Summary: The study aimed to evaluate the impact of rifaximin on liver function, regeneration, and volumetric recovery in patients undergoing major hepatectomy, but it was prematurely terminated due to the unlikelihood of reaching a significant primary endpoint. Rifaximin did not show significant effects on postoperative liver function, volume regeneration, or surgical complications.

Summary: This study evaluated the association between future liver remnant volume (FLRV) and post-hepatectomy liver failure (PHLF). The results showed a significant decrease in FLRV% in PHLF patients, with over half of them having FLRV% < 30%. Therefore, the authors recommend risk stratification and additional CT volumetry for high-risk patients prior to major hepatectomy.

Summary: The aim of this study was to evaluate the efficacy of donor lifestyle optimization on liver regeneration and outcome following live donor liver transplantation. The results showed that donors who received a customized low-calorie diet and exercise had better outcomes in terms of postoperative biomarkers and liver regeneration, as well as fewer instances of early graft dysfunction (EGD) in recipients.

Summary: This study analyzed the outcomes of major hepatectomies in a tertiary academic setting and emphasized the impact of training and obesity on surgical outcomes. The study found that the achievement of textbook outcomes varied among different types of surgeries and surgical experience levels. Additionally, obese patients and those undergoing more complex surgeries were at a higher risk of poor postoperative outcomes.

Summary: This retrospective cohort study aimed to compare postoperative outcomes in major liver surgery patients with and without thoracic epidural anesthesia. The study found that thoracic epidural anesthesia did not reduce postoperative hospital length of stay but resulted in lower perioperative analgesia doses. Thoracic epidural anesthesia was safe in this cohort of patients undergoing major liver surgery.

Summary: This study aimed to explore potential sex differences in outcomes and regenerative parameters after major hepatectomies. The results showed that males had poorer outcomes in terms of complications and liver dysfunction, while females had shorter interstage intervals, although not in postmenopausal subgroups. Mouse experiments further demonstrated the role of estrogen in these sex disparities. These findings suggest the potential benefits of estrogen in postmenopausal women and perhaps men undergoing major liver surgery.

Summary: In this study, the researchers investigated the mechanisms of liver regeneration and found that Vegf, endocan, endostatin, and eEF2K play important roles in the process of liver regeneration after partial hepatectomy. Specifically, endocan may serve as a useful biomarker for clinical prognosis.

Summary: This study investigated the role of ubiquitin-specific peptidase 47 (USP47) in liver regeneration. The results showed that USP47 expression was up-regulated in hepatocytes isolated from mice subjected to partial hepatectomy or exposed to HGF treatment. E2F1 was identified as the primary regulator of USP47 transcription. USP47 was found to play an essential role in liver regeneration, as its inhibition impaired liver regeneration in mice and its over-expression enhanced hepatocyte proliferation in vitro and promoted liver regeneration in mice. A positive correlation between USP47 expression and hepatocyte proliferation was also observed in patients with acute liver failure.

Summary: The liver is a complex organ with vital functions and regenerative capacity after tissue injury. Mechanical cues play a significant role in liver regeneration, including changes in hemodynamics and mechanical forces within the liver. Understanding the mechanotransductive pathways and mechanocrine responses can contribute to a comprehensive understanding of liver regeneration. Proper adjustment of mechanical loading may serve as an effective therapy for liver injury and diseases.

Summary: This study investigates the expression of cytokines and growth factors in post-hepatectomy regeneration and finds that certain expression trajectories are associated with liver failure, morbidity, and mortality. Regression models can predict mortality following major liver resection based on preoperative cytokine and growth factor signatures. Liquid-biopsy based risk profiling can predict postoperative liver failure and mortality on the first day after surgery.

Summary: This study investigated liver transplantations performed in France for major bile duct injuries (BDI) following cholecystectomy. The findings show that iatrogenic BDI remains a serious concern, often requiring treatment in expert centers. The post-transplant morbidity rate is high and the results are not optimal. Therefore, early referral to expert centers is necessary for all major BDI patients to prevent severe outcomes.

Summary: The COVID-19 pandemic has had a significant impact on the diagnosis and treatment of pancreatic adenocarcinoma. During the lockdown, there was a decrease in new cases and an increase in borderline tumors, while the rate of metastatic diseases decreased. There was no difference in time-to-diagnosis and time-to-treatment. Neoadjuvant chemotherapy was favored over upfront surgery, and the use of a specific preoperative chemotherapy regimen decreased. After the lockdown, there was a decrease in borderline tumors and an increase in advanced diseases.

Summary: This study reported the incidence of delayed gastric emptying (DGE) following elective distal pancreatectomy (DP) and identified age, surgical approach, and postoperative pancreatic fistula (POPF) grade B-C as predisposing factors for DGE. DGE significantly increased the length of hospital stay. The study suggests that laparoscopic approach and prevention of POPF should be encouraged to reduce the incidence of DGE.

Summary: This study investigated the prognostic factors of non-cirrhotic hepatocellular carcinoma (HCC) and developed a prognostication algorithm. The results showed that microvascular invasion, HCC differentiation, tumor number, and size were important factors affecting overall survival and recurrence-free survival of patients. Stratification based on recurrence-free survival provided an algorithm based on tumor size and number, which could be used as a reference for treatment decision-making during the perioperative period.
