Summary: Anastomotic leak after esophagectomy is a serious complication with potential negative impact on long-term survival. This study found that patients who experienced anastomotic leak had a higher mortality risk during the first 2 years of follow-up.

Summary: This study found that male sex, older age, lower preoperative albumin, and a 5-cm-wide gastric tube are risk factors for anastomotic leakage, while the grade of leakage is significantly associated with the development of stricture. These factors could be valuable in improving perioperative strategies to prevent anastomotic complications.

Summary: Background: This study aimed to investigate the impact of pleural adhesions on surgical and oncological outcomes in patients undergoing McKeown minimally invasive esophagectomy (MIE) for esophageal cancer. The presence of pleural adhesions was found to be associated with prolonged operation time, longer hospital stay, postoperative pneumonia, and hydrothorax requiring drainage. However, it did not have an unfavorable effect on long-term survival.

Summary: This retrospective observational study aimed to identify risk factors for anastomotic stricture after esophageal cancer surgery and evaluate the effect of different anastomosis techniques on stricture development. Chronic obstructive pulmonary disease, leakage, and anastomosis techniques other than triangular linear stapling with 60-mm staplers were significant risk factors for stricture. Triangular linear stapling significantly reduced the stricture rate compared with other techniques.

Summary: The routine use of LapTHE for DE and GEJ cancers, along with the inclusion of six operative principles, results in a low rate of anastomotic complications compared to national benchmarks. The median number of lymph nodes procured was 19, and negative margins were achieved in all cases. Major complication rate was 24%, 90-day mortality was 2.0%, and reoperation rate was 5.4%.

Summary: This study evaluated the complications of supercharged cervical anastomosis for esophagectomy in 80 patients. Results show that patients who underwent the supercharged procedure had lower rates of anastomotic leakage and stricture, along with improved perfusion in the anastomotic area.

Summary: In this study, a classification system based on CT findings was developed to predict the outcomes of sleeve gastrectomy leaks. The system showed good agreement and could help guide patient management and interventions.

Summary: This study aimed to understand small bowel necrosis after esophagectomy, a rare but fatal complication. A retrospective review was conducted on patients who underwent esophagectomy for esophageal cancer, and 11 patients (0.2%) underwent reoperation due to bowel necrosis. The study collected and analyzed clinical information on the demographics, presenting features, and outcomes of these cases.

Summary: This study aimed to clarify the features and risk factors for late-onset postoperative pneumonia (LOPP) after esophagectomy. The study found that clinical stage, postoperative hospital stay, and early postoperative decrease of psoas muscle area were independent risk factors for clinically relevant late-onset postoperative pneumonia (CR-LOPP).

Summary: This study systematically reviewed the literature on chyle leak after pancreatectomy and proposed a management algorithm based on the available evidence. The results showed that the extent of surgery is the most common predictor of risk, and conservative treatment has been shown to be effective in most cases.

Summary: This study is an international cohort study investigating the rates and management of anastomotic leak and conduit necrosis after esophagectomy. The results showed that the severity of AL and CN is closely related to patient outcomes, and reintervention can be successful, indicating that surgeons should try alternative management strategies.

Summary: The study investigated the incidence and risk factors for post-operative outcomes including chyle leak following minimally invasive esophagectomy. It found that robotic and VATS esophagectomy have similar rates of re-operation, length of stay, discharge needs, and complications. Differences in outcomes appear to be related to surgeon experience with the robot but may also be associated with specific techniques.

Summary: This study found that thoracoscopic esophagectomy has advantages in terms of operative duration and blood loss compared to open esophagectomy, with no significant difference in complication rates. The incidence of postoperative pneumonia was lower and recurrent laryngeal nerve paralysis was higher after thoracoscopic esophagectomy compared to open esophagectomy.

Summary: The risk of post-esophagectomy hiatal hernia is significantly higher after minimally invasive esophagectomy compared to the open technique. Future studies should focus on exploring preventive measures to reduce PEHH after minimally invasive esophagectomy.

Summary: This study aimed to assess the differences in failure to rescue (FTR) after anastomotic leak (AL) between different centers. The results showed that high-volume centers had lower FTR, which could be explained by lower leak severity, lower secondary ICU readmission rate, and higher availability of therapeutic modalities.
