Relationship Between the Neutrophil to Lymphocyte Ratio and Parathyroid Adenoma Size in Patients With Primary Hyperparathyroidism

The aim of our study was to evaluate the relationship between neutrophil to lymphocyte ratio (NLR) and adenoma size in parathyroidectomized patients who underwent a parathyroidectomy. The neutrophil to lymphocyte ratio has recently become popular as a biomarker for malignant diseases or for estimating tumor size preoperatively. This study aimed to estimate the relationship between adenoma size and NLR. Furthermore, we assessed whether a higher level of NLR is correlated with the presence of parathyroid carcinoma. A retrospective chart review was performed for patients with parathyroid adenoma who underwent parathyroidectomy between January 2012 and August 2014. Data related to age, sex, NLR, parathyroid hormone level (PTH), preoperative calcium, phosphorus, adenoma size, and pathology reports were collected. The neutrophil to lymphocyte ratio was significantly correlated with calcium levels, PTH levels, parathyroid adenoma size, and the presence of cancer. However, there was no correlation between NLR and age, sex, and phosphorus levels. This study is the first to document a positive correlation between NLR and parathyroid adenoma size, as well as the presence of cancer, in patients who underwent surgery as a result of primary hyperparathyroidism.

SARS-CoV-2 infection and venous thromboembolism after surgery: an international prospective cohort study

Summary: Surgical patients with peri-operative or recent SARS-CoV-2 infection are at increased risk of postoperative venous thromboembolism compared to those without a history of SARS-CoV-2. However, the optimal prophylaxis and treatment for venous thromboembolism in this patient population remains unknown. Venous thromboembolism is independently associated with 30-day mortality.

Summary: This study evaluated the changes in care and short-term outcomes of surgical patients with gynecologic cancers during the COVID-19 pandemic. It found that about 20.7% of patients had their care adjusted and about 11.2% experienced a delay in surgery of more than 8 weeks, which was associated with adverse outcomes. About 7.9% of patients did not receive their planned operations, with a mortality rate of about 2.7% and about 18% experiencing disease progression or death within 3 months of the decision for surgery by the multidisciplinary team.

Summary: This study investigated the impact of the SARS-CoV-2 pandemic on emergency surgery. The results showed that the pandemic continues to have a negative impact on the treatment and outcome of emergency surgery patients. The problems with in-hospital logistics have not been adequately resolved, and there has been a dramatic worsening of medical staff shortages and reduced capacities over the past two years.

Summary: This study developed and validated a novel index (SPI) to support the strengthening of local elective surgical systems and address growing backlogs. The findings showed that hospitals with a higher SPI were associated with an increased planned surgical volume ratio, independent of income status, COVID-19 burden, and hospital type. Annual self-assessment of surgical preparedness is recommended to identify areas for improvement and enhance the resilience of local surgical systems.

Diagnostic Accuracy Rates of Appendicitis Scoring Systems for the Stratified Age Groups

Summary: The predictive values of scoring systems for acute appendicitis vary in different age groups. Age should be considered when selecting a scoring system for patients with a prediagnosis of acute appendicitis. The Alvarado and Fenyo-Linberg scoring systems are the most accurate for the 18-45 and 46-65 age groups, respectively, while the Eskelinen scoring system is the most accurate for the >65 age group, but may not be suitable for use alone.

Summary: The incidence of gangrenous cholecystitis is higher in COVID patients compared with non-COVID patients with acute cholecystitis. Gangrenous cholecystitis in COVID patients is associated with high-grade Clavien-Dindo postoperative complications, longer hospital stay, and higher mortality rate. COVID-19 infection and gangrenous cholecystitis are not absolute contraindications for laparoscopic cholecystectomy and should be evaluated on a case-by-case basis by experts.

The results show that there are technology enthusiasts and skeptics among surgeons, with a preference towards more classical decision-making aids. Lack of knowledge about AI and mistrust are also observed.

Summary: This study explores the understanding, barriers, and facilitators of shared decision-making (SDM) in trauma and emergency surgery. Findings show that less than half of the surgeons understand SDM and 30% still see value in exclusively engaging multidisciplinary provider teams without involving the patient. The inclusion of SDM practices in clinical guidelines is recommended.

Should multifocality be an indication for prophylactic central neck dissection in papillary thyroid cancer?

Summary: Although multifocality is common in papillary thyroid cancer, its impact on lymphatic metastasis and the necessity of central dissection are still debated. In our study of 258 patients who underwent thyroidectomy, lymph node metastasis was not significantly increased in the presence of multifocality. However, bilateral multifocal tumors exhibited more aggressive features and a higher risk for central lymph node metastasis. Prophylactic central lymph node dissection could be considered in patients with multifocal tumors and no preoperative or intraoperative lymph node metastasis.

COVID-19 and related gastrointestinal symptoms: An observational study

Summary: Gastrointestinal symptoms are common in COVID-19 patients, with nonspecific abdominal pain and diarrhea being the main manifestations. These symptoms do not seem to have a detrimental effect on the progression of the disease, and surgical intervention is not necessary.
