Summary: Intrapartum initiation of extended-release nifedipine is effective in preventing severe hypertension among individuals with preeclampsia with severe features. The use of nifedipine resulted in a lower rate of acute hypertension therapy, cesarean delivery, and neonatal intensive care unit admission compared to placebo. The neonatal composite outcome was similar between groups.

Summary: This retrospective study aimed to compare the risk of readmission in patients with peripartum hypertension who received no treatment, labetalol, nifedipine, or both at hospital discharge. The study found that nifedipine monotherapy and combined nifedipine-labetalol treatment significantly reduced the risk of readmission for hypertensive complications. In contrast, labetalol monotherapy was associated with an increased risk of readmission.

Summary: This review provides a comprehensive overview of the pharmacokinetics (PK) of the antihypertensive drugs methyldopa, labetalol, and nifedipine during pregnancy. Despite decades of prescribing these drugs during pregnancy, the available studies are limited and heterogeneous. Further research on the PK and pharmacodynamics of these drugs during pregnancy is urgently needed for evidence-based and personalized dosing to prevent undertreatment, overtreatment, and side effects.

Summary: Hypertensive disorders of pregnancy (HDP) are becoming more common and have negative effects on both mothers and infants. Labetalol, nifedipine, and methyldopa are recommended as the first-line medications for HDP treatment. However, the actual usage and selection of antihypertensive drugs remains unclear. A retrospective cohort study involving 1,641 HDP patients was conducted, and it was found that 77.8% of patients used antihypertensive medications. Labetalol was the most commonly used medication, followed by nifedipine and hydralazine. The severity of HDP was associated with a higher likelihood of antihypertensive use. Further research comparing medication effectiveness in different types of HDP is needed.

Summary: This review article analyzes the role of C19MC miRNA in the development of pre-eclampsia and summarizes the current research data. The results indicate that C19MC miRNAs are significantly upregulated in placental tissue of pre-eclampsia patients and can be detected in high levels in the blood. Among them, miR-525-5p shows low expression in the plasma of the first trimester. By identifying these miRNAs, it may be possible to predict the development of pre-eclampsia before the onset of clinical symptoms, thereby avoiding serious complications.

Summary: This qualitative evidence synthesis identified key factors influencing the underutilization of MgSO4 for the prevention and treatment of pre-eclampsia and eclampsia, with themes such as provider competence, health system capability, and knowledge translation. Mapping facilitators and barriers within the COM-B behavior change theory framework highlighted the complex interplay between physical and psychological capability, social and physical opportunity, and motivation. The findings from this review can guide policy and guideline development to improve the appropriate use of MgSO4 in clinical care worldwide.

Summary: This study aimed to investigate the value of angiogenic marker assessment in women with chronic hypertension and suspected superimposed PE. The study found that adding angiogenic markers to the traditional diagnostic criteria significantly improved the prediction of adverse outcomes for both mothers and infants. Therefore, the assessment of angiogenic markers should be considered when evaluating pregnant women with chronic hypertension.

Summary: This study investigates the use of MRI, MRS, and serum metabolomics to differentiate between pre-eclampsia (PE) and gestational hypertension (GH). Increased T1 signal intensity index (T1SI), lactate/creatine (Lac/Cr), and glutamine and glutamate (Glx)/Cr, as well as decreased apparent diffusion coefficient (ADC) value and myo-inositol (mI)/Cr, were found in PE patients. Combination of Lac/Cr, Glx/Cr, and mI/Cr showed the highest differentiating performance. Serum metabolomics analysis identified 12 differential metabolites associated with various metabolic pathways. MRS is expected to be a noninvasive and effective tool for monitoring GH patients to avoid the development of PE.

Additionally, there were no significant differences between the two groups in secondary outcomes.

Summary: In this study, 10 independent predictors of maternal satisfaction with labor induction were identified, including maternal education, previous cesarean delivery, maternal involvement, information provided, and decision-making time pressure. These findings have important implications for patient selection, specific preinduction and intrapartum care, and focusing on induction methods that can reduce induction to delivery interval, cesarean birth, and delivery blood loss to maximize women's satisfaction with labor induction.

However, all growth charts had poor diagnostic accuracy in the second trimester, highlighting the need for alternative techniques for early detection of SGA.

Summary: Comparison between 6-hour and 12-hour Foley balloon catheter placement for cervical ripening in labor induction of nulliparas showed that the shorter placement time led to a shorter time to birth, although with less cervical ripening and more additional methods used for ripening. However, the 6-hour placement was less likely to be recommended by participants compared to the 12-hour placement.

Summary: The study aimed to evaluate the impact of immediate oral full feeding and on-demand oral full feeding during labor after unplanned cesarean delivery on vomiting and maternal satisfaction. The results showed that immediate feeding did not increase maternal satisfaction and was not noninferior in terms of post-operation vomiting compared to on-demand feeding. It is recommended to encourage and provide early full feeding.
