Vitamin Analogues in Chemoprevention of Hepatocellular Carcinoma After Resection or Ablation-A Systematic Review and Meta-analysis

OBJECTIVE: While hepatic resection or local ablative therapy may provide a potentially curative treatment for hepatocellular carcinoma (HCC), more than half of these patients develop recurrent HCC within 5 years after treatment. Thus identification of any therapy which can decrease or delay the incidence of recurrence will improve the results of treatment. However, no chemopreventive agent has been approved for HCC. METHODS: A MEDLINE database, Embase, Cancerlit (National Cancer Institute), and CBM (Chinese Biomedical Database) search from 1990 to 2009 was performed to identify relevant articles using the keywords hepatocellular carcinoma, vitamin analogue, and chemoprevention. Additional papers were identified by a manual search of the references from the key articles. The fixed effect model was used for a meta-analysis. RESULTS: Oral administration of acyclic retinoids (vitamin A analogue), and menatetrenone (vitamin K-2 analogue) have been tested as chemopreventive agents after hepatic resection or local ablative therapy for HCC. There were one and four randomised, controlled trials (RCTs) which evaluated the efficacy of polyprenoic acid and menatetrenone, respectively. All studies were conducted in Japan. One RCT showed the preventive effect of polyprenoic acid in lowering the incidence of HCC recurrence after hepatic resection or percutaneous ethanol injection, and this effect lasted up to 199 weeks after randomization (or 151 weeks after completion of retinoid administration). Four RCTs evaluated the preventive efficacy of menatetrenone on HCC recurrence after hepatic resection or local ablative therapy. The results of three studies, as well as the meta-analysis of all four studies, showed significantly better tumour recurrence-free survival. The beneficial effect on the overall survival was less definite. CONCLUSION: There is evidence to suggest that chemopreventive therapy after partial hepatectomy or local ablative therapy is beneficial in prolonging disease-free survival, but the evidence is less for an effect on the overall survival. To confirm the beneficial role of vitamin A or K analogues in the chemoprevention of HCC further and larger randomised trials are now required. [Asian J Surg 2010;33(3):120-6]

All-Trans-Retinoic Acid Plus Oxaliplatin/Fluorouracil/Leucovorin for Advanced Hepatocellular Carcinoma with Pulmonary Metastasis: A Multicenter Retrospective Study

Summary: The study aimed to compare the impact of All-trans-retinoic acid (ATRA) plus FOLFOX4 with FOLFOX4 alone on advanced hepatocellular carcinoma (HCC) patients with pulmonary metastasis. The results showed that ATRA plus FOLFOX4 significantly improved survival outcomes and had better overall survival rates compared to FOLFOX4 alone. The most common acute toxicities were leukocytopenia, fatigue, anorexia, and nausea, but there was no significant difference in adverse events between the two groups.

A deep learning model with incorporation of microvascular invasion area as a factor in predicting prognosis of hepatocellular carcinoma after R0 hepatectomy

Summary: This study aimed to develop a deep learning prognostic prediction model by incorporating the maximal microvascular invasion (MVI) area as an additional factor to other independent risk factors for hepatocellular carcinoma (HCC) patients after R0 liver resection. The results showed that patients in the large-MVI group had worse overall survival (OS) compared to the small-MVI group. The deep learning model, based on independent risk factors including MVI stage, maximal MVI area, presence/absence of cirrhosis, and maximal tumor diameter, outperformed the traditional COX proportional hazards model in predicting OS.

Heterozygous disruption of beclin 1 alleviates neurotoxicity induced by sub-chronic exposure of arsenite in mice

Summary: Arsenite, a common neurotoxicant, affects neurological functions through mechanisms not fully understood. This study investigated the role of autophagy in arsenite neurotoxicity using beclin 1(+/+) and beclin 1(+/-) mice. The results showed that arsenite exposure caused cognitive impairment, anxiety-like behavior, and motor coordination disorder in beclin 1(+/+) mice, accompanied by morphological and electrophysiological changes in cortical tissues. Heterozygous disruption of beclin 1 attenuated these neurotoxic effects, suggesting that regulating autophagy through beclin 1 may be a potential therapeutic approach against arsenite neurotoxicity.

Effect of Acupuncture on Postoperative Pain in Patients after Laparoscopic Cholecystectomy: A Randomized Clinical Trial

Summary: This study aimed to evaluate the efficacy and safety of acupuncture compared to parecoxib sodium on postoperative pain, postoperative nausea and vomiting, and the Bruggemann Comfort Scale in patients following laparoscopic cholecystectomy. The results showed that the pain score in the acupuncture group was lower than the control group at 6 h and 9 h after the operation, and the number of patients requesting parecoxib sodium was also lower in the acupuncture group. The acupuncture group also had lower levels of postoperative nausea and vomiting at 6 h after the operation. Acupuncture may be a potential method for treating postoperative pain following laparoscopic cholecystectomy.

Summary: This multicentric study aimed to investigate the impact of tumor location and size on the difficulty of Laparoscopic- Left Hepatectomy (L- LH). The study found that tumors larger than 40 mm in diameter and located in PS Segment 4a were associated with the highest degree of technical difficulty. However, post-operative outcomes were not significantly different compared to smaller tumors or tumors located in other segments.

A reasonable identification of the early recurrence time based on microvascular invasion for hepatocellular carcinoma after R0 resection: A multicenter retrospective study

Summary: The definition of early recurrence for hepatocellular carcinoma (HCC) remains controversial. This study aimed to identify the early recurrence time for HCC and evaluate the efficacy of postoperative adjuvant TACE for HCC with MVI within this interval. The results showed that 13 months may be a reasonable early recurrence time point for HCC patients with MVI who underwent R0 resection, and within this interval, adjuvant TACE may result in longer survival.

The results showed that IMRT combined with atezo/bev demonstrated promising treatment efficacy and an acceptable safety profile for HCC patients with ePVTT, making it a promising option.
