development of GPP, the response to treatment appears to differ between the two conditions. Based on the findings, ACH should be classified separately from GPP rather than being considered a subtype of GPP.

In order to restore or maintain health, healthcare should focus on the patient's overall well-being, including life satisfaction, quality of life, and mental well-being. This expert review emphasizes the importance of incorporating well-being into the holistic therapeutic approach for psoriasis patients, and provides recommendations for clinical practice to implement well-being assessment and treatment.
