Osteoporosis prevalence and fracture characteristics in elderly female patients with fractures

The aim of this study was to characterize the features of fractures that occur in female patients with osteoporosis in the setting of a typical trauma hospital. Observational study was conducted on 233 women aged 50 years and above reporting to a trauma center with a fracture. Fracture location, fracture classification and cause of fracture were obtained and compared with bone mineral status assessed by DXA at the spine. Our findings indicate that with increasing patient age, the majority of fractures occur after a minor traumatic event. In more than one-third of all fractures after minor trauma, the patients were diagnosed with osteoporosis. The overall prevalence of osteoporosis in patients older than 50 years with fractures was more than 30% and was increasing with age. Most strikingly, one-third of the patients have had a previous fracture at a location that should have triggered the assessment of bone health status, but only 25% of them had received any form of osteoporosis treatment. For patients older than 50 years reporting to a trauma unit with a fracture, osteoporosis has a considerable prevalence. The large number of patients who have had a previous fracture emphasizes the necessity for an effective implementation of treatment algorithms for elderly patients with osteoporosis in the setting of a trauma hospital.

The results showed that supplemental cable cerclage significantly increased fixation stiffness and reduced interfragmentary movements, allowing immediate mobilization without weight-bearing restrictions from a biomechanical perspective.

Early biomechanical outcome in patients with acetabular fractures treated using the pararectus approach: a gait and stair climb analysis study

Biomechanical comparison of acetabular fracture fixation with stand-alone THA or in combination with plating

Summary: The purpose of this study was to compare the stabilization of anterior column acetabular fractures fixed with a cranial socket revision cup with flange and iliac peg or with a suprapectineal plate osteosynthesis combined with an additional revision cup. Results showed that both surgical approaches showed comparable fracture reduction in terms of initial construct stiffness and interfragmentary movement. The potential benefit of the less-invasive cranial socket revision cup needs further investigation in clinical studies.

Recovery of gait and function during the first six months after tibial shaft fractures

Three internal fixation methods for Danis-Weber-B distal fibular fractures: A biomechanical comparison in an osteoporotic fibula model

Summary: A common agreement for the surgical treatment of osteoporotic ankle fractures has not been defined yet. This study evaluated the mechanical stability of locked and conventional plates on osteoporotic Danis-Weber-B-fibula fracture models. The results showed that although no overall mechanical superiority was found, the locking plate groups exhibited higher shear and rotational stability compared to the neutralization plate group.

Recovery of the medial gastrocnemius muscle after calcaneus fracture differs between contractile and elastic components.

Summary: This study investigated the biomechanical and morphological adaptations of the gastrocnemius muscle after unilateral calcaneal fracture. The results showed that patients had an increase in muscle fascicle length and a decrease in the shortening of the serial elastic element. Compared to healthy controls, patients had differences in fascicle lengthening and pennation angle increase. Even at twelve months after surgery, there were still restrictions in the shortening behavior of the serial elastic element and muscle-tendon unit. This suggests that the dynamic morphological behavior of the medial gastrocnemius is not fully restored in patients with calcaneal fracture.

Summary: Surgical simulators are evolving educational tools for developing surgical skills, with virtual and hybrid simulators being preferred. A Smart Artificial Soft Tissue is presented to accelerate the revolution of hybrid simulators, providing objective positional feedback for haptically correct artificial soft tissue. The development of a laryngeal phantom demonstrates the capabilities of the simulator, which also uses a tablet computer for virtual representation and additional information display.

Risk of Interprosthetic Femur Fracture Is Associated with Implant Spacing-A Biomechanical Study

Summary: Ipsilateral revision surgeries for total hip or knee arthroplasties are increasing due to aging population, and the risk of interprosthetic fractures depends on the distance between adjacent implants. This study investigated the strain within the femoral shaft between two implants using a physical model and finite element model. Results showed that a smaller interprosthetic distance and thinner cortices increased strain peaks, while a minimum interprosthetic distance of 10 mm seemed crucial to avoiding strain accumulation. Interprosthetic fracture management is especially important for patients with reduced bone quality.

Contribution to the 3R Principle: Description of a Specimen-Specific Finite Element Model Simulating 3-Point-Bending Tests in Mouse Tibiae

Summary: The present study established and validated a finite element model (FEM) to predict the mechanical properties of mice tibiae. The FEM showed good correlation with biomechanical data obtained from bending tests. Infected nonunion: diagnostic and therapeutic work-up
