Ethnogeographical structure of hepatitis B virus genotype distribution in Indonesia and discovery of a new subgenotype, B9

The distribution of hepatitis B virus (HBV) in the populations of island Southeast Asia is of medical and anthropological interest and is associated with an unusually high genetic diversity. This study examined the association of this HBV genetic diversity with the ethnogeography of the populations of the Indonesian archipelago. Whole genome analysis of 21 HBV isolates from East Nusa Tenggara and Papua revealed two recently reported HBV/B subgenotypes unique to the former, B7 (7 isolates) and B8 (5 isolates), and uncovered a further novel subgenotype designated B9 (4 isolates). Further isolates were collected from 419 individuals with defined ethnic backgrounds representing 40 populations. HBV/B was predominant in Austronesian-language-speaking populations, whereas HBV/C was the major genotype in Papua and Papua-influenced populations of Moluccas; HBV/B3 was the predominant subgenotype in the western half of the archipelago (speakers of the Western Malayo-Polynesian [WMP] branch of Austronesian languages), whereas B7, B8 and B9 were specific to Nusa Tenggara (Central Malayo-Polynesian (CMP)). The result provides the first direct evidence that the distribution of HBV genotypes/subgenotypes in the Indonesian archipelago is related to the ethnic origin of its populations and suggests that the HBV distribution is associated with the ancient migratory events in the peopling of the archipelago.

Summary: The 69th World Health Assembly endorsed the goal to eliminate hepatitis infection as a public health threat by 2030, followed by global targets for the care and management of hepatitis B and hepatitis C infections. However, tracking countries' progress towards these elimination goals revealed limitations of existing targets and recommended changes to simplify and adjust the targets for better impact and recognition of countries' efforts.

These non-coding RNAs (ncRNAs) show high stability in the blood and are being explored as possible diagnostic biomarkers for HCC, with further research also delving into their pathophysiological mechanisms in liver carcinogenesis.

Summary: This study evaluated the performance of two methods for detecting HCV viral load and found that the Xpert HCV VL test showed good sensitivity and specificity compared to the Roche HCV test, with a strong correlation and agreement between the two methods. The testing method was proven to be suitable for the diagnosis and treatment of hepatitis C, with the potential benefit for decentralized services in resource-limited areas.

Summary: This study developed a method to add complex-sugars to the aglycone 4-methylumbelliferone (4MU) to enhance their bioavailability and tumor cell intake. The sugar-modified rutinosyl-4-methylumbelliferone (4MUR) showed significant antiproliferative effect on liver tumor cells by targeting the asialoglycoprotein receptor (ASGPR) and reducing hyaluronan deposition, thereby demonstrating potential as an effective therapy for HCC.

Summary: This study aims to evaluate the burden of hepatitis C virus (HCV) in 2020 and forecast the burden by 2030. Using literature review, Delphi process, and mathematical modelling, it estimated the global prevalence of viraemic HCV infection to be 0.7% in 2020, corresponding to 56.8 million infections. The findings suggest that the current efforts are not on track to achieve the global elimination targets by 2030.

The Relevance of SOCS1 Methylation and Epigenetic Therapy in Diverse Cell Populations of Hepatocellular Carcinoma

Summary: The study revealed that SOCS1 methylation is significantly higher in HCC compared to peri-HCC and non-tumoral tissues. Treatment with 5-AZA reduced DNMT1 protein expression in different HCC cell subtypes, with the strongest effect observed in stromal subtype lines. The modulatory effect of 5-AZA on SOCS1 expression varied among cell types, with more efficient restoration in CSC HCC cells compared to sorafenib.

Summary: This study examines the HBV characteristics and potential risk of mother-to-child transmission among HBeAg-negative/HBsAg-positive expectant mothers in an area with a high prevalence of e-CHB. It highlights the importance of considering antiviral prophylaxis eligibility in situations with limited resources and in regions where e-CHB is prevalent, as some HBeAg-negative mothers may still pose a high transmission risk.

Network Analysis for the Discovery of Common Oncogenic Biomarkers in Liver Cancer Experimental Models

Summary: This study aimed to identify potential therapeutic targets for hepatocellular carcinoma (HCC) heterogeneity. In silico analysis identified 16 proto-oncogenes as potential pharmacological targets. In vitro experiments evaluated the effect of three treatment modalities on the expression of the proto-oncogene targets. The results showed that certain targets were consistently upregulated in HCC cells, and different treatments had varying effects on these targets, suggesting that targeting cellular heterogeneity may be important for HCC therapy.

Molecular Characterization of Influenza A/H3N2 Virus Isolated from Indonesian Hajj and Umrah Pilgrims 2013 to 2014

Summary: This study describes the influenza infection among Indonesian pilgrims upon arrival and the genetic characterization of imported influenza A/H3N2 virus. Results show that 39.5% of the samples tested positive for influenza virus, with no detection of MERS-CoV. Mutations in the HA gene mainly occurred within antigenic sites A, B, and D. No mutations related to oseltamivir resistance were observed in the NA gene. These findings suggest continuous mutation of the influenza A/H3N2 virus over time.

Summary: This study investigates the potential of a combination strategy using immunotherapy and epigenetic reprogramming against hepatocellular carcinoma (HCC). The results show that combination therapy of PD-L1 silencing and 5-AZA treatment can effectively inhibit the growth of HCC cells, indicating its potential use as a treatment for heterogeneous HCC.

Convalescent plasma therapy in patients with moderate-to-severe COVID-19: A study from Indonesia for clinical research in low- and middle-income countries

Summary: This study explored the outcomes of convalescent plasma (CP) treatment in patients with moderate and severe COVID-19 in Indonesia. The CP transfusion was well tolerated, and good clinical improvement was achieved in patients with moderate disease. Although most patients had detectable NAbs at baseline, it was inconclusive whether the outcomes were solely due to CP treatment. Further randomized controlled trials are needed for a better understanding.
