Summary: For patients undergoing endodontic microsurgery while taking anticoagulant or antiplatelet agents, assessment of bleeding risk is crucial, especially for surgeries exceeding 45 minutes. Individualized risk assessment and consultation with the patient's physician are recommended to balance the effects of medications and potential risks effectively.

Summary: This study investigated the influence of discontinuing perioperative antithrombotic agents in elective posterior spinal surgery. The results showed that discontinuing antithrombotic agents normalized the intraoperative bleeding tendency and the incidence of postoperative epidural hematoma, and it did not significantly influence the incidence of postoperative thromboembolism.

Summary: This study aimed to investigate the association between aspirin use and subarachnoid haemorrhage (SAH) in the general population. The results showed no relationship between aspirin and SAH of all causes or aneurysmal SAH/non-aneurysmal SAH. However, aspirin use was associated with SAH resulting in death, especially out of hospital death.

Summary: Contrary to previous reports, this study found that long-term antiplatelet therapy can lead to the development of new solid tumors. The researchers discovered that a specific type of antiplatelet agent, purinergic P2Y12 inhibitor, actually increased the motility of colon cancer cells. They also identified a gene called SERPINE1, which was associated with cell migration and cell death, and found that antiplatelet treatment increased its expression and secretion, resulting in increased cell motility. These findings suggest that SERPINE1 could be a potential target gene for preventing cancer development and metastasis in patients undergoing long-term antiplatelet therapy.

Summary: The study aimed to confirm or refute previous DDI signals, indicating a potential association between clopidogrel and eszopiclone or zolpidem with serious bleeding. However, due to statistical imprecision, a clear conclusion could not be drawn. Further examination is needed for the associations of other hypnotics with serious bleeding.

Summary: Antiplatelet medications are crucial in the treatment of cardiovascular and cerebrovascular diseases, with different choices and risks depending on the specific conditions of the patients. Personalized therapy based on individual patient characteristics is essential to optimize treatment outcomes.

Summary: Analysis of patient data from studies on primary intracerebral haemorrhage showed that prior antiplatelet therapy did not impact outcomes after the haemorrhage, regardless of haematoma location. However, antiplatelet therapy was associated with admission haematoma volumes in lobar haemorrhages.

Summary: This study reviewed elderly patients admitted for low-level falls aged 65 and older, finding that the use of antithrombotic agents was common and associated with an increased rate of intracranial hemorrhage (ICH); anticoagulants were also associated with higher mortality rates.

Summary: Pharmacogenetics plays a crucial role in predicting the response to pharmacological treatments based on individual genetic background, especially in anticoagulant and antiplatelet therapy. Despite accumulating evidence of its effectiveness, there are still barriers to the implementation of pharmacogenetics in clinical practice.

Summary: This review evaluates the evidence and risk surrounding the management of direct oral anticoagulants (DOACs) in patients undergoing skin surgeries. The findings suggest a low risk of hemorrhagic complications with both continuation and interruption of DOAC therapy. The decision to continue DOACs perioperatively should be based on procedural bleeding risk and patient clotting risk.

Summary: While intraoperative mortality has decreased significantly in recent decades, the risk of death within 30 days of surgery remains high, mainly due to perioperative organ failure. Various perioperative neurological disorders are associated with poor long-term outcomes. There are limited interventions available to mitigate the risk of perioperative central nervous system dysfunction.

Summary: This review aims to discuss the current information on drug interactions in the perioperative period. Drug interactions during this period can affect the efficacy and safety of treatment. There are three types of drug interactions. It is important to recognize that drug interactions may increase toxicity and reduce efficacy, increasing the risk of complications. Thoroughly reviewing a patient's medication list and vigilant monitoring are essential in minimizing risks.

Summary: The lack of guidelines for preoperative management of psychiatric medications can result in inconsistent care and increased risks of perioperative complications and surgery cancellations. This study aims to provide recommendations to clinicians regarding the management of psychiatric medications before surgery, addressing different categories of drugs and individual considerations.

Summary: This narrative review outlines the current evidence on optimal perioperative management of idiopathic pulmonary fibrosis (IPF). Postoperative pulmonary complications pose a significant barrier to safe perioperative care of IPF patients. Medical optimization of IPF patients before surgery is crucial for minimizing this risk.

Summary: This manuscript reviews the risks and current treatments for postdural puncture headache (PDPH). PDPH is a relatively common complication after neuraxial blocks, characterized by orthostatic headaches with associated auditory and/or visual dysregulation. Certain factors such as female sex, young age, previous PDPH, bearing down during labor, and the neuraxial technique itself may predispose patients to PDPH. Long-term complications, including chronic headache, challenge the concept of PDPH as a self-limiting condition. The underlying mechanism of PDPH is still under investigation, and various prophylactic and therapeutic measures have been explored.

Summary: Perioperative medical management is challenging due to the complexity of patients and the lack of guidelines and consensus statements. The Society for Perioperative Assessment and Quality Improvement aims to provide guidance on perioperative medication management by synthesizing literature and expert consensus. A panel of experts was appointed to identify common medications and generate consensus recommendations.

Summary: Cardiovascular conditions such as hypertension, arrhythmias, and heart failure are common in patients undergoing anesthesia. The Society for Perioperative Assessment and Quality Improvement established a group to provide evidence-based recommendations for the management of cardiovascular medications during the preoperative evaluation. Using a modified Delphi process, consensus was reached on the management of 21 classes of medications.
