Estimating nocturnal stroke onset times by magnetic resonance imaging in the WAKE-UP trial

Bastian Cheng, Hans Pinnschmidt, Alina Koenigsberg, Eckhard Schlemm, Florent Boutitie, Martin Ebinger, Matthias Endres, Jochen B. Fiebach, Jens Fiehler, Ivana Galinovic, Robin Lemmens, Keith W. Muir, Salvador Pedraza, Josep Puig, Claus Z. Simonsen, Vincent Thijs, Anke Wouters, Christian Gerloff, Gotz Thomalla

Summary: Fluid-attenuated inversion recovery (FLAIR) sequences are increasingly used to guide stroke treatment in patients with unknown onset time. This study aimed to estimate stroke onset during night-sleep using relative signal intensities FLAIR in acute stroke lesions. Results suggest that nocturnal strokes may predominantly occur during the early morning hours, consistent with diurnal patterns of cardiovascular events.

High-Sensitivity Cardiac Troponin T and Cognitive Decline in Older Adults: Results of the Berlin Aging Study II

Regina von Rennenberg, Thomas Liman, Christian H. Nolte, Alexander H. Nave, Jan F. Scheitz, Sandra Duezel, Vera Regitz-Zagrosek, Denis Gerstorf, Elisabeth Steinhagen-Thiessen, Ilja Demuth, Matthias Endres

Summary: There is evidence of an association between levels of high-sensitivity cardiac troponin T (hs-cTnT) and cognitive decline in older men, as measured by the Digit Symbol Substitution Test (DSST). This association remained significant even after adjusting for age, sex, education, and cardiovascular risk factors. However, there was no significant association between hs-cTnT and different cognitive domains at baseline.

Elevation of cardiac biomarkers in stroke is associated with pathological findings on cardiac MRI-results of the HEart and BRain interfaces in Acute Stroke study

Regina von Rennenberg, Juliane Herm, Thomas Krause, Simon Hellwig, Helena Stengl, Jan F. Scheitz, Thomas Elgeti, Sebastian N. Nagel, Matthias Endres, Karl Georg Haeusler, Christian H. Nolte

Summary: This study found that elevated cardiac biomarkers, such as high-sensitivity cardiac troponin T (hs-cTnT) and brain natriuretic peptide (BNP), in acute stroke patients are strongly associated with pathological findings on cardiac magnetic resonance imaging (CMR). The elevation of these biomarkers may help identify patients who require a more comprehensive cardiology work-up.

Zfp580 Regulates Paracrine and Endocrine Igf1 and Igfbp3 Differently in the Brain and Blood After a Murine Stroke

Christian J. Hoffmann, Melanie T. C. Kuffner, Janet Lips, Stephanie Lorenz, Matthias Endres, Christoph Harms

Summary: This study demonstrated the differential control of paracrine and endocrine Igf1 and Igfbp3 by Zfp580 after stroke, suggesting that inhibition of Zfp580 might be a new treatment target leading to increased activity of Igf1 to improve stroke outcome.

Summary: The global burden of small vessel disease (SVD) predicts cognitive and functional outcomes in stroke patients, but the current score used for assessment does not improve prediction capability. Assessing the severity of SVD lesions adds value in predicting outcomes beyond known predictors.

Personalized knowledge to reduce the risk of stroke (PERKS-International): Protocol for a randomized controlled trial

Seana L. Gall, Valery Feigin, Amanda G. Thrift, Timothy J. Kleinig, Dominique A. Cadilhac, Derrick A. Bennett, Mark R. Nelson, Tara Purvis, Shabnam Jalili-Moghaddam, Gemma Kitsos, Rita Krishnamurthi

Summary: This study aims to reduce stroke risk through a personalized mobile app. The study will compare the difference in modifiable cardiovascular risk scores between a group using the app and a group receiving usual care, and assess various secondary outcomes.

Summary: Current guidelines do not recommend screening for asymptomatic carotid artery stenosis (AsxCS) due to the potential risks associated with unnecessary interventions. Conversely, screening for abdominal aortic aneurysms is strongly recommended.

Carotid Vessel-Wall-Volume Ultrasound Measurement via a UNet plus plus Ensemble Algorithm Trained on Small Data Sets

Ran Zhou, Fumin Guo, M. Reza Azarpazhooh, J. David Spence, Haitao Gan, Mingyue Ding, Aaron Fenster

Summary: In this study, a UNet++ ensemble approach was developed for automated vessel wall volume (VWV) measurement, trained on five small data sets and tested on 100 participants with coronary artery disease. The results showed that our approach could provide accurate and reproducible carotid VWV measurements using relatively small training data sets, supporting deep learning applications for monitoring atherosclerosis progression.

Secondary prevention by structured semi-interactive stroke prevention package in INDIA (SPRINT INDIA): Findings from the process evaluation of a randomized controlled trial

Shweta Jain Verma, Puja Gulati, Ranjit J. Injety, Deepti Arora, Aneesh Dhasan, Anusha Singhania, Himani Khatter, Meenakshi Sharma, Padmavathyamma Narayanapillai Sylaja, Jeyaraj Durai Pandian

Summary: The trial in India aimed to provide secondary stroke prevention intervention to sub-acute stroke patients using workbooks, videos, and SMS, but it did not lead to a reduction in vascular events. Through process evaluation, it was found that there was good fidelity to intervention, but inclusion criteria were diluted and patient motivation to view the intervention decreased over time. The results suggest that the intervention was acceptable but improvements are needed.

Ultrasound carotid plaque segmentation via image reconstruction-based self-supervised learning with limited training labels

Ran Zhou, Yanghan Ou, Xiaoyue Fang, M. Reza Azarpazhooh, Haitao Gan, Zhiwei Ye, J. David Spence, Xiangyang Xu, Aaron Fenster

Summary: This article proposes an image reconstruction-based self-supervised learning algorithm (IR-SSL) for carotid plaque segmentation when few labeled images are available. Through evaluation on two independent datasets, it was found that IR-SSL improved segmentation performance when trained on limited labeled images and showed strong correlation with manual results.
