Summary: This study compared the epidemiological features and clinical presentations of deep infiltrating endometriosis with endometrioma and adenomyosis, and identified risk factors for these conditions. The results showed that deep infiltrating endometriosis was associated with severe dysmenorrhoea and private surgical care, while adenomyosis was associated with higher fertility desire and lower body mass index. Therefore, patients with pain symptoms and subfertility should be referred early to a tertiary center for the diagnosis and management of deep infiltrating endometriosis.

Summary: The study aimed to demonstrate the coexistence of endometriomas with peritoneal and deep infiltrating endometriosis, as well as with adhesions. It was a randomized retrospective study including 1054 patients treated for endometrioma at Academic Hospital Cologne Weyertal from January 2014 to October 2019. The results showed a high prevalence of peritoneal and deep infiltrating endometriosis in patients with endometriomas, with the need for surgical treatment to be cautious due to the potential presence of adhesions and complications.

Summary: This systematic review and meta-analysis evaluated the diagnostic accuracy of the sliding sign on transvaginal ultrasound in detecting pouch of Douglas obliteration and bowel involvement in patients with suspected endometriosis, using laparoscopy as the reference standard. The results showed that the sliding sign had good diagnostic performance for predicting pouch of Douglas obliteration and bowel involvement.

Summary: The diagnosis of endometriosis may be delayed due to non-deterministic symptoms and avoiding surgical interventions. This study found that serum kisspeptin levels were significantly different between endometriosis patients and healthy controls, but were unable to differentiate the severity of endometriosis. This suggests that kisspeptins may play a role in the pathogenesis of endometriosis and further research is needed.

Summary: Chronic pelvic pain is commonly associated with additional pathologies such as adenomyosis and endometriosis. A retrospective study found that 40% of patients with associated pathologies did not experience relief from pelvic pain at the early follow-up and 32% at the 12-month follow-up after laparoscopic niche repair. Therefore, careful patient selection is important for uterine scar repair in patients with concomitant adenomyosis and endometriosis.

Summary: This study molecularly characterized the gene expression signatures of adenomyosis uteri (FA) and deep infiltrating endometriosis (DIE). The results showed significant differences in RNA expression levels between the two diseases. Genes belonging to the PI3K pathway were highly expressed in DIE, while genes belonging to the RAS pathway were highly expressed in FA.

Summary: This study retrospectively analyzed 63 patients with deep infiltrating endometriosis treated by transumbilical single-port laparoscopy based on retroperitoneal pelvic spaces anatomy. The operation duration, estimated blood loss, postoperative hospital stay, and incidence of postoperative complications were all favorable, with low rates of intestinal injury, ureteral injury, and pelvic infection. The postoperative scar score and satisfaction score were also high. In conclusion, transumbilical single-port laparoscopic surgery based on retroperitoneal pelvic spaces anatomy is a feasible and advantageous approach for treating deep infiltrating endometriosis.

Summary: The purpose of this consensus article was to develop guidelines for the interpretation and reporting of deep infiltrating endometriosis (DIE) with magnetic resonance imaging (MRI). Consensus was reached on pelvic compartment delineation and reporting template using the RAND-UCLA Appropriateness Method. A tailored MRI lexicon and standardized region-based report were proposed.

Summary: Endometriosis-associated pain plays a crucial role in determining the need for surgical intervention. However, there is currently no quantitative method to diagnose the intensity of local pain, especially in cases of deep endometriosis. This study examines the clinical significance of a pain score system that utilizes a pelvic examination to assess endometriotic pain. The results show a significant decrease in the pain score postoperatively, suggesting the potential use of this score to develop surgical strategies for deep endometriosis.

Summary: This study compared the staining levels of T-cadherin, E-cadherin, PR, and ER in deep infiltrating endometriosis (DIE) tissue, ovarian endometriomas, and normal endometrial tissues in the same individuals. The results showed that T-cadherin, E-cadherin, ER, and PR were lowest in DIE tissue and highest in endometrial tissue, indicating different biological properties between DIE and endometriomas.

This suggests that postoperative TVUS can be useful for detecting persistent disease or pelvic adhesions in symptomatic patients.

Summary: This study aimed to evaluate the need for better-defined Morphological Uterus Sonographic Assessment (MUSA) features of adenomyosis and to reach consensus on updated definitions through a Delphi procedure. The experts identified a need for more detailed definitions of some MUSA features and recommended the use of 3D ultrasound for optimal visualization. Consensus was achieved on the revised definitions and classification of MUSA features as direct or indirect signs of adenomyosis.

Summary: Preeclampsia is a pregnancy-specific disease with an unknown precise cause. Multi-omics approaches have been used to study the molecular mechanisms of preeclampsia, and various genetic variants, altered gene expressions, potential biomarkers, and metabolic changes have been identified. Microbiomics studies have also found dysbiosis in the microbiota of pregnant women with preeclampsia. Further research is needed to integrate and translate these findings into clinical practice.
