Summary: This study investigated the risk stratification and outcomes of patients with moderate aortic stenosis. The results showed that factors such as elevated NT-proBNP levels, diabetes, increased mitral valve E/e' ratio, and atrial fibrillation were associated with an increased risk of adverse cardiovascular events. These findings highlight the poor short-medium term outcomes and risk stratification in patients with moderate aortic stenosis and support the need for randomized trials to assess the efficacy of transcatheter aortic valve replacement in this population.

Summary: This study reveals the heterogeneity of major adverse cardiovascular event (MACE) risk in individuals with type 2 diabetes mellitus (T2D) and identifies circulating metabolites reflecting mitochondrial dysfunction that can predict incident MACE. These metabolites remain associated with MACE in multivariate models and show favorable changes with exenatide therapy. The findings suggest that these biomarkers may improve cardiovascular risk prediction models and highlight emerging risk mechanisms.

Summary: Microvascular dysfunction during acute mental stress is an important determinant of major adverse cardiovascular events (MACE), particularly among younger and middle-aged women survivors of a heart attack. In this study, microvascular response to stress and transient endothelial dysfunction were measured using reactive hyperemia index and flow-mediated dilation, respectively. The results showed that worse microvascular response to stress was associated with higher risk of MACE among women, while endothelial dysfunction was related to MACE in both men and women.
