New perspectives for staging and prognosis in soft tissue sarcoma

Background: Data suggest that the current American Joint Committee on Cancer (AJCC) soft tissue sarcoma (STS) staging criteria merit further evaluation. We sought to identify and validate factors as enhanced descriptors of STS clinical behavior. Methods: Prospectively accrued data were analyzed for 1,091 AJCC stage I to III primary STS patients who had complete macroscopic resection at our institution from 1996 to 2007. Study factors were examined by univariable and multivariable analyses to identify independent prognostic factors for disease related mortality and overall survival (OS). Results: In contrast to the current AJCC STS staging system, which stratifies size into T1 (<= 5 cm) and T2 (> 5 cm) groups, we demonstrated three distinct cohorts (P < 0.0001): T1 (<= 5 cm; 5-year OS 85%), T2 (5 to 15 cm; OS 68%), and T3 (> 15 cm; OS 52%). A two-category system of histologic grade was demonstrably as informative as the current four histologic grade AJCC system. A multivariable Cox proportional hazard model identified tumor size (5 to 15 cm vs. <= 5 cm, P = 0.03; or > 15 cm vs. <= 5 cm; P < 0.0001), nonextremity primary site (P = 0.0016), disease of high histologic grade (P = 0.001), specific histology (P = 0.001), and margin positivity (P < 0.0001) as statistically significant adverse independent prognostic factors. Recurrence during follow-up was the most significant risk factor for STS-specific mortality (P < 0.0001). Conclusion: Tumor size and grade in the AJCC STS staging system need revision; moreover, primary site, histologic subtype, margin status, and recurrence offer additional relevant prognostic insight. Incorporation of these factors may enhance the AJCC staging system, thereby further facilitating individualized therapeutic strategies for STS patients.

Summary: Regional lymph node metastasis (RLNM) is uncommon in patients with soft tissue sarcoma (STS), although certain STS subtypes are more prone to RLNM. The impact of node-positive disease on patient survival is still controversial. There is ongoing debate about the benefits of sentinel lymph node biopsy (SLNB) in STS patients. Further multicenter prospective studies are needed to determine the true incidence and potential benefits of SLNB in patients with clinically occult RLNM.

Summary: This study aimed to evaluate the short-term outcomes and morbidity after re-resection of unplanned excision of trunk and extremity soft tissue sarcoma (STS) to better understand the risks and benefits of this treatment strategy. The results showed that the rate of wound complications after re-resection was 45%, and radiation therapy and plastic surgery involvement were independently associated with wound complications. Additionally, 67% of patients had no evidence of residual disease in the re-resection specimen. Therefore, treatment plans and discussions with patients should include information about the expected pathological findings and morbidity of surgery.

Summary: This study investigated the association between body mass index (BMI) and tumor or microbiome characteristics in patients with metastatic melanoma. The findings suggest that the host metabolic phenotype influences melanoma metabolism and may explain the improved outcomes observed in overweight/obese patients treated with immune checkpoint inhibitors and targeted therapies.

Superficial low-grade fibromyxoid sarcoma

Summary: This study reviewed cases of superficial LGFMS diagnosed between 2008 and 2020 and found that these tumors are more common in the extremities of children and young adults, and may have a better clinical outcome. All cases tested positive for MUC4 and/or FUS gene rearrangement. Follow-up showed no evidence of recurrence or distant metastasis in 14 cases.

Summary: By integrating histopathologic, proteogenomic, and metabolomic analyses, this study reveals the inter- and intratumoral heterogeneity of clear cell renal cell carcinomas (ccRCCs). It identifies high-risk disease subsets based on tumor grade, BAP1 mutation, genome instability, hypermethylation, and protein glycosylation signature. RNA sequencing provides insights into gene signatures and potential targeted therapies, while in vitro cell line studies validate the efficacy of inhibiting identified phosphoproteome targets.

Summary: Single-cell RNA-seq is an innovative technology to study complex tissues and characterize cell types, states, and lineages. However, recent evidence suggests that the methods used for tissue processing and cell disaggregation may introduce biases in gene expression and alter observed cell types. This study evaluated the impact of dissociation methods on sarcomas and demonstrated the feasibility of computational correction.

Oesophageal glomus tumours: rare neoplasms with aggressive clinical behaviour

Summary: This study describes the clinicopathologic features of three oesophageal glomus tumours diagnosed at two large academic institutions between 1984 and 2022. These tumours exhibited aggressive behavior with metastases to multiple sites. Management by a multidisciplinary sarcoma team is recommended for optimal outcome.

Small Non-Coding RNAs in Soft-Tissue Sarcomas: State of the Art and Future Directions

Summary: Soft-tissue sarcomas (STS), a rare and diverse group of tumors, have various subtypes characterized by differences in tumor biology and clinical outcomes. Small non-coding RNAs (sncRNAs), including miRNAs and tRNA-derived ncRNAs, play important roles in STS tumorigenesis and differentiation. However, the translation of these findings into clinical applications, such as biomarkers or therapeutic targets, remains challenging. This review summarizes the current understanding of sncRNAs in STS, with a focus on liposarcoma, and discusses their potential as novel biomarkers and therapeutic targets.

Three dimensional models of dedifferentiated liposarcoma cell lines: scaffold-based and scaffold-free approaches

Summary: This study aimed to establish 3D cell culture of liposarcomas using two different methods: scaffold-based (Matrigel extracellular matrix [ECM] scaffold method) and scaffold-free (Ultra-low attachment [ULA] plate). The distinctive differences in structures and different responses of cell lines to 3D culture were observed. The upregulation of MDM2 was confirmed after treatment with MDM2 inhibitor SAR405838, and DDLPS spheroids showed different sensitivity patterns from 2D models. The 3D models have the potential to provide a new predictability of efficacy and toxicity, and are considered as an important process in the pre-clinical phase.

Summary: Patients with BRAF-mutant melanoma respond well to combined BRAF and MEK inhibition, but relapse is common within 2 years. Minimal residual disease (MRD), which consists of drug-tolerant tumor cells in a dormant state, is a major cause of drug resistance. By studying a mouse model, researchers found that sustaining chemokine signaling can impair MRD maintenance by increasing the recruitment of effector T cells.

Morbidity and Mortality after Surgery for Retroperitoneal Sarcoma

Summary: Retroperitoneal sarcoma (RPS) is a rare disease with various histologic types. The Transatlantic RPS Working Group (TARPSWG) in Europe and North America aims to study and establish best practices for managing this disease. Complete resection of all macro and microscopic disease is the gold standard, but it often requires multi-visceral resection. Recent evidence suggests en bloc compartmental resections can reduce local recurrence, but the potential for added morbidity must be considered.
