Prediction of Recurrence after Curative Resection of Hepatocellular Carcinoma using Liver Stiffness Measurement (FibroScanA®)

The purpose of this study was to investigate whether preoperative liver stiffness measurement (LSM) can predict recurrence after curative resection of hepatocellular carcinoma (HCC). LSM using FibroScan(A (R)) can assess the severity of liver fibrosis, which is significantly associated with recurrence after curative resection of HCC. Between February 2006 and March 2009, 133 patients who underwent preoperative LSM and curative resection for HCC were enrolled in this prospective study. LSM values were analyzed for association with recurrence, together with other clinical variables. The mean age of the patients (117 men and 16 women) was 57 years. During the follow-up period (median, 25.0 (range, 3.0-54.6) months), HCC recurred in 62 (46.6 %) patients. In multivariate analysis, together with satellite nodule and Edmonson-Steiner grade III-IV, LSM was selected as an independent predictor of recurrence (P < 0.05; hazard ratio, 1.034; 95 % confidence interval, 1.007-1.061). When the study population was stratified into two groups using the optimal cutoff value (13.4 kPa) that maximized the sum of sensitivity (64.7 %) and specificity (76.1 %) from time-dependent receiver operating characteristic curves (area under the receiver operating characteristic curve = 0.676), patients with LSM values > 13.4 kPa were at a significantly greater risk for recurrence with a hazard ratio of 1.925 (P = 0.01; 95 % confidence interval, 1.17-3.168) compared with those with LSM values a parts per thousand currency sign13.4 kPa. Our data suggest that LSM can be a useful predictor of recurrence after curative resection of HCC.

Risk Stratification for Sarcopenic Obesity in Subjects With Nonalcoholic Fatty Liver Disease

Summary: This study aimed to determine the relationship between sarcopenic obesity (SO) and the risk of significant liver fibrosis or cardiovascular disease (CVD) in nonalcoholic fatty liver disease (NAFLD). Through a multicenter retrospective study, it was found that high-risk subjects with SO had a higher risk of liver fibrosis and CVD development. The results demonstrated that high-risk subjects with SO had significantly increased risks of liver fibrosis and CVD development.

Summary: In patients with HBV-related cirrhosis, baseline and on-treatment ALT levels during ETV therapy are not associated with achieving subcirrhotic LS or the risk of HCC development. However, achieving subcirrhotic LS at 5 years of ETV therapy is independently associated with a lower risk of HCC development beyond 5 years of ETV therapy.

Suboptimal Performance of Hepatocellular Carcinoma Prediction Models in Patients with Hepatitis B Virus-Related Cirrhosis

Summary: This study evaluated the predictive performance of pre-existing well-validated hepatocellular carcinoma (HCC) prediction models in patients with HBV-related cirrhosis who started potent antiviral therapy (AVT). The results showed that the existing risk prediction models for patients with chronic hepatitis B had suboptimal predictive performances for assessing HCC development in patients with HBV-related cirrhosis. Although the models were not highly accurate, they were still able to stratify patients into low-, intermediate-, or high-risk groups.

Data from 14,603 patients with gastric cancer were analyzed, and the incidence of SMGC was found to be 4.04%, which was associated with male sex, age ≥65 years, and early gastric cancer. Unlike single gastric cancer, SMGC had a high survival rate.

Hepatic Steatosis but Not Fibrosis Is Independently Associated with Poor Outcomes in Patients with Inflammatory Bowel Disease

Summary: This study investigated the effects of nonalcoholic fatty liver disease (NAFLD) on the outcome of inflammatory bowel disease (IBD) patients. The results showed that the presence of NAFLD may increase the risk of clinical relapse in patients with IBD. Additionally, the study found that hepatic steatosis was significantly associated with increased risks of clinical relapse in patients with ulcerative colitis and Crohn's disease, while fibrotic burden in the liver was not.

SEPT9 Expression in Hepatic Nodules: An Immunohistochemical Study of Hepatocellular Neoplasm and Metastasis

Summary: The methylated SEPT9 DNA (mSEPT9) in plasma is an FDA-approved screening biomarker in colorectal cancer and is emerging as a promising diagnostic and prognostic biomarker in hepatocellular carcinoma (HCC). In this study, the SEPT9 protein expression in various hepatic tumors was evaluated using immunohistochemistry (IHC), and its correlation with demographics, risk factors, tumor characteristics, and oncologic outcomes was analyzed. The findings suggest that SEPT9 may be implicated in liver carcinogenesis and that SEPT9 staining by IHC could serve as an adjunct diagnostic biomarker with potential prognostic ramifications.

Self-Management and Associated Factors among Patients with Non-Alcoholic Fatty Liver Disease: A Cross-Sectional Study

Summary: Self-management is crucial for controlling non-alcoholic fatty liver disease. This study explores the characteristics and levels of self-management in patients with this disease in Korea. The findings indicate that self-management levels are moderate and self-efficacy is significantly associated with self-management. This research could assist healthcare professionals in promoting self-management compliance and developing interventions for sustainable self-management.
