Summary: A 54-year-old man presented with intermittent painless gross hematuria and urodynia, and was diagnosed with upper tract urothelial carcinoma (UTUC) based on imaging and urine-based cytology. After surgery, the patient was unexpectedly diagnosed with MiT family translocation renal cell carcinoma, which is extremely rare and can be easily confused with UTUC, leading to the expansion of surgical scope.

Summary: Accurately predicting the clinical prognosis of upper tract urothelial carcinoma (UTUC) is crucial. This study evaluated the impact of urothelial bladder carcinoma (UBC) involvement on overall survival (OS) and progression-free survival (PFS). The results showed that synchronous or metachronous UBC diagnosis in UTUC patients was associated with worse PFS, but not with significantly worse OS. Lymphovascular invasion (LVI) was found to be a prognostic factor for both PFS and OS in UTUC.

Summary: A rare case of metastatic urothelial carcinoma of the renal pelvis presenting as a duodenal ulcer in a 78-year-old male patient is reported. Endoscopic examination revealed an ulcerative lesion in the duodenum, while colonoscopy showed no abnormalities. Computed tomography revealed tumor infiltration in multiple tissues. Histopathology and immunohistochemistry confirmed the diagnosis of metastatic urothelial carcinoma.

Summary: This study aimed to establish and validate a nomogram for predicting extraurothelial recurrence (EUR) after radical nephroureterectomy (RNU) in patients with upper urinary tract urothelial carcinoma (UTUC). The study retrospectively analyzed data from 521 UTUC patients who underwent RNU and divided them into a training cohort (n=301) and an external validation cohort (n=220). The nomogram was developed using multivariable Cox regression and validated using various parameters such as the concordance index (C-index) and decision curve analysis (DCA). The nomogram showed good discrimination ability and clinical net benefit in predicting EUR in UTUC patients after RNU.

Summary: This study retrospectively collected data from 1018 patients with upper tract urothelial carcinoma (UTUC), and found that pre-nephroureterectomy hydronephrosis can predict post-nephroureterectomy renal function change, chronic kidney disease progression, and preserved eligibility rate for adjuvant therapy. Pre-nephroureterectomy hydronephrosis is an independent protective factor for post-nephroureterectomy renal function decline, CKD progression, and eligibility for adjuvant therapy.

Summary: Sarcomatoid urothelial carcinoma (SUC), a rare tumor of the urinary tract epithelium, is often misdiagnosed as benign inflammatory disease due to the lack of specific clinical presentations and imaging findings. We present a case of SUC of the renal pelvis, initially misdiagnosed as a renal abscess, which was only correctly diagnosed through exfoliative cytology. The patient underwent radical surgery and postoperative chemotherapy, and no recurrence or metastasis occurred during the one-year follow-up.

Summary: According to national cancer registry data, segmental ureterectomy is not associated with inferior survival compared to radical nephroureterectomy in upper-tract urothelial carcinoma patients. Segmental ureterectomy provides a valid surgical approach that does not meaningfully sacrifice survival outcomes within appropriately selected patients of upper urothelial carcinoma of the ureter. It should be considered for selected patients, especially if there is an increased probability of requiring adjuvant chemotherapy.

Summary: Survival of upper urinary tract variant histology tumors (UTVH) compared to upper urinary tract urothelial carcinoma (UTUC) is poorly understood. UTVH patients tend to have more advanced disease stage at diagnosis, with squamous cell carcinoma (SCC) showing the highest cancer-specific mortality. However, in patients with stage T1-2 disease, those with SCC or adenocarcinoma who undergo radical nephroureterectomy have similar survival outcomes compared to UTUC patients.

Summary: Nested subtype of urothelial carcinoma (NVUC) is rare in the upper urinary tract, and its behavior and diagnosis in the renal pelvis and ureter are not well understood. A study identified 19 cases of NVUC in the renal pelvis and ureter, with varying morphologies and pathologic stages. NVUC in the upper tract is different from that in the bladder, as it is more frequently associated with an overlying papillary urothelial carcinoma and often has a large nested pattern, but appears to behave less aggressively.
