Summary: This study retrospectively analyzed patients who underwent Hartmann's procedure for acute diverticulitis between 2006 and 2017, with follow-up until March 2021. The study found that one-third of the patients died, half underwent stoma reversal, and one-fifth did not undergo stoma reversal. Risk factors for non-reversal included old age, a need for outside assistance, low Helsinki Staging for Acute Diverticulitis stage, and higher C-reactive protein level upon hospital admission.

Summary: This study evaluated the trends and factors associated with Hartmann's procedure (HP) and primary anastomosis (PA) in emergency surgery for diverticulitis over a 25-year period. Despite the demonstrated safety of PA, HP remained the most performed procedure in the emergency setting for diverticulitis. Older age, complicated diverticulitis, and greater severity of illness were associated with increased odds of undergoing HP.

Summary: This study evaluated the factors associated with rectal stenosis after anterior rectal resection (ARR) in rectal cancer patients. The results showed that both surgery and radiotherapy were risk factors for rectal stenosis. In particular, a protective ileostomy was found to be a critical factor associated with rectal stenosis compared to preoperative radiotherapy.

Summary: This study investigated the relationship between the timing of protective ileostomy closure and post-closure complication rates. It found no significant association between the duration of ileostomy closure and the rate of complications. While intestinal obstruction was more common in the late closure group, early closure group had a higher incidence of surgical site infections.

Summary: This study aimed to compare the long-term outcomes of Hartmann's Procedure (HP) with sigmoidectomy with primary anastomosis (PA) in the treatment of perforated diverticulitis. The results showed that PA had a higher stoma-free rate, lower overall hospitalization days, and fewer parastomal hernias compared to HP.

Summary: This study found that patients with prior hospital admissions for complicated diverticulitis had a higher mortality rate. Additionally, patients who had a previous percutaneous drain placement during hospitalization had a lower mortality rate. By identifying patients at high risk for readmission and emergency surgery, perioperative outcomes can be improved.

Summary: The article presents a case of a 54-year-old female patient with stage 3C ovarian cancer who underwent primary debulking surgery (PDS) with a mesorectal-sparing technique for colorectal resection. This technique may be a viable option for selected cases of ovarian cancer, maximizing blood supply to the colorectal anastomosis while reducing the risk of complications.

Summary: The study found that extrasphincteric APR has higher overall and pelvic-perineal complications compared to HP. However, HP has a significantly higher incidence of isolated pelvic abscess formation. Subgroup analysis did not show significant differences in overall complications, pelvic-perineal complications, or pelvic abscess formation between intersphincteric APR and HP.

Summary: This qualitative study investigated the decision-making process of surgeons regarding protective ileostomy creation after rectal cancer surgery. The results showed that 76% of surgeons supported the creation of ileostomy. When making the decision, 88% of surgeons considered risk factors. Emotions influenced the decision-making process in 22% of cases, and personal experiences influenced it in 49% of cases. The most frequently considered risk factors were the distance between the anus and the tumor (96%), neoadjuvant chemoradiotherapy (88%), positive intraoperative leak test (65%), blood loss (37%), and immunosuppression therapy (35%).

Summary: Pancreatic cancer often presents as locally advanced or borderline resectable. It is currently unclear which chemotherapy is preferred for patients in these categories. This study found that initial treatment with FOLFIRINOX appears to provide a survival benefit for patients that are ultimately unresectable, compared to gemcitabine-based chemotherapy.

Summary: Despite recent therapeutical advances, gastric adenocarcinoma remains associated with a poor long-term survival. Diagnosis is often made at advanced stages, affecting prognosis. Factors such as tumor microenvironment, patient ethnicity, and therapeutic strategy play a role in patient outcome. This study aims to review existing knowledge on prognostic factors in gastric adenocarcinoma.

Summary: This study analyzed HIPEC trials registered on ClinicalTrials.gov to identify their focus and assess publication of trial results. Among 234 identified trials, 11% were published and 6% reported results but not as full papers. Most trials were recruiting patients (36%) or completed but unpublished (20%). The majority of trials focused on treatment strategies (68%) and oncological outcomes (53%). The most studied neoplasms were peritoneally metastasized colorectal cancer, gastric cancer, and ovarian cancer.

Summary: Pancreatic cancer is a deadly disease with unknown origins and limited treatment options. Surgical resection, the primary curative treatment, is associated with high morbidity and mortality rates and frequent recurrence. Patient-centered care and understanding patient perspectives are crucial for improving outcomes in pancreatic cancer surgery. Standardized patient-reported outcome and experience measures are needed to assess patient perspectives accurately.

Summary: In pheochromocytomas and paragangliomas, hypoxia upregulates the expression of PD-L1 and HIF-2 alpha, promoting tumor progression and immune microenvironment alterations. Head and neck paragangliomas undergoing embolization show significantly higher expression levels of PD-L1 and HIF-2 alpha, suggesting that targeted treatment using HIF-2 alpha and checkpoint inhibitors may be beneficial.

Summary: This study evaluated therapy strategies for symptomatic abdominal haemophilic pseudotumours and found that primary surgical resection can be considered as the first-line treatment, with preoperative embolisation as a bridging therapy before surgery. Diagnostic biopsy and percutaneous drainage should be avoided to prevent complications.
