Similar levels of low-grade inflammation and arterial stiffness in masked and white-coat hypertension: comparisons with sustained hypertension and normotension

Objective The clinical significance of masked hypertension (MHT) and white-coat hypertension (WCHT) remains controversial, whereas subclinical inflammation and arterial stiffness are associated with an adverse prognosis. We examined the interrelationships of MHT, WCHT, and sustained hypertension (SHT) with high-sensitivity C-reactive protein (hs-CRP) and arterial stiffness. Methods Our population consisted of 291 untreated nondiabetic patients with MHT [ office blood pressure (BP) < 140/90mmHg and daytime BP >= 135/85mmHg; n= 32], WCHT (office BP >= 140/90mmHg and daytime BP < 135/85mmHg; n= 81), SHT (office BP >= 140/90mmHg and daytime BP >= 135/85mmHg; n= 178), and 44 age-matched and sex-matched control normotensives. Results SHT compared with WCHT, MHT, and normotension exhibited higher pulse wave velocity (PWV; 8.2 +/- 1.4 vs. 7.5 +/- 1.2 vs. 7.3 +/- 0.9 vs. 6.8 +/- 0.5 m/s, respectively; P < 0.05) and hs-CRP (2.8 +/- 0.7 vs. 2.2 +/- 0.6 vs. 1.9 +/- 0.4 vs. 1.2 +/- 0.3 mg/l, respectively; P < 0.05), independently of confounders. Of note, there was no difference between the MHT and WCHT groups with regard to hs-CRP and PWV levels (P= not significant). In hypertensives, hs-CRP was associated with 24-h systolic BP (r= 0.350, P < 0.0001) and PWV (r= 0.228, P < 0.0001), whereas PWV was associated with 24-h systolic BP (r = 0.330, P < 0.0001). Conclusion MHT and WCHT represent two states of equivalent subclinical vascular dysfunction reflected by hs-CRP and PWV. Moreover, MHT and WCHT are characterized by a higher degree of inflammatory activation and arterial stiffening compared with normotension and by a lesser degree compared with SHT. The association of 24-h BP with both hs-CRP and PWV underscores the dominant role of hemodynamic load on hypertensive damage progression. Summary: The complexity of critical limb ischemia (CLI) requires a dedicated multidisciplinary team for treatment. This study aimed to assess the effectiveness of referring CLI patients scheduled for major amputation elsewhere to a dedicated CLI center. The analysis showed that patients in severe stages of CLI who received a second opinion from a dedicated multidisciplinary team had high limb salvage and survival rates after major amputation.

Age over 65, male gender, chronic kidney disease, diabetes, and left ventricular hypertrophy were identified as independent predictors of CAD.

Stress, increased salt intake, and medication non-adherence were identified as main triggers. Patients with urgencies commonly presented with dizziness and headache, while those with emergencies often showed symptoms of dyspnea and chest pain.

Summary: This study compared the acute effects of tobacco cigarette, electronic cigarette, and sham smoking on sympathetic nerve activity and blood pressure, finding that electronic cigarette smoking led to a decrease in muscle sympathetic nerve activity and an increase in mean arterial pressure similar to traditional cigarette smoking in healthy habitual smokers.

Summary: This article examines tippy network markets that allow price discrimination and finds that under certain conditions, network competition may lead to a unique subgame-perfect equilibrium where the winner's identity is fully determined by its product features. The concept of the dominant value margin is introduced as a metric for the effectiveness of divide-and-conquer strategies. However, the outcome of a market where suppliers with larger dominant value margins can sell to all customers does not necessarily result in social efficiency.

Summary: This study conducted a systematic review and meta-analysis to evaluate the effectiveness and safety of a time to isolation (TTI)-based strategy of cryoballoon ablation (CBA) in the treatment of atrial fibrillation. The results suggest that an individualized CBA dosing strategy based on TTI and extended duration of CBA can reduce recurrence post-AF ablation and decrease procedure time and incidence of phrenic nerve palsy.

Three-dimensional electroanatomically guided slow pathway elimination is associated with procedural improvements and clinical benefit in atrioventricular node reentrant tachycardia patients

Summary: This study compared the clinical outcomes of a three-dimensional electroanatomical mapping (3D-EAM)-based strategy targeting slow pathway (SP) elimination with the conventional fluoroscopic approach in atrioventricular node reentrant tachycardia (AVNRT) ablation. The results showed that 3D-EAM improved procedural parameters, reduced fluoroscopy time and dose, and increased SP elimination frequency. Follow-up results demonstrated lower recurrence rates and redo procedure rates in the 3D-EAM-based group.

Preexisting hypertension and cancer therapy: evidence, pathophysiology, and management recommendation

Summary: Cancer is a leading cause of death and hypertension is a common comorbidity in cancer patients. Preexisting hypertension is an independent risk factor for increased blood pressure during anticancer therapy. Recognizing and managing hypertension as a modifiable risk factor prior to cancer therapy initiation is crucial to prevent premature chemotherapy cessation. Studying onco-hypertension will help establish specific guidelines for managing hypertension in cancer patients.

Best Practice Guide for Cryoballoon Ablation in Atrial Fibrillation: The Compilation Experience of More than 1000 Procedures

Summary: Nowadays, cryoballoon (CB) has become an established alternative to radio frequency (RF) ablation for pulmonary vein isolation (PVI), with the ability to isolate PVs in a single application. In a study on over 1000 consecutive patients, our center optimized the CB PVI procedure. It is expected that future guidelines will recommend CB as the first-line PVI for patients with paroxysmal AF and a class I indication. In a long-term follow-up of the EARLY-AF trial, CB showed a lower incidence of persistent atrial fibrillation compared to anti-arrhythmic drugs.

Ethanol Induced Oxidative Stress in the Vasculature: Friend or Foe

Summary: Oxidative stress is implicated in the development of many ethanol-induced pathologies, contributing to endothelial dysfunction and cardiovascular diseases such as hypertension. Ethanol consumption may alter vascular cell function and microvascular function, increasing the risk for hypertension. Data from human investigations have shown an association between alcohol consumption, changes in blood pressure, and hypertension risk.
