Summary: Diabetes is a common metabolic condition, affecting around 20% of surgical patients and posing a risk for worse surgical outcomes. Medical management programs can help reduce peri-operative complications. While regional anaesthesia has benefits for function restoration, there are concerns about higher complications in diabetic patients.

Summary: This paper reinterprets the concept of rent as geographical rent and proposes geographical rent platforms (GRPs) as a means to drive regional development. Using regional development initiatives in Kaikoura, New Zealand as examples, the paper demonstrates the potential of collectively developed and place-based initiatives in shaping regional futures. This research is particularly valuable for rural regions and Indigenous-led development initiatives, but aims to contribute to regional development approaches globally through its unique analytical framework and questioning.

Summary: A recent study published in the British Journal of Anaesthesia explores the role of artificial intelligence in ultrasound-guided regional anaesthesia. The researchers found that non-experts' ability to identify key sono-anatomical structures was improved with the assistance of proprietary AI software. Further studies are needed to determine whether such software could enhance learning efficiency and increase patient access to regional anaesthesia.

Summary: The study analyzed data from 45,492 patients who underwent lower extremity amputation between 2005 and 2018 to compare the impact of regional anesthesia and general anesthesia on post-operative pulmonary complications. The results showed that patients who received regional anesthesia had a statistically significant reduction in pulmonary complications compared to those who received general anesthesia. In addition, patients who received general anesthesia were more likely to experience blood transfusions, septic shock, and re-operation. However, there was no significant difference in mortality rate between the two groups.

Summary: Regional anaesthesia shows superior outcomes in postoperative pain control, reducing nausea and vomiting, pulmonary complications, and length of stay. It may also have potential benefits in reducing blood transfusion recurrence and mortality rates, although functional outcomes appear to be short-lived.

Summary: The administration of local anaesthetic adjuncts can extend the duration of peripheral nerve blocks, with dexmedetomidine and dexamethasone showing the best potential for improving block duration. Despite side effects and potential neurotoxicity, off-label use of systemic dexamethasone is recommended as a local anaesthetic adjunct for postoperative pain management, considering concerns related to nerve and muscle injury.

Summary: The current fourth industrial revolution is characterized by the blurring of physics, computing, and biology, driven by data and artificial intelligence. In the field of regional anaesthesia, the application of artificial intelligence is limited, but new technologies such as robotics and artificial sensing may have an impact. The future of robotics in anaesthesia includes pharmaceutical, mechanical, and cognitive robots, with a focus on developing accurate sensors and providing training in augmented reality environments.

Summary: Children's regional anesthesia has evolved rapidly, incorporating new peripheral nerve blocks and rediscovering older techniques like infant spinal anesthesia. The goal is to provide safer and more effective pain management strategies, especially to avoid potential risks associated with long-term general anesthesia. Debate continues on certain practices, but data suggests that providing regional anesthesia to children under general anesthesia is as safe, if not safer, than in sedated or awake patients.
