Summary: Antimicrobial-resistant Neisseria gonorrhoeae infections are a threat to public health. Molecular assays for predicting antimicrobial susceptibility can guide therapy and combat resistance. This review discusses the current status and future directions of resistance-guided therapy for three antimicrobials.

Summary: This study identified a ceftriaxone-resistant Neisseria gonorrhoeae FC428 clone with moderate-level azithromycin resistance in Shenzhen, China in 2020. The isolate YL201 showed specific NG-STAR and MLST types and clustered with other FC428 clones from different regions in China. Genomic analysis indicated that the FC428 clones have undergone further transmission and may lead to increased severity of gonococcal resistance.

Summary: This study compiled a global collection of Neisseria gonorrhoeae strains with decreased susceptibility to ceftriaxone and assessed the genetic characteristics associated with this phenomenon. Molecular algorithms based on genetic mutations were proposed to predict decreased susceptibility to ceftriaxone in N. gonorrhoeae, which may serve as a foundation for the development of future assays predicting ceftriaxone decreased susceptibility globally.

Summary: The study demonstrated that gentamicin showed excellent in vitro susceptibility against clinical gonococcal isolates, including multidrug-resistant strains. Combinations of gentamicin with ertapenem, ceftriaxone, and azithromycin exhibited synergistic effects against certain multidrug-resistant isolates, with no antagonism observed in any of the antimicrobial combinations, showing potential for guiding clinical testing of combination therapies.

Summary: The study aimed to use machine learning models to predict ceftriaxone susceptibility and decreased susceptibility in Neisseria gonorrhoeae. Seven machine learning models were evaluated using a public database of N. gonorrhoeae genomes, and individual SNPs' contributions to the prediction were calculated. The results highlight the practical application of machine learning in predicting antimicrobial resistance and aiding in the development of assays.

Summary: The study aimed to evaluate the efficacy of ertapenem, gentamicin, and fosfomycin as alternative treatments for anogenital Neisseria gonorrhoeae infections. The findings showed that single-dose 1000 mg ertapenem is non-inferior to single-dose 500 mg ceftriaxone in gonorrhoea treatment, while 5 mg/kg gentamicin is not non-inferior to ceftriaxone. Ertapenem is a potential effective alternative for anogenital Neisseria gonorrhoeae infections and merits evaluation for ceftriaxone-resistant infections.

Summary: In addition to antimicrobial resistance, bacteria have other mechanisms, such as tolerance, to survive antibiotic exposure. This study demonstrates the induction of tolerance to ceftriaxone in Neisseria gonorrhoeae through antibiotic exposure and the presence of ceftriaxone tolerance in clinical isolates.

Summary: Antibiotic tolerance is associated with treatment failure, but the molecular mechanisms behind it are poorly understood. In a recent study, researchers induced tolerance to ceftriaxone in N. gonorrhoeae and identified mutations in four genes that may contribute to this tolerance. These findings provide insight into the development of antibiotic tolerance in bacteria.

Summary: This study described the molecular epidemiology of antimicrobial resistance and the distribution of resistance-associated genotypes in Neisseria gonorrhoeae in Shenzhen, China from 2014 to 2018. Typing schemes revealed specific genotypes associated with decreased susceptibility to ceftriaxone, with certain sequence types showing international spread.

Summary: Neisseria gonorrhoeae isolates collected in Nanjing, China that showed decreased susceptibility or resistance to extended-spectrum cephalosporins (ESCs) were found to have varying susceptibility to ertapenem. The study also identified genetic determinants of ESC resistance and analyzed associations with ertapenem susceptibility. The findings suggest that ertapenem may be an effective therapy for ESC-resistant isolates with identifiable resistance determinants.

Summary: This study investigated the susceptibility of N. gonorrhoeae isolates collected in three Chinese provinces to gentamicin, as well as the correlation between the MICs of gentamicin, azithromycin, and ceftriaxone. The efficacy of combination therapy using these three antibiotics was also examined. The majority of N. gonorrhoeae isolates demonstrated some level of susceptibility to gentamicin. Overall, the combination therapy of gentamicin with ceftriaxone or azithromycin had indifferent effects.

Summary: The study aimed to determine the minimum inhibitory concentrations (MICs) of ertapenem on Neisseria gonorrhoeae collected from eight Chinese provinces in 2018. The results suggest that ertapenem has satisfactory susceptibility in isolates from China, making it a promising treatment option for resistant gonococcal infections.
