Two Pseudomonas aeruginosa urine isolates from Bulgaria and Germany produced two new VIM-2 variants. VIM-15 had one amino acid substitution (Tyr218Phe) which caused a significant increase in hydrolytic efficiency. The substitution Ser54Leu, characterizing VIM-16, showed no influence on enzyme activity. Both genes were part of class I integrons located in the chromosome.

Investigation of multidrug-resistant Helicobacter pylori in pediatric patients: A Bulgarian study and literature data

Molecular epidemiology, virulence and antimicrobial resistance of Bulgarian methicillin resistant Staphylococcus aureus isolates

Summary: This study investigated the clonal spread, virulence, and antimicrobial resistance rates of Bulgarian MRSA isolates between 2016 and 2020. The results showed the presence of different clones and ST types among the MRSA isolates, with CC5 isolates exhibiting higher virulence potential and almost all being macrolide resistant, while CC8 isolates showed higher levels of resistance. This study is the first to describe the clonal spreading of MRSA in Bulgaria and its association with virulence and resistance determinants, highlighting the importance of monitoring and understanding MRSA epidemiology, resistance, and virulence profiles for better prevention and treatment choices in severe infections.

Detection of different colistin resistance mechanisms among multidrug resistant Klebsiella pneumoniae isolates in Bulgaria

Summary: This study aimed to reveal the prevalence and mechanisms of colistin resistance among multidrug-resistant Klebsiella pneumoniae isolates collected in Bulgaria. It was found that carbapenemases and mgrB alterations played a key role in colistin resistance, particularly in the ST11 clone. These findings are of great importance for the control of the spread and development of colistin-resistant strains.

Increase in amoxicillin resistance in Helicobacter pylori from Bulgarian patients over 15 years

Dissemination of High-Risk Clones Enterobacterales among Bulgarian Fecal Carriage Isolates

Summary: The gastrointestinal tract serves as a significant reservoir for high-risk Enterobacteria clones, which contribute to antimicrobial resistance in hospitals. This study conducted in Bulgaria identified various cefotaxime-resistant Enterobacterales isolates in fecal samples of patients, including ESBL, carbapenemase, and plasmidic AmpC producers. High-risk clones of Klebsiella pneumoniae and Escherichia coli were detected, indicating the importance of routine screenings and infection control measures, particularly amid the COVID-19 pandemic.

Geographic distribution of antibiotic resistance of Helicobacter pylori: A study in Bulgaria

Summary: This study compared the prevalence of antibiotic resistant Helicobacter pylori between patients living outside the capital city and capital city residents. The resistance rates were different for different antibiotics, with higher rates observed in the capital city for levofloxacin. The results suggest the need for careful antibiotic consumption control and restriction of unjustified use of levofloxacin, particularly in large cities.

Evolution of Helicobacter pylori Resistance to Antibiotics: A Topic of Increasing Concern

Summary: Antibiotic resistance among Helicobacter pylori strains varies across countries and over time. Resistance to different antibiotics, such as amoxicillin, metronidazole, clarithromycin, tetracycline, and levofloxacin, shows diverse trends in different countries. Factors such as non-recommended regimens, national antibiotic consumption, patient compliance, host factors, strain virulence, migrations, and azithromycin overuse during the COVID-19 pandemic can influence resistance evolution. New drugs, eradication regimens, and diagnostic methods, such as next-generation sequencing, have the potential to improve H. pylori infection control.

Anaerobes in specific infectious and noninfectious diseases: new developments

Summary: Through new research data, the associations between anaerobic bacteria and diseases/conditions were evaluated. The aim of the review was to draw attention and encourage further multidisciplinary studies. Microbiome-disease correlations including IBD, cardiovascular diseases, Alzheimer's and Parkinson's disease, neurodegenerative diseases, severe Clostridioides difficile infections, and various cancers were discussed. However, there are controversies and more studies are needed. The present data enriches knowledge and guides future research directions.

Clostridioides difficile, a New Superbug

Summary: Clostridioides difficile is a Gram-positive, spore-forming, anaerobic bacterium that causes a range of clinical features, from asymptomatic carriage to severe pseudomembranous colitis. The infections are associated with disruption of the gut microbiota caused by antimicrobial agents.

Clostridioides difficile resistance to antibiotics, including post-COVID-19 data

Summary: This study summarizes the data on Clostridioides difficile antibiotic resistance from 20 countries, showing variations in resistance rates for different drugs and high levels of resistance and multidrug resistance in some regions. Regular monitoring of C. difficile antibiotic susceptibility is recommended, and the use of fidaxomicin or vancomycin for CDI treatment is recommended.

Summary: This study in Bulgaria focused on the presence of msr(D) gene among Streptococcus pyogenes isolates and its association with macrolide resistance and virulence factors. The co-existence of mef(A/E) and msr(D) was found in all macrolide-nonsusceptible strains, with emm1 and emm4 being the most common emm genotypes.

Summary: This study evaluated the epidemiological relationship between carbapenem-resistant (CR) and/or 3rd generation cephalosporin resistant Klebsiella pneumoniae blood isolates collected from patients in the University Hospital in Varna city during the period January 2014 - December 2017. The study also aimed to characterize the ESBLs and carbapenemase production in these isolates. The findings revealed a wide intrahospital dissemination of KPC-2 and/or CTX-M-15 producing Klebsiella pneumoniae, with ST15 persisting throughout the study period, demonstrating high cross transmission, epidemic and invasive potential.

HIGH LEVEL OF COLISTIN RESISTANT GRAM-NEGATIVE BACTERIA IN A UNIVERSITY HOSPITAL IN BULGARIA

Summary: The study aimed to assess colistin susceptibility in 200 multidrug resistant Gram-negative bacteria from Alexandrovska University Hospital in Sofia, Bulgaria. It found that 16.5% of the tested strains were colistin resistant, with the highest rates observed in Klebsiella pneumoniae. Carbapenem resistance in Enterobacteriaceae spp. was mainly mediated by carbapenemase production, and NDM-positive and KPC-producing strains were identified in significant numbers.
