Summary: The use of chlorhexidine-alcohol for surgical site skin preparation was found to be more effective in reducing the risk of surgical site infections compared to iodophor-alcohol. However, the efficacy of chlorhexidine-alcohol may vary depending on the type of surgical procedure.

Summary: Probiotics or synbiotics can prevent infections, reduce delayed gastric emptying, and shorten the hospital stay and antibiotic-use durations in patients undergoing pancreaticoduodenectomy. These findings are clinically important for promoting recovery from pancreaticoduodenectomy, reducing the incidences of antibiotic resistance and iatrogenic infections, and reducing the medical burden.

Summary: The study suggests that prophylactic negative-pressure wound therapy (pNPWT) is protective against surgical site infection (SSI) after laparotomy, especially in cases where the incidence of SSI in the control group is high. However, more high-quality evidence is needed to confirm these findings.

Summary: The objective of this study is to evaluate the efficacy of strategies intended to prevent surgical site infection (SSI) after lower limb revascularization surgery. The results showed that preincision antibiotics and incisional negative-pressure wound therapy (iNPWT) can reduce the risk of early SSI, while the effects of other strategies on SSI risk are uncertain and require further confirmatory trials.

Summary: The use of high fraction of inspired oxygen (FiO(2)) intraoperatively for the prevention of surgical site infection (SSI) remains controversial. Published data lacks participant level details to test hypotheses. The purpose of this meta-analysis is to evaluate the benefits and harms of high FiO(2) and its effect modifiers.

Summary: This study aimed to report on successful characteristics and methodological strategies of pilot surgical and perioperative RCTs for optimizing recruitment. The study found that most pilot studies did not specify target sample size or recruitment rate, and there was minimal reporting on recruitment methods. By conducting qualitative evaluations of recruitment methods and involving the research team, recruitment strategies in perioperative and surgical RCTs can be optimized.

Summary: This study aimed to compare the effects of acupuncture and related techniques-assisted general anesthesia (GA) on the dosage of anesthetic drugs during surgery. A total of 76 RCTs were included in the analysis. The results showed that acupuncture-assisted GA significantly decreased the total dose of propofol and remifentanil administered, with electroacupuncture (EA) producing the greatest reduction. Overall, acupuncture techniques can be considered as a feasible option for reducing the dosage of anesthetic drugs during surgery.

Summary: Fear of medical liability is a major driver for broad administration of perioperative prophylactic anticoagulation, despite the persistently low rates of clinically symptomatic venous thromboembolism events (VTE) postoperatively. This study aimed to evaluate the medicolegal landscape of perioperative VTE and its pharmaceutical prophylaxis, and found that the main legal claims were failure to diagnose and treat VTE, and failure to prescribe/administer anticoagulation.

The results showed that UDD indeed increases the odds of developing HGD/IC, which is not accounted for by the current Fukuoka risk criteria. Summary: Background: Mucinous cystic neoplasms of the pancreas are rare tumors that can potentially develop into cancer. This study aimed to identify preoperative factors that can predict high-grade dysplasia or invasive carcinoma in these tumors. Methods: The study analyzed the data of 157 patients who had undergone resection for mucinous cystic neoplasms of the pancreas. Results: Tumor size >= 4 cm, mural nodularity, and a serum CA 19-9 level >37 U/mL were associated with high-grade dysplasia or invasive carcinoma. Tumors with a size >= 4 cm and/or a high CA 19-9 level should be surgically resected promptly, while tumors <4 cm with no other high-risk features may benefit from nonoperative surveillance.
