Summary: In recent years, the use of combination therapy has increased in patients with pulmonary arterial hypertension, but most patients still receive monotherapy. There was no change in the one-year survival rate over time, but there was a slight, nonsignificant improvement in the three-year survival rate.

COMPERA 2.0: a refined four-stratum risk assessment model for pulmonary arterial hypertension

Marius M. Hoeper, Christine Pausch, Karen M. Olsson, Doerte Huscher, David Pittrow, Ekkehard Gruenig, Gerd Staehler, Carmine Dario Vizza, Henning Gall, Oliver Distler, Christian Opitz, J. Simon R. Gibbs, Marion Delcroix, H. Lange, Stephan Rosenkranz

Summary: The study investigated a modified risk stratification approach based on four risk categories, using refined cutoff levels for functional class, 6MWD, and BNP/NT-proBNP. The four-stratum model was found to be more sensitive in predicting long-term survival and prognostic changes compared to the original three-stratum model.

Swift, Alfred A. Roger Thompson, Charlie A. Elliot, Stephan Rosenkranz, Robin Condliffe, David G. Kiely, Michael Halank

Summary: This study provides a detailed characterization of patients meeting diagnostic criteria for idiopathic pulmonary arterial hypertension (IPAH) and identifies a distinct lung phenotype characterized by low diffusion capacity for carbon monoxide (DLCO) and smoking history. These patients resemble those with pulmonary hypertension due to lung disease rather than classical IPAH. These findings have important implications for understanding the pathogenesis, diagnosis, and treatment of the disease and warrant further exploration.

Ghofrani, Ralf Ewert, Han -Joachim Kabitz, Dirk Skowasch, Juergen Behr, Katrin Milger, Michael Halank, Heinrike Wilkens, Hans-Juergen Seyfarth, Matthias Held, Laura Scelsi, Claus Neurohr, Anton Vonk-Noordegraaf, Silvia Ulrich, Hans Klose, Martin Claussen, Stephan Eisenmann, Kai-Helge Schmidt, Bjoern Andrew Remppis, Andris Skride, Elena Jureviciene, Lina Gumbiene, Skaidrius Miliauskas, Judith Loeffler-Ragg, Tobias J. Lange, Karen M. Olsson, Marius M. Hoeper, Christian Opitz

Summary: This study analyzed the response to PAH therapy in IPAH patients and compared the differences between patients without comorbidities, those with 1-2 comorbidities, and those with 3-4 comorbidities. The results showed that patients with comorbidities had smaller improvements in functional class, 6-minute walking distance, BNP/NT-pro-BNP, and mortality risk compared to patients without comorbidities. However, the 4-strata risk tool predicted the outcome in IPAH patients regardless of the presence of comorbidities.

Summary: The prevalence and characteristics of SSc-associated interstitial lung disease (SSc-ILD) vary between geographical regions. This study aimed to explore the differences in prevalence, phenotype, treatment, and prognosis in patients with SSc-ILD from different regions in the EUSTAR database. The results showed significant variations in clinical characteristics, survival rates, and treatment approaches among different regions.

Comprehensive autoantibody profiles in systemic sclerosis: Clinical cluster analysis

Jakob Hoeppner, Christoph Tabeling, Vincent Casteleyn, Claudia Kedor, Wolfram Windisch, Gerd Ruediger Burmester, Doerte Huscher, Elise Siegert

Summary: The aim of this study was to comprehensively analyze the serum autoantibody status in patients with systemic sclerosis (SSc) and correlate it with the clinical course of the disease. The study found that specific autoantibodies are associated with specific clinical manifestations, while the association of some rare antibodies is not fully clarified. The results of this study reveal important associations between the serologic status of SSc patients and disease manifestation, comorbidities, and complications.

Association of kidney function and albuminuria with frailty worsening and death in very old adults

Nina Mielke, Alice Schneider, Muhammad Helmi Barghouth, Natalie Ebert, Markus van der Giet, Doerte Huscher, Martin K. Kuhlmann, Elke Schaeffner

Summary: This study investigates the association between kidney function decline and albuminuria with frailty worsening or death in very old adults. The findings suggest that both advanced stages of CKD and albuminuria are independently associated with an increased risk of frailty worsening and death in older adults.

Michael Borte, Leif G. G. Hanitsch, Nizar Mahlaoui, Maria Fasshauer, Dorte Huscher, Matthaios Speletas, Maria Dimou, Marta Kamieniak, Corinna Hermann, David Pittrow, Cinzia Milito

Summary: The FIGARO study aims to provide insights on the real-world utilization and tolerability of fSCIG for PID and SID patients. This prospective, multicenter study analyzed medical records, charts, and diaries of patients receiving fSCIG treatment for at least 1 year. The results showed that fSCIG is a feasible, well-tolerated treatment option with good infection control for both PID and SID patients.
