In a retrospective review of 24 patients with histoplasmosis, blastomycosis, or coccidioidomycosis treated with voriconazole (most for salvage therapy), the outcome was favorable (improved or stable) for 22 (95.8%) within 2 months of starting voriconazole and for 20 (83.3%) at the last follow-up. Prospective studies are required to determine its role in the treatment of endemic mycoses.

Clinical Features of Human Metapneumovirus-Associated Community-acquired Pneumonia Hospitalizations

Leigh M. Howard, Kathryn M. Edwards, Yuwei Zhu, Carlos G. Grijalva, Wesley H. Self, Seema Jain, Krow Ampofo, Andrew T. Pavia, Sandra R. Arnold, Jonathan A. McCullers, Evan J. Anderson, Richard G. Wunderink, Derek J. Williams

Summary: Clinical features did not reliably distinguish HMPV-associated pneumonia from other pathogens. HMPV-associated pneumonia was less severe than bacterial and adult RSV pneumonia, but was otherwise as or more severe than other common pathogens.

Summary: The study found that PIV was more commonly detected in children, but often co-detected with other pathogens. Clinical features of PIV-1, PIV-2, and PIV-3 infections were similar in children and adult pneumonia patients. In children, PIV pneumonia was less severe than bacterial pneumonia.

Summary: The study found that the susceptibility of AmpC organisms to TGC was negatively correlated with TGC utilization, and limiting TGC exposure to <= 48 hours may help reduce resistance of AmpC organisms in future infections.

Summary: This study investigated the geospatial patterns of community-acquired pneumonia in children and found an association between PM2.5 exposure and high-risk areas for pneumonia. Children with private insurance and bacterial infections had a decreased risk of residing in high-risk areas for pneumonia.

Summary: This clinical practice guideline for the diagnosis and treatment of acute hematogenous osteomyelitis in children was developed by a multidisciplinary panel representing PIDS and IDSA. The recommendations are based on evidence from systematic literature reviews and follow a standardized methodology for rating the certainty of the evidence and strength of recommendation using the GRADE approach.

Analysis of Community-Acquired Urinary Tract Infection Treatment in Pediatric Patients Requiring Hospitalization: Opportunity for Use of Narrower Spectrum Antibiotics

Jeremy S. Stultz, Natalie Francis, Samantha Ketron, Bindiya Bagga, Chasity M. Shelton, Kelley R. Lee, Sandra R. Arnold

Summary: The study found that using narrow-spectrum antibiotics for pediatric UTI treatment is an appropriate choice, and factors such as age, circumcision status, ethnicity, and recent hospitalization are associated with antibiotic susceptibility. In addition, factors like urine white blood cell count and having only one narrow-spectrum treatment option are related to the use of unnecessarily broad-spectrum antibiotics for treatment.
