However, patients who underwent mastectomy had worse tumor characteristics and higher risk of recurrence, especially without adjuvant radiotherapy. Long-term follow-up and enrollment of mastectomy patients are crucial.

Summary: This study aimed to evaluate the rate of positive non-sentinel lymph nodes in breast cancer patients after neoadjuvant systemic therapy following positive sentinel lymph node biopsy. Positive non-SLNs were found in 62.3% of cases and were significantly associated with older age, clinically positive lymph nodes, SLN extracapsular extension, and a higher ratio of positive SLNs/total SLNs. ECE and higher nodal ratio were independent predictors of III axillary level positivity.

Summary: This study aimed to investigate the risk factors of non-sentinel lymph node metastasis in breast cancer patients with 1-2 sentinel lymph node macrometastases undergoing total mastectomy. The results showed that tumor size and ratio of sentinel lymph node macrometastases were independent risk factors for non-sentinel lymph node metastasis. The incidence of non-sentinel lymph node metastasis could be reduced to 17.1% for patients with tumor size ≤ 22 mm and ratio of sentinel lymph node macrometastases ≤ 0.33.

Summary: Long-term follow-up data from multicenter phase III non-inferiority trials confirmed the safety of omitting axillary dissection in selected patients with clinically node-negative, sentinel node-positive breast cancer. Ongoing trials are investigating the extended eligibility of the Z0011 protocol in the adjuvant setting. De-escalation of axillary surgery in patients with clinically node-positive breast cancer is currently limited to the neoadjuvant setting.

The results showed that there were similar outcomes in overall survival and locoregional recurrence between the two groups, supporting the feasibility of de-escalating ALND to SLND in certain cases.

Summary: NRG/RTOG 9804 is the only randomized trial assessing the impact of whole breast irradiation versus observation in women with good-risk ductal carcinoma in situ (DCIS) following lumpectomy. The study found that whole breast irradiation significantly reduced the risk of ipsilateral breast recurrence for DCIS with durable effects at 15 years.
