The conversion to vNOTES allows for surgeon flexibility.

Summary: The study aimed to evaluate the efficacy and safety of transvaginal natural orifice transluminal endoscopic surgery (vNOTES) in the treatment of female infertility. The results showed that compared to the LESS group, the vNOTES group had a shorter postoperative bowel movement time and less pain at 4 and 12 hours. vNOTES represents a new, less invasive approach for infertility diagnosis and treatment that is particularly suitable for women who have special esthetic requirements, and it may be an ideal choice for scarless infertility surgery.

Summary: Transvaginal natural orifice transluminal endoscopic surgery for hysterectomy showed better outcomes in terms of hospitalization duration, visual analog scale pain scores at 1 and 18 hours postoperatively, and lower complication rates compared to single-port laparoscopic hysterectomy. Despite not being widely adopted in gynecology departments, it could be a promising alternative approach to other minimally invasive surgical procedures for selected patients.

The learning curve analysis showed that port-installation time and total operation time appeared to reach proficiency level by the 10th case.

Summary: This study compared the surgical outcomes of gasless and traditional robot-assisted transvaginal natural orifice transluminal endoscopic surgery (GR-vNOTES vs. TR-vNOTES) in hysterectomy. The results showed that both techniques had similar operative outcomes and no complications. GR-vNOTES is a feasible and safe alternative to TR-vNOTES in hysterectomy.

Summary: This article introduces a novel robotic instrument that can assist in Natural Orifice Transluminal Endoscopic Surgery (NOTES). The instrument has a diameter of 6.5 mm and 7 degrees of freedom (DOF). It utilizes helical springs with high torsional rigidity for three continuum elastic joints, and patterned concentric tubes to change the stiffness of the force transmission structure. Analytical solutions for inverse kinematics are obtained based on the arrangement of coupled joints. A disk-elastomer model is established to simplify the derivation of kinematics, and the relationship between cable slack and joint parameters is derived. Experimental results verify the trajectory tracking accuracy, bending accuracy, and variable stiffness ability of the proposed instrument.
