Summary: This study found that respiratory viruses play an important role in acute lower respiratory tract infections, especially in cases of bronchiolitis and pneumonia. In contrast, adenovirus, rhinovirus/enterovirus, and human coronaviruses are detected in both healthy children and children with ALRI. Furthermore, these viruses are also commonly detected in patients undergoing surgery during the same respiratory season.

Summary: Patients diagnosed with environmental/occupational bronchiolitis obliterans (BO) often present with a slowly progressing respiratory symptom evolution, without acute exposure to toxins. Exertional dyspnea is the most common symptom, and standard evaluation may not be diagnostic. Lung biopsies commonly reveal pathological abnormalities in all distal lung compartments. The diagnosis of environmental/occupational BO should be considered in patients with subacute exertional dyspnea and a history of prolonged or recurrent exposure to environmental toxins. Important future areas of research include less invasive diagnostic approaches and testing of new agents for prevention and treatment.

Summary: According to the recommendation of the American Academy of Pediatrics, routine use of bronchodilators for infants with bronchiolitis does not show significant effects. From 2010 to 2018, the use of bronchodilators, hospital admissions, and emergency department visits decreased, while ICU admissions and respiratory support increased.

Summary: This study aimed to investigate whether 3% NaCl triggers inflammation in human macrophages and identify the molecular mechanisms involved. The results showed that 3% NaCl induces inflammatory responses in human macrophages through NLRP1 and inflammasome assembly. Targeting inflammation in addition to inhalation with hypertonic saline may benefit patients with inflammatory airway disease.

Summary: Viruses as biomaterials exhibit minimal toxicity, good biocompatibility, and well-defined structures. They have been utilized in various applications such as targeted therapy, antimicrobial agents, tissue regeneration, disease detection, bioimaging, vaccines, and immunotherapy. This review discusses the exciting applications and the challenges and future directions of using virus-based biomaterials.

Summary: This study found that nebulized magnesium sulfate as a bronchodilator in young children aged 1-24 months with moderate to severe bronchiolitis did not show superiority over standard therapy. There were no significant differences between the groups in terms of improvement in bronchiolitis severity score and length of hospitalization. No adverse events were observed in either group.

Summary: This study examined the performance of models predicting waitlist and posttransplant survival in lung transplant allocation. The results showed that different models had similar performance in predicting waitlist and posttransplant survival, and using alternative modeling strategies and contemporary cohorts did not improve the models' performance in determining access to lung transplant.

Summary: Acute Viral Bronchiolitis is a common cause of hospitalization for infants, with international guidelines recommending minimal therapeutic approach and not the use of bronchodilators. However, growing evidence suggests that bronchiolitis has different types and some are strongly associated with asthma features and higher risk for asthma development. A bronchodilator-trial using short-acting beta 2 agonists with metered-dose inhalers and valved holding chambers seems appropriate for children over 6 months old with bronchiolitis while waiting for more research on the relationship between therapeutic options and different phenotypes.

Summary: The social distancing measures during COVID-19 led to a reduction in respiratory virus infection rates, which correlated with less decline in lung function, fewer episodes of TDLF, and possibly lower incidence of CLAD in lung transplant recipients.

Summary: Pathogenic enteric viruses are a significant cause of illness and death, especially in children in developing countries. The intestinal immune system plays a critical role in the mucosa, balancing protection against pathogens and tolerance to commensal bacteria and food. Understanding intestinal immune mechanisms can lead to improved vaccine development.

Summary: This study retrospectively presents our experience with intravenous immunoglobulin (IVIG) treatment in patients with postinfectious bronchiolitis obliterans (PIBO) who had a clinically severe course despite receiving commonly used treatment protocols. The results showed that patients who underwent IVIG therapy had significantly reduced hospital visits and frequency of hospitalizations, discontinued oxygen therapy, and showed improvements in radiological severity scores and BMI z-scores.
