The value of occult disease in resection margin and lymph node after extrapleural pneumonectomy for malignant mesothelioma

Background. The purpose of this study was to examine the prognostic impact of occult disease after extrapleural pneumonectomy for malignant mesothelioma. Methods. We reviewed the resection margin and node specimens from 41 consecutive patients undergoing extrapleural pneumonectomy for malignant pleural mesothelioma in different institutions between 1985 and 2004. The specimens were reassessed by immunohistochemical staining with anticalretinin and antimesothelin monoclonal antibodies, and results were used to draw Kaplan-Meier survival curves and perform Cox regression analyses. Results. Histologic examination showed 34 epithelioid, 4 biphasic, and 3 sarcomatoid subtypes. Results of postoperative TNM staging were that 14 patients were in stage I, 6 were in stage II, and 21 were in stage III. One patient died during the early postoperative period. Median survival was 13 months. Survival was affected by nonepithelial histologic type (p = 0.001), TNM stage (p = 0.007), positive resection margins (p = 0.002), and N disease ( p = 0.01). Immunohistochemistry revealed occult positive resection margins in 6 patients, not correlated with T stage. Microscopic N disease was discovered in 5 patients, of whom 2 had their nodes retrieved through cervical mediastinoscopy. No correlation with nodal diameter was found. In all patients microscopic N disease could have been accessible through mediastinoscopy. Overall, the presence of occult disease was diagnosed in 5 new patients and influenced survival more than any other variable, both at univariate (p < 0.001) and multivariate Cox regression analysis (p < 0.0001; odds ratio, 5.4; 95% confidence interval, 3 to 15). Conclusions. In malignant pleural mesothelioma, the presence of occult disease in resection margins and lymph nodes can be identified by immunohistochemistry and significantly influences the prognosis. Cervical mediastinoscopy is useful in all patients considered for radical resection, but all specimens should be processed with immunohistochemical staining. Endometriosis: A Retrospective Analysis on Diagnostic Data in a Cohort of 4,401 Patients

Summary: Endometriosis is a prevalent gynecological disease with uncertainties in its pathogenesis, diagnosis, and therapy. This study analyzed a large number of patients' clinical data and diagnostic results, revealing that MRI has high sensitivity and specificity for ovarian lesions, while laparoscopy is more reliable for detecting peritoneal lesions. Clinical symptoms are crucial for diagnosing deep lesions, and certain biomarkers may have value in monitoring patients' condition.

Novel insights into the BAP1-inactivated melanocytic tumor

Summary: BAP1-inactivated melanocytic tumors (BIMT) are characterized by loss of function of the BAP1 gene and often contain BRAF mutations. These tumors show extreme cytomorphological heterogeneity with distinctive morphological variants, and may also exhibit underrecognized microscopic features. Our study identified several novel BAP1 mutations and revealed a progression of BIMTs from conventional BRAF-mutated nevi to epithelioid BAP1-inactivated melanocytes.

In this study, AI analysis of radiomic features achieved a high predictive value for malignancy in elderly patients with SPNs, reducing unnecessary surgery.

Further studies are needed to understand the mechanisms involved and develop therapeutic strategies.

Dermatoscopic, Histological and Confocal Microscopic Analysis of a Kissing Nevus of the Penis

Summary: Kissing nevus is a rare congenital melanocytic neoplasm that occurs in areas of embryological split, such as the eyelid or penis. This case study describes the dermatoscopic, histological, and confocal microscopic analysis of a new case of kissing nevus of the penis in a 57-year-old man. The findings revealed characteristic features of the nevus and suggested cellular activity through confocal microscopy. A conservative treatment approach was adopted, with a planned clinical follow-up after six months.

Adjuvant Transthoracic Negative-Pressure Ventilation in Nonintubated Thoracoscopic Surgery

Summary: In nonintubated thoracoscopic-surgery, adjuvant transthoracic negative-pressure ventilation is effective in reducing the risks of barotrauma. In patients with severe emphysema or interstitial lung disease, this method improves postoperative lung re-expansion and oxygen saturation. The results show better outcomes in oxygenation measures and lung expansion with transthoracic NPV.

Acute Foot Drop Caused by Intraneural Ganglion Cyst of the Peroneal Nerve: Literature Review and Case Report

Summary: This study reports a clinical case of acute non-traumatic foot drop caused by peroneal intraneural ganglion and reviews the methodological management of this disease. The results show that careful clinical examination and differential diagnosis can effectively manage acute foot drop caused by peroneal intraneural ganglion and achieve satisfactory motor recovery through surgery and rehabilitation.

Endometriosis: A Retrospective Analysis of Clinical Data from a Cohort of 4,083 Patients, With Focus on Symptoms

Summary: This retrospective study on 4,083 patients with endometriosis revealed that chronic fatigue syndrome and dyspareunia are among the main symptoms of the disease, while urinary disorders and chronic pelvic pain have a low occurrence. The study also found an increase in symptom incidence from the age group of 25-29 years to 40-44 years, followed by a significant decrease in the age group of 55-59 years. Additionally, employers, graduates, and freelancers had a higher incidence of symptoms, and the number of gestations showed a protective effect on chronic fatigue syndrome.
