Is integration of healthy lifestyle promotion into primary care feasible? Discussion and consensus sessions between clinicians and researchers

Background: The adoption of a healthy lifestyle, including physical activity, a healthy diet, moderate alcohol consumption and abstinence from smoking, is associated with a major decrease in the incidence of chronic diseases and mortality. Primary health-care (PHC) services therefore attempt, with rather limited success, to promote such lifestyles in their patients. The objective of the present study is to ascertain the perceptions of clinicians and researchers within the Basque Health System of the factors that hinder or facilitate the integration of healthy lifestyle promotion in routine PHC setting. Methods: Formative research based on five consensus meetings held by an expert panel of 12 PHC professionals with clinical and research experience in health promotion, supplied with selected bibliographic material. These meetings were recorded, summarized and the provisional findings were returned to participants in order to improve their validity. Results: The Health Belief Model, the Theory of Planned Action, the Social Learning Theory, stages of change models and integrative models were considered the most useful by the expert panel. Effective intervention strategies, such as the 5 A's strategy (assess, advise, agree, assist and arrange) are also available. However, none of these can be directly implemented or continuously maintained under current PHC conditions. These strategies should therefore be redesigned by adjusting the intervention objectives and contents to the operation of primary care centres and, in turn, altering the organisation of the centres where they are to be implemented. Conclusion: It is recommended to address optimisation of health promotion in PHC from a research perspective in which PHC professionals, researchers and managers of these services cooperate in designing and evaluating innovative programs. Future strategies should adopt a socio-ecological approach in which the health system plays an essential role but which nevertheless complements other individual, cultural and social factors that condition health. These initiatives require an adequate theoretical and methodological framework for designing and evaluating complex interventions.

Nursing Home Survey on Patient Safety Culture: Cross-cultural Validation Data From Spanish Nursing Homes

Summary: Developing an adequate patient safety culture is a relevant objective for all health care levels. The Nursing Home Survey on Patient Safety Culture (NHSOPS) is a 12-dimension tool assessing safety culture in nursing homes for elder people. The results of this study suggest that the Spanish adaptation of NHSOPS can be represented by a 10-factor model. Convergent and divergent validity of the survey were supported, while known-groups validity was acceptable for managers and administrators who considered their NH to be a safe place for residents.

The intervention group showed significant improvements in body mass index, healthy eating habits, and physical activity over 24 months, confirming the program's effectiveness in reducing risk factors for future metabolic diseases like T2DM.

Summary: The development of a family-based interactive lifestyle intervention for adolescents at risk of T2D involved a multidisciplinary team from different countries collaborating to gather potential program topics and theoretical frameworks. Through two iterative cycles of piloting, feedback, and refinement, stakeholders were involved in decision-making regarding content and delivery, leading to the creation of an eight-session program tailored to local languages and adaptations. This approach could provide a flexible intervention method for 'at risk' groups and their families.

Inflammaging markers characteristic of advanced age show similar levels with frailty and dependency

Summary: The study investigates the sociodemographic and clinical variables associated with COVID-19 severity in the Basque Country, Spain, finding that age, sex, patient healthcare complexity, chronic drug consumption, and other factors are closely related to severity. Older cases are more likely to experience severe outcomes, but factors such as sex, health status, and drug consumption may also interfere with and alter this relationship. Understanding these factors is crucial for designing and implementing public health intervention plans for the COVID-19 pandemic.

Summary: This study evaluated the health status and lifestyle habits of vulnerable older adults during the first COVID-19 lockdown in Spain. It found that there were difficulties in walking and reported sleep problems and pain among the participants, although many did not feel anxious/depressed or lonely. The study also identified a decline in functional capacity, worsening of nutritional status, but an increase in family support compared to a previous assessment.
