Summary: This retrospective study aimed to identify predictors of interstitial lung disease (ILD) in patients with mixed connective tissue disease (MCTD). The study found that gastroesophageal involvement and lymphadenopathy at disease onset were associated with ILD. Additionally, it established that lymphadenopathy at disease onset and older age at diagnosis independently predicted ILD in MCTD patients.

Summary: This nationwide multicentre study prospectively evaluated 376 patients with IIP, finding that 18.6% of them met the IPAF criteria. Patients with IPAF were more likely to develop systemic autoimmune diseases, had fewer acute exacerbations of IIP, and showed better survival outcomes. Cluster analysis identified a subgroup with more autoimmune features, predominantly female, and with a better prognosis.

Summary: MCTD is a rare and complex autoimmune disease with high levels of anti-U1 small nuclear ribonucleoprotein 70k autoantibodies. Basophils and IgE play a role in MCTD pathophysiology and may serve as therapeutic targets for patients. Basophil activation and autoreactive IgE against U1 small nuclear ribonucleoprotein 70k were observed in both patients and an MCTD-like mouse model.

Summary: Diaphragm ultrasound can be used to assess diaphragm function in patients with ILD. The prevalence of diaphragmatic dysfunction was 29% in patients with ILD and it was associated with dyspnea. CTD-ILD had lower diaphragm displacement compared to IPF and a higher prevalence of diaphragmatic dysfunction compared to controls.

Summary: Elevated levels of serum IL-6, IL-8, and TNF-alpha were associated with the complication of ILD in CTD patients and the occurrence of AE in CTD-ILD patients. IL-6 could be a promising serum biomarker of severity and the occurrence of AE in CTD-ILD patients. The combination of the three variables (IL-6 level, TBil and CD3 + CD4 + T cells) predicted the AE-CTD-ILD better.

Summary: The clinical presentation of primary Sjogren's syndrome (pSS) varies widely. A multicentre study identified two patient subgroups based on the presence of SSA/SSB antibodies and genetic markers in the HLA region, with strong association signals found in HLA-DQA1 locus for the antibody-positive subgroup. Replication confirmed the association with MHC class I and II variants, indicating distinct clinical manifestations in pSS subgroups.
