Summary: As the obesity pandemic worsens, gastroenterologists will encounter more obese patients. They can play an important role in managing obesity by understanding the relationship between obesity and gastrointestinal conditions, as well as the interaction between obesity pathophysiology and gut hormones. Additionally, knowledge of post-surgical anatomy and medical/endoscopic management of bariatric surgical complications is essential due to the increasing number of patients undergoing bariatric surgery.

Summary: In a multicenter randomized trial conducted in the Netherlands, laparoscopic gastrectomy did not show a significant advantage over open gastrectomy in terms of hospital stay, postoperative complications, and oncological outcomes for Western patients with predominantly advanced gastric cancer undergoing multimodality treatment. Both procedures demonstrated similar results in postoperative complications and oncological efficacy.

Summary: The quality of end-of-life care for gynecologic cancer patients in Ontario has shown improvement over time, with fewer patients dying in hospitals and an increased utilization of supportive care. However, a significant proportion of patients still receive aggressive care in the final 30 days of life.

Summary: The majority of ovarian cancer patients in France are treated in low-volume hospitals, with only a minority having access to high-volume providers meeting quality indicator criteria. Efforts are needed to improve centralization and quality assurance in ovarian cancer management in the country.

Summary: This study assessed the quality of gastric cancer surgery using a photo-scoring method and implemented quality control measures during the LOGICA trial. The results showed that high radicality and nodal yield were indicators of good quality D2-gastrectomy. However, the photo-scoring method used in the study needs further improvement.

Summary: The study found that gastric cancer patients with limited peritoneal involvement may benefit from surgery with radical oncological intent, including extended lymphadenectomy and R0 resection. However, the retrospective nature of the study may introduce bias, highlighting the need for a prospective multicentric study to provide more answers.

Summary: The study identified 89 QIs from 22 selected documents, covering various aspects of breast cancer care. Most countries have not established national clinical pathways or integrated health care processes for breast cancer care, leading to heterogeneity in QIs for evaluating care quality.

Summary: The study found a strong association between achieving Textbook Outcome (TO) and improved long-term survival in gastric cancer patients based on the analysis of data from nearly two thousand patients. TO includes multiple aspects such as negative margins, number of lymph nodes sampled, complications, etc., and can serve as a comprehensive indicator for evaluating surgical quality.

Summary: We conducted a combined analysis of clinical trial data and the National Cancer Database to evaluate factors associated with failure to surgical resection in non-metastatic gastric cancer. The analysis revealed that effects related to neoadjuvant therapy and patient/provider characteristics were associated with surgical failure. This study highlights specific opportunities for improving surgical rates and outcomes.

Summary: This study aimed to identify areas of consensus among international experts on the use of fluorescence imaging with indocyanine green for mapping sentinel lymph nodes during gastric cancer surgery. Consensus was reached on various aspects, including the injection location and dosage of the fluorescent agent, as well as the advantages and limitations of the technique. However, further research is needed before this technology can be widely used in clinical practice.

Summary: This study found that the neutrophil-to-lymphocyte ratio (NLR) and CA19-9 were independent prognostic factors for gastric cancer patients. The NLR-CA19-9 score (NCS), which is a combination of the two factors, can provide more accurate predictions of the prognosis of gastric cancer patients, and its predictive value is significantly better than that of traditional inflammatory indicators or tumor markers.

Summary: For advanced gastric cancer patients undergoing palliative treatment, factors such as postoperative chemotherapy, better baseline QOL, improvement in postoperative QOL, and improvement in oral intake are associated with better survival rates. These factors were identified as prognostic factors in the study.

Summary: The study compared the clinical characteristics, survival, and recurrence patterns between MMRd and MMRi EEC. The results showed that MMRd tumors have more aggressive features, lower recurrence-free survival, and are more likely to recur in retroperitoneal lymph nodes. MLH1 methylated tumors had the worst prognostic features and survival outcomes.

Summary: The study developed an integrated targeted sequencing protocol called MultiMMR, which can test for promoter methylation, mutations, copy number alterations, copy neutral loss of heterozygosity, and microsatellite instability from a single aliquot of DNA. The study showed that MultiMMR can recapitulate clinical testing results, explain MMR loss in incomplete or inconclusive testing, and identify known variants. Promoter methylation and microsatellite instability analysis were highly concordant with clinical testing results.

Summary: The study found that the brief Family History Questionnaire can be used to identify women with ovarian cancer at risk of Lynch syndrome. Compared to the extended Family History Questionnaire, the brief Family History Questionnaire had higher sensitivity. A combined approach with both the brief Family History Questionnaire and immunohistochemistry correctly identified all patients with Lynch syndrome.

Summary: This study explores the views of health professionals on the utility of circulating tumor DNA (ctDNA) testing in the management of hereditary cancer syndromes (HCS). The study finds that professionals have varying opinions on the use of ctDNA testing, with some optimistic about its potential to transform early cancer detection, while others are hesitant due to concerns about invasiveness and limited utility.
