We report on the results of a randomized study (n - 200) to compare total knee arthroplasty performed using conventional instrumentation or electromagnetic computer assisted surgical technique. 92% of navigated and 85% of conventional knees were implanted within +/- 3 degrees from neutral mechanical alignment; there was no statistically significant difference between these proportions. There was also no difference in femoral or tibial rotation assessed by CT scan. At 1 year follow up there was no statistical difference between the two groups in American Knee Society Score, Oxford Knee Scores, patient satisfaction, quality of life, hospital length of stay, complication rates or other adverse events. Tourniquet time in the navigated group was longer. Proving value for navigation in total knee arthroplasty surgery remains a challenge. Crown Copyright (C) 2014 Published by Elsevier Inc. Significant differences in rates of aseptic loosening between two variations of a popular total knee arthroplasty design

Michael Brown, Rohan Ramasubbu, Mark Jenkinson, James Doonan, Mark Blyth, Bryn Jones

Summary: This study found an increased risk of aseptic loosening in non-augmentable components, highlighting the impact of minor implant changes on outcomes. Clinicians are advised to remain vigilant to implant changes and publish their own research results when important trends are observed.

The relation between acute changes in the systemic inflammatory response and circulating thiamine and magnesium concentrations after elective knee arthroplasty

Donogh Maguire, Anthony Catchpole, Owen Sheerins, Dinesh Talwar, Alana Burns, Mark Blyth, Andrew Shaw, Bryn Jones, Colin Drury, Johann Harten, Innes Smith, Donald C. McMillan

Summary: This study aimed to investigate the relationship between acute changes in the systemic inflammatory response (SIR) and thiamine and magnesium dependent enzyme activity in patients undergoing elective knee arthroplasty. The findings showed that pre-operatively TDP and magnesium concentrations were normal, but post-operatively TDP concentrations increased while magnesium concentrations decreased. Thiamine and magnesium could be potentially modifiable therapeutic targets to modulate the host inflammatory response.

Quantifying graft impingement in anterior cruciate ligament reconstruction

W. T. Wilson, G. P. Hopper, M. O'Boyle, L. Henderson, M. J. G. Blyth

Summary: This study investigates the prevalence and causes of graft impingement during ACL reconstruction surgery. The results show that impingement requiring repair occurs in a significant number of cases. The tunnel position does not correlate with the amount of impingement. The size of the intercondylar notch is associated with the degree of impingement.

Yu Wei Chua, Szu-Ching Lu, Anna Anzulewicz, Krzystof Sobota, Christos Tachtatzis, Ivan Andonovic, Philip Rowe, Jonathan Delafield-Butt

Summary: The study revealed differences in feedforward and feedback mechanisms of prospective action organization between autistic and neurotypical children, suggesting specific developmental trends in movement kinematics in autistic children.

Anterior cruciate ligament repair with internal brace augmentation: A systematic review

W. T. Wilson, G. P. Hopper, M. S. Banger, M. J. G. Blyth, P. E. Riches, G. M. MacKay

Summary: This systematic review with meta-analysis demonstrates that ACL repair with internal bracing is a safe technique for the treatment of proximal ruptures, with a failure rate of 10.4%. Subjective scores and clinical laxity testing also showed satisfactory results. Therefore, ACL repair with internal bracing should be considered as an alternative to ACL reconstruction for acute proximal tears, offering the potential benefits of retained native tissue and proprioception, while avoiding the need for graft harvest.

Robotic arm-assisted versus manual unicompartmental knee arthroplasty A SYSTEMATIC REVIEW AND META-ANALYSIS OF THE MAKO ROBOTIC SYSTEM

J. Zhang, N. Ng, C. E. H. Scott, M. J. G. Blyth, F. S. Haddad, G. J. Macpherson, J. T. Patton, N. D. Clement

Summary: This systematic review compares the precision of component positioning, patient-reported outcome measures, complications, survivorship, cost-effectiveness, and learning curves between MAKO robotic arm-assisted unicompartmental knee arthroplasty (RAUKA) and manual medial unicompartmental knee arthroplasty (mUKA). The review found that MAKO RAUKA has better precision in implant positioning, but no improvement in patient-reported outcome measures. In addition, MAKO RAUKA is shown to be cost-effective, but this is related to the volume of surgeries.

Soft-tissue landmarks for tibial baseplate rotational alignment in total knee arthroplasty: A cadaveric study

Onyedikachi Eseonu, Mhairi Mactier, Eilidh Ferguson, Fabio Quondamatteo, Mark Blyth, Bryn Jones

Summary: The aim of this study was to determine the relationship between soft-tissue landmarks and the tibial tubercle tip (TTT) and identify any sex differences in these measures. The results indicate that the patellar tendon footprint is evenly spread around the TTT, but with large variability.

No advantage with navigated versus conventional mechanically aligned total knee arthroplasty-10 year results of a randomised controlled trial

Omer M. Farhan-Alanie, Tareq Altell, Sinead O'Donnell, Pauline May, James Doonan, Philip Rowe, Bryn Jones, Mark J. G. Blyth

Summary: This study compares the ten-year revision rates and patient-reported outcome measures of electromagnetically (EM) navigated and conventional total knee arthroplasty. The results show that there is no difference in revision rates or clinical outcomes between the two methods after ten years of follow-up. Patients who received EM navigated surgery had better outcomes at 3 months post-operatively, but the outcomes were equivalent between the two interventions from 1 to 10 years post-operatively.

Survivorship of high tibial osteotomy in the treatment of osteoarthritis of the knee: a retrospective cohort study with fourteen years' follow-up

Rahul Bhattacharyya, Almothenna Alloush, Christina Wilson, James Doonan, Brian Rooney, Colin Walker, Angus Maclean, Mark Blyth

Summary: This study aimed to evaluate the survivorship of HTO for the treatment of medial compartment OA in young and active patients. A retrospective cohort multicenter study was conducted, analyzing a case series of HTO's performed by four surgeons over a 14-year period. The results showed comparable survivorship at five and ten year follow-up, with an association found between age and larger corrections requiring bone graft.

Are there functional biomechanical differences in robotic arm-assisted bi-unicompartmental knee arthroplasty compared with conventional total knee arthroplasty? A prospective, randomized controlled trial

M. S. Banger, J. Doonan, B. G. Jones, A. D. MacLean, P. J. Rowe, M. J. G. Blyth

Summary: This study aimed to compare the differences in the primary outcome of biphasic flexion knee moment during gait between robotic arm-assisted bi-unicompartmental knee arthroplasty (bi-UKA) and conventional mechanically aligned total knee arthroplasty (TKA) at one year post-surgery. The results showed that both interventions were effective in improving gait, but there were residual deficits compared to a healthy age-matched control group.

A Co-creation Centre for Accessible Rehabilitation Technology

Andy Kerr, Madeleine Ann Grealy, Anja Kuschmann, Rosie Rutherford, Philip Rowe

Summary: The prevalence of disabling conditions is increasing globally, and the limited workforce cannot deliver intensive rehabilitation. Technology provides tools for self-rehabilitation but access is limited due to barriers such as cost and poor design. A new centre has been established to develop accessible rehabilitation technology through a co-creation model, currently undergoing testing with stroke survivors, with the potential to revolutionize the way rehabilitation is delivered.
