Summary: Biliary leakage is relatively common following hepatectomy with hepaticojejunostomy for liver malignancy (33.4%), but most patients (90.4%) do not require re-exploration. Intraoperatively placed drains successfully controlled 46.7% of bile leakages. IR-guided drain placement had a 88.7% success rate for adequate leak control.

Summary: The study aimed to identify risk factors for biliary leakage (BL) after liver surgery. The results showed that BL occurrence was associated with bilio-digestive anastomosis, neoadjuvant chemotherapy, body mass index, anatomical resection, and blood loss. Identifying high-risk patients can guide the positioning of drainage after surgery.

Summary: This study investigated the impact of intraoperative bile leakage (IOBL) during laparoscopic liver resection (LLR) on postoperative outcomes, finding that IOBL led to a higher incidence of postoperative bile leakage and identified predictors such as age, diabetes mellitus, indocyanine green retention rate, and the Glissonean approach. IOBL during LLR must be promptly identified and appropriately managed based on these predictive factors.

Summary: This study assessed the feasibility of a new polyurethane-based sealant patch (PBSP) for hepato-pancreato-biliary (HPB) surgery and compared it with commercially available products. The results showed that PBSP had strong adherence to liver tissue, induced constant hemostasis, and prevented postoperative pancreatic fistulas and bile leakage. Promising preclinical data suggest the need for further evaluation through clinical trials.

Summary: The study aimed to assess the safety of using fibrin sealant on the CBD incision. Although there were no significant differences in operative time and bile flow, the study suggests that incision closure may be an option to reduce the risk of leakage. Further studies are needed to confirm this finding.

Summary: This meta-analysis aimed to analyze the risk factors for postoperative bile leakage (POBL) after hepatectomy. The study identified several factors, including gender, partial hepatectomy, repeat of hepatectomy, extended hepatectomy, abdominal drain, diabetes, Child=B, solitary tumor, and chemotherapy, as risk factors for POBL. Meanwhile, cirrhosis, benign diseases, left hepatectomy, and Segment 1 resection were not significant risk factors for POBL.

Summary: A new high throughput method for generating fibrin nanoparticles using probe sonication was reported in this study, which can promote clot formation and cell migration. When combined with fibrinogen, sonicated fibrin nanoparticles can form a fibrin gel and support cell migration, which is relevant for wound healing. The results suggest that sonicated fibrin nanoparticles may be a promising therapy for wound healing that is easy to produce and deliver in a flowable formulation.

Summary: The purpose of this study was to assess the specific risk factors for morbidity of hepatic hydatid cyst after conservative surgery. A retrospective study was conducted on 102 patients over a 13-year period. The results showed that preoperative hydatid cyst infection, compressive cysts, and preoperative fever and jaundice could be predictor factors of morbidity after conservative surgery for liver hydatid cyst.

Summary: A machine learning-based radiomics model was able to predict the occurrence of colorectal liver metastases in patients with colorectal cancer and provide valuable biomarkers for identifying high-risk patients. The predictive models including radiomics features, clinical features, and a combination of both showed promising results with AUC values of 86%, 71%, and 86% in the validation cohort, respectively.

Summary: Treatment options for obstructing colon cancer are controversial, with emergency resection resulting in higher mortality and complication rates compared to staged treatment for potentially curable right-sided OCC. Staged treatment appears to have lower mortality rates, fewer complications, and fewer anastomotic leaks and stomas created compared to emergency resection in these patients.

Summary: In this study, radiomics was used to distinguish gastrointestinal stromal tumors (GISTs) from other intra-abdominal tumors and predict the mutational status and mitotic index of GISTs. The results showed that the radiomics model performed similarly to three radiologists in distinguishing GISTs from non-GISTs, but it was less observer dependent. However, the model was unable to predict any genetic or molecular features, limiting its use in treatment planning.

Summary: Desmoid-type fibromatosis (DTF) is a rare soft-tissue tumor that can negatively impact health-related quality of life (HRQoL). A specific HRQoL tool, the DTF-QoL, was previously developed for assessing DTF-specific issues. This study pre-tested the DTF-QoL and selected 96 questions to form the final questionnaire, which can be used to evaluate HRQoL of DTF patients in clinical and research settings, aiming to personalize care and improve patient experience.

Summary: This study assessed the potential and limitations of near-infrared fluorescence imaging with indocyanine green in intraoperative tumor identification of GISTs. The results showed that GISTs typically have similar fluorescence intensity to the surrounding tissue, indicating that intraoperatively administered indocyanine green is currently not suitable for adequate tumor identification.
