Summary: This study aims to investigate the factors contributing to the severity of anastomotic leakage after low anterior resection for rectal cancer, create a severity score, and evaluate the effects of different treatment approaches on outcome parameters. The international multicentre retrospective cohort study, TENTACLE-Rectum, will provide valuable evidence-based recommendations for improving outcomes in patients with severe treatment-related morbidity.

Summary: Visceral fat was found to be associated with anastomotic leakage in elective colon cancer resections, but not in emergency surgeries. The previously reported risks of male sex and BMI for anastomotic leakage were explained by visceral fat. The effects of visceral fat on anastomotic healing were shown to be local rather than systemic.

Summary: This study evaluated the risk factors of anastomotic leakage (AL) in the surgical management of primary ovarian cancer and developed a nomogram to predict the risk of AL. The results showed that diabetes, co-operation with distal pancreatectomy, macroscopic residual tumor, and anastomotic level from the anal verge shorter than 10 cm were significant prognostic factors for AL. The nomogram can be used for preoperative counseling and intraoperative decision-making.

The results showed that pelvic inlet and intertuberous distance were independent predictors for postoperative anastomotic leak. The incorporation of pelvic dimensions with other factors in the nomogram may provide a clinical tool for predicting anastomotic leak.

Summary: This study aimed to investigate whether collagenase producing bacteria could be detected in fluid collected from chest tubes before the clinical presentation of anastomotic leakage after esophageal resection. A single-center study was conducted, and drain fluid was collected and cultured from patients who underwent resection of the gastroesophageal junction. The results showed no significant association between cultured fluid from the pleural cavity and anastomotic leakage.

The results showed that EVT had a longer treatment duration after nRCT, but the success rates were comparable. Importantly, no mortality was reported within 30 days and 90 days in both groups.

Summary: This study investigated the preferred treatment for anastomotic leakage after low anterior resection for rectal cancer by expert colorectal surgeons worldwide. The results showed that early leaks in septic patients were treated preferentially by major salvage surgery, while early leaks in non-septic patients were treated with drainage and fecal diversion. Late leaks had a wide range of treatment options, with minimal influence of symptoms on the proposed strategy. Treatment strategies for leaks of the blind loop and rectovaginal fistulae varied greatly.

Summary: In a population-based cohort study in Sweden, the authors found differences in incidence and survival between early-onset and later-onset esophagogastric adenocarcinoma. Male predominance was higher in early-onset cases, except for noncardia gastric cancer. Early-onset patients had more advanced stages and signet ring cell morphology. Incidence trends were similar between early-onset and later-onset cases, with an increase in esophageal adenocarcinoma, stable cardia cancer, and a decrease in noncardia gastric cancer. Early-onset patients had better survival, especially in localized stages and women.

Summary: This qualitative study aimed to explore the experience of family caregivers of patients treated for oesophageal cancer 2 years after treatment and to guide future interventions. The most significant stress factors for family caregivers were distress regarding patients' nutrition, fear of tumour recurrence, and worry about the future. Despite long-term patient survival, family caregivers continue to struggle with psychosocial consequences and there is a need to improve support interventions throughout the disease trajectory.

Summary: This study calculated years of life lost (YLL) due to the top 10 solid tumors causing the most deaths, using data from the Swedish National Cancer and Cause of Death Registers. The results showed that lung and colorectal cancer remained the leading causes of YLL, while pancreatic and breast cancer moved up in rank. Prostate cancer, on the other hand, moved down in rank. Women consistently lost more life years due to lung and pancreatic cancer, and the decline in colorectal cancer mortality was only reflected as a YLL decline in women. YLL is a simple and intuitive measure that expands our understanding of the cancer burden on society.

Summary: This study aimed to investigate whether laparoscopic gastrectomy (LG) had a positive impact on compliance with adjuvant chemotherapy compared with open gastrectomy (OG) in patients with gastric cancer. The results showed no significant difference between LG and OG in terms of initiation of adjuvant chemotherapy, modification of chemotherapy, and time interval from surgery to start of treatment. However, major complications had a significant negative impact on both outcomes.

Summary: This nationwide cohort study investigated the association between prediagnostic prescription antibiotics use and survival in colorectal cancer patients. The study found that overall, the use of antibiotics before diagnosis did not have a substantial detrimental effect on cancer-specific survival, except for very high use in stage I-III colorectal cancer. Different types of antibiotics may have different effects on survival outcomes. In stage IV colorectal cancer, an inverse relationship between antibiotics use and survival was observed. Further research is needed to confirm and understand these findings.

The results show that the use of oral antibiotics in colorectal surgery may reduce the risk of infection and alter the structure of the intestinal microbiome. However, further research is needed on the application methods of different oral antibiotics and the mechanism of the gut microbiome.
