Effect of Body Composition on Outcomes after Hepatic Resection for Hepatocellular Carcinoma

Purpose. To evaluate the effect of body composition on outcomes after hepatic resection for patients with hepatocellular carcinoma (HCC). Methods. We performed 190 hepatic resections for HCC and divided the patients into 2 groups on the basis of visceral fat area (VFA), assessed by computed tomographic measurement at the level of the umbilicus, into high VFA (H-VFA) (n = 106) and low VFA (L-VFA) (n = 84) groups. We compared the surgical outcomes between the two groups. Results. L-VFA was significantly correlated with a lower body mass index, sarcopenia, lower serum albumin, and liver cirrhosis. There was no difference in the incidence of postoperative complications and mortality between the 2 groups. Patients in the L-VFA group had a significantly poorer prognosis than those in the H-VFA group in terms of both overall (P = 0.043) and recurrence-free (P = 0.001) survival. The results of multivariate analysis showed that sarcopenia rather than L-VFA was an independent and prognostic indicator after hepatic resection with HCC. Conclusions. Body composition is an important factor affecting cancer outcomes after hepatic resection for HCC in Japan.

Summary: The association between tumor microenvironment (TME) and cancer-associated fibroblasts (CAFs) in intrahepatic cholangiocarcinoma (ICC) progression is poorly understood. This study aimed to investigate the role of specific microRNAs (miRNAs) in extracellular vesicles (EVs) derived from CAFs in ICC progression. The results showed that miR-493-5p in CAF-derived EVs promoted tumor cell proliferation by suppressing CARTPT expression in ICC cells.

Summary: This study aimed to analyze the influence of the Global Anatomic Staging System (GLASS) and inframalleolar (IM) disease on the treatment outcomes of patients with chronic limb threatening ischaemia (CLTI) who undergo endovascular treatment (EVT). The study found that IM disease and GLASS stage were associated with major amputation, major adverse limb events (MALE), and wound healing, while GLASS stage was only associated with MALE.

Conservative treatment for residual lung congestion after left upper trisegmentectomy: a case report

Summary: This case report describes a case of improved congestion in the residual lung after left upper trisegmentectomy with conservative treatment under careful observation. The patient developed blood sputum after surgery, and CT scan showed consolidation and stenosis in the left lung. Conservative treatment with antibiotics and close follow-up by CT led to gradual improvement of pulmonary congestion and inflammation, and the patient was discharged home on day 26 after surgery.

Summary: Macrophages play crucial roles in AAA formation through inflammation and matrix degradation. IL-38, which has an anti-inflammatory effect, may suppress AAA formation by regulating macrophages. In this study, IL-38 treatment reduced M1 macrophage accumulation and MMP-2 and -9 expression in the AAA wall, leading to a decreased incidence of AAA. Additionally, p38 phosphorylation was found to be involved in the protective effects of IL-38 on regulating macrophages and reducing AAA incidence.

Summary: A combined therapy of atezolizumab and bevacizumab showed efficacy in treating an unresectable advanced-stage hepatocellular carcinoma (HCC) patient with lung metastases. Despite the side effects during treatment, the patient's tumor shrank significantly after multiple cycles, leading to a successful curative hepatic resection and no cancer recurrence.
