Relationship between genetic risk factors and markers for Alzheimer's disease pathology

Alzheimer's disease (AD) is a neurodegenerative disorder characterized by neuritic plaques (main constituent: beta-amyloid [A beta]) and neurofibrillary tangles (hyperphosphorylated tau protein) in the brain. Abnormalities in A beta and tau can be measured upon neuropathological examination, in cerebrospinal fluid or by PET. Etiologically, a growing body of evidence suggests that susceptibility to AD is genetically controlled. However, the precise nature of the underlying risk genes and their relation to AD biomarkers remains largely elusive. To this end, we performed a qualitative review of 17 studies (covering 47 polymorphisms in 26 genes) and investigated the potential relation between the most compelling AD risk genes and markers for A beta and tau in cerebrospinal fluid, PET imaging and neuropathological examination. Of all covered genes, only APOE and PICALM showed consistent effects on A beta but not on tau, while no obvious effects were observed for CLU, CR1, ACE, SORL and MAPT.

Summary: We found heterogeneity in regional-atrophy patterns through latent-profile analysis of 1,482 longitudinal magnetic resonance imaging observations. The results supported a 2-group solution reflecting differences in atrophy rates in cortical regions and hippocampus along with comparable caudate atrophy. Our findings suggest distinct mechanisms of atrophy in striatal versus hippocampal-cortical systems.

Predicting progression to Alzheimer's disease with human hippocampal progenitors exposed to serum

Summary: Adult hippocampal neurogenesis is important for learning and memory and is altered early in Alzheimer's disease. An assay using a human hippocampal progenitor cell line and longitudinal serum samples revealed that the systemic environment affects hippocampal neurogenesis in individuals with mild cognitive impairment, predicting Alzheimer's disease progression. This assay shows potential for early prognosis, monitoring disease progression, and further research.

A comparison between similarity matrices for principal component analysis to assess population stratification in sequenced genetic data sets

Summary: Genetic similarity matrices are commonly used to assess population substructure, and in this study, the performance of three different matrices was evaluated. The results showed that rare single-nucleotide variations (SNVs) provided the best clustering performance, with the unweighted Jaccard matrix being the most preferable for rare variants. The visual inspection and clustering metrics confirmed its superiority compared to other methods and different allele frequency cutoffs. The application of these matrices to Alzheimer's disease data sets and different populations revealed the role of rare variants in local and global population substructure.

Summary: Hearing loss in older adults is associated with increased dementia risk, and the underlying mechanisms connecting the two remain unclear. This study found an association between hearing loss and biomarkers for dementia risk, and the association varied between different age groups. In older adults, hearing loss was associated with a decline in memory, global cognition, and language.

Summary: This study employed an integrative system and causal inference approach to explore molecular signatures in blood and CSF, the AT(N) framework, MCI conversion to AD, and genetic risk for AD. Using the EMIF-AD cohort, the researchers measured proteins and metabolites in blood and CSF, as well as genotyped whole-blood samples. They found associations between AT(N) framework and protein/lipid hubs, and identified Proprotein Convertase Subtilisin/Kexin Type 7 as a potential causative factor for AD.

Summary: This study has identified CSF biomarkers associated with early tau pathology-associated unfolded protein response (UPR) activation through biomarker discovery and validation. The levels of PDIA1 and PDIA3 correlate with total and phosphorylated tau levels in CSF, and PDIA1 levels are increased in AD patients compared to controls and patients with tau-unrelated dementia.

Summary: This study aims to provide guidance on prognosis and assessment indicators for patients with Alzheimer's clinical syndrome. A consensus was reached on priority outcomes, measures, and statements across nine domains using the Delphi method and modified GRADE criteria. This work provides clues for clinicians on the domains and relevant measurement tools that may be used to follow patients with cognitive impairment. More work is needed to develop measurement tools that are more feasible in the context of clinical routine.

Summary: The amyloid cascade hypothesis has played a significant role in Alzheimer's disease research and clinical trial designs. This study investigated the relationship between amyloid-beta and tau at the individual and twin-pair levels, aiming to rule out genetic and shared environmental effects as confounders. The findings suggest that the associations between amyloid-beta, tau, neurodegeneration, and cognition are unbiased by genetic factors, and the effects of amyloid-beta on neurodegeneration and cognitive decline are mediated by tau.

Summary: Aging-related cognitive decline can be accelerated by a combination of genetic factors, cardiovascular and cerebrovascular dysfunction, and amyloid-beta burden. The study revealed that cerebral blood flow (CBF) in cognitively unimpaired individuals has a genetic component, is negatively associated with cerebrovascular damage, and is positively associated with the interaction between cardiovascular risk scores and early amyloid-beta burden.

Discovery of novel CSF biomarkers to predict progression in dementia using machine learning

Summary: This study used machine learning techniques to identify cerebrospinal fluid (CSF) biomarkers that predict the rate of cognitive decline in dementia patients. Longitudinal mini-mental state examination scores (MMSE) were used to create fast and slow progression groups. Random forest classifiers were trained on CSF proteomic profiles and a well-performing prediction model for the progression group was obtained. TNFRSF4 and TGF beta-1 emerged as the top markers for predicting cognitive decline.

Physical activity levels in cognitively normal and cognitively impaired oldest-old and the association with dementia risk factors: a pilot study

Summary: The study aimed to determine the physical activity levels of adults older than 90 years of age and its association with risk factors for dementia and brain pathology biomarkers. The results showed that cognitively impaired oldest-old individuals had lower movement intensity. This indicates that physical activity is related to the physical condition and brain pathology biomarkers of older adults.

Summary: Lecanemab, an anti-amyloid antibody, has been granted accelerated approval by the FDA in 2023 for early Alzheimer's Disease. Regulatory review in Europe is ongoing. However, the estimated treatment costs in the 27 EU countries would be unsustainable if priced similarly to the United States. This could result in limitations to patient access and deepen inequalities in health outcomes across Europe.
